    Abstract

    Clinical procedure

    Due to imprecise bite registration, but also

    because we have to obtain a passive-f itprovisional restoration , the authors decided not

    to f ix any of the pr osthetic caps in the

    provisional, thus having the option of fixing the

    whole arch more vestibulary or to the palatal

    aspect, as requested by the function or

    aesthetics.

    It is also easy to notice a better distribution of

    the implants on the maxillary arch.

    After mounting the prosthetic caps in themouth of the patient, t he fixation of the

    provisional restoration with Qu-resin allows us

    to obtain a passive-fit restoration

    Conclusion

    Exposure of the bone after the continuousincision and extraction of the remaining mobile

    teeth is followed by the drilling of 6 implantneoalveollae; The 2nd implant is placed from the

    canine pos ition, tilted to the apex of the lat eral

    incisor; Usually is necesary to use 35 abutment

    for SKY fast&fixed (R) in order to achieve the

    paralellism of the oclusal sc rews.

    The final position of the antirotational system, the

    Torx (R) is se t after the probation of the oclusal

    screws orientation. This can be done both with thenew handle of the SKY fast&fixed (R) abutments

    or with the copings for impression.

    After the fixation of the abutments with 25 Ncm,

    the impression copings are set in the position

    before suture.

    A good and detailed impression of the situation

    can be achieved with the brec ision implant lightand brecision implant heavy; both A-s ilicones

    can be easily disinfected and are transmitting to

    the lab the informatio ns needed to fabricate an

    aesthetic and functional provisional restoration.

    Unlike poliether materials, the A-silicones are

    more elastic and are good registering details.

    With the SKY fast&fixed therapy concept the implantologyst is looking forward for the

    optimum use of the existing bone. The classical concept was recommend ing to placethe anterior implants in the positions of central and lateral incisors. The authors have

    considered that the distance between the lateral incisor and the 2nd premolar is toolarge; inserting a straight implant in the canine position would increase the risk to touch

    the distal implants; the ideea was then to use the existing bone in the region of the

    canine, but also from the lateral incisor; This was the point when occured us the ideea

    to tilt also the canine implants;

    With the components of the SKY fast&fixed sistem, the implementation of this ideea

    was very easy: we have used 35 degrees abutments for the implants in th e canine

    position. Ocasionaly and depending of the anathomy of the patient maxilla,

    17,5 degrees abutment can be a solution also for paralelising the oclusal screws.

    The benefits of the new implant distribution are smaller stress on the provisional

    restoration and better distribution of loading of the bone.

    SKY fast&fixed is an open concept, the freedom of the clinician been given by themultiple prosthetic possibilities of the SKY Implant System. The use of the native

    bone , with a single operation fulfills most of the times the wishes of the patient.

    Inserting the second i mplant tilted, in the place of the canine, is allowing the clinician

    to use in a proper maner the existing bone, and also to improve the stability of the

    implants, by using longer implants, better distributed forces, reduce the stress on the

    pontic of the implant bridge.
