All injury accidents or incidents to be reported to a Supervisor immediately

They should be recorded in the accident book on the same day if possible

Returning To Work

After absence or sickness you will be required to sit down with your line manager to complete a return to work interview

This is to ensure you are fit and well enough to return to work without endangering yourself or others

Remember if you have sickness or diarrhoea, you must be clear and well for two days prior to working with water again

Actions in an Emergency

In an emergency dial 999 and ask for the required emergency service

Tell the emergency service the nature of the emergency and the address where you need them to respond to

Immediately after telephone your line manager

Directions to nearest A&E shall be in every van, canteen and site office

In the event of any emergency or incident on site, all employees and visitors must assemble at the nearest Assembly Point and wait for further instructions from the H&S Advisor, Site Agent or his/her deputy

Ensure where work is undertaken in areas of poor phone signal, alternative emergency arrangements are made

Reporting of Incidents

A recent incident occurred where a gang damaged a gas service. The gang did not report the incident and made a

makeshift repair. The consequences of this incident could have been disastrous.

Under no circumstances will the non reporting of incidents be tolerated

Repairs to damaged services can only be undertaken by an approved contractor on behalf of the utility owner

Clancy Docwra actively promotes a culture of reporting incidents and near misses, and see the reporting as a

positive act

Clancy Docwra will undertake various inspections and audits which may include:

Senior Managers attending site to engage with staff and to review work activity – please be open with them

Inspections: By site manager to check compliance to method

statement and risk assessments By other appointed persons to check excavations,

scaffolds, plant, equipment By Safety Advisor to check standards are being

achieved and maintained

BMS Inspections to ensure the Health, Safety Environment and Quality Management System is being followed

Non-Conformance (NCR)The NCR is a management tool used to record when things don’t go to plan. It triggers a root cause analysis and when analysed

with others can indicate trends

The aim of an NCR is to find the part of the system at fault (not a person)

area of work?

Manual Handling

make sure the route is clear of obstructions there is somewhere to put the load down wherever it is to be moved to

stand as close to the load as possible, and spread your feet to shoulder width, bend your knees and try and keep the back's natural, upright posture

grasp the load firmly as close to the body as you can and use the legs to lift the load in a smooth motion. This offers more leverage & reduces strain on your back

carry the load close to the body with the elbows tucked into the body and avoid twisting whilst carrying the load

ensure that the object is light enough to lift (as per manual handling assessment), is stable and unlikely to shift or move when being carried

heavy or awkward loads should be moved using a mechanical aid

Traffic Management Plan

A safe site not only promotes safety but reflects the image of Clancy Docwra

Please add and make specific according to your site plans

A Traffic Management Plan has been developed for this site

No Chapter 8 to be moved without approval by the Site Manager and will only be carried out by a NRSWA certified person

All deliveries are to be made via a designated works access point and must be controlled by competent traffic marshal

All deliveries should be planned and controlled

Other small vehicles entering / leaving site MUST be controlled by competent person.

Cars MUST be parked within designated area

Plant Operation

Only trained and authorised personnel can operate driver operated plant

This may include but not be limited to; Dumper Trucks, Cherry Pickers, Scissor Platforms, Excavators, Road Rollers, etc.

All plant operators will be required to produce evidence of their training before being given authorisation to operate any given item of plant. This may include the production of a CPCS or IPAF card or suitable equivalent.

Avoid reversing on site or use trained banksmen and ensure that loads are properly slung/secured.

Don’t take passengers

Ensure that where vehicles are provided with seat-belts they are used properly

Plant Operators should be aware of the site requirements/hazards e.g. traffic route, dangerous slopes, high voltage cables

Ensure item is within calibration/servicing date

Check daily for leaks

Report any leaking or defective plant & equipment

Turn off when not in use, remove ignition key & secure vehicle.

Get it maintained if it is ‘smokey’

Use drip trays under plant e.g. pumps and generators

Maintenance should be completed at permanent yards only

Electrical Safety“Do not use” unsafe or damaged equipment

110v Electrical Equipment used out on site must have a PAT test label indicating that tests done is within a 3 month period.

Before cleaning, adjusting, repairing, or removing a guard on any electrical equipment, always switch off and isolate. (to be carried out by a certified and competent person)

Always replace a fuse element with one of the correct type and rating. If a fuse, or thermal cut out, continually operates, get expert advice

240v electrical equipment must not be used on site

All equipment to be checked by the end user prior use for defects and to ensure all equipment are safe.

Any extension cables used on site will be placed safe by position to prevent a trip hazard

Excavations

Obtain underground utility plans, but don’t rely on them as they can be incorrect and misleading

Carry out a Full CAT & Genny Survey & repeat as excavation progresses

Look for visual indications of underground utilities

Mark all utilities located (inside and outside the area to be excavated)

Treat all services encountered as live

No mechanical means of excavation within 500mm of any existing services

Spades and shovels are safer than picks, pins or forks

Excavation must be carried as per Risk Assessment / Method Statement

Report all underground utility damages to your supervisor immediately

All excavations 1.2m in depth and deeper will require trench supports

Asbestos and Suspicious Materials

If you are unsure of the look of something or suspect that it is dangerous report it to your Supervisor – do not touch!

Disturbing asbestos causes fibres to be released into the air.

Described by many sufferers as being like “your lungs being slowly filled with wet concrete”

Assume that all cables passing through floor slabs are fire stopped with Asbestos material

If in doubt – seek advice immediately

Control of Substances Hazardous to Health

COSHH

All COSHH substances must have clear and concise instructions on usage and protection from any associated hazards

Always follow COSHH assessment instructions for protection COSHH products must be signed out of the stores on the COSHH Register You must sign for appropriate PPE at the same time or confirm that you already

have the relevant PPE as per COSHH assessment Never play with or abuse any chemical substance Place empty chemical containers in the hazardous waste bins provided. Never

dispose of them in skips Keep oil and fuel cans on drip trays at all times Wash thoroughly after contact with a chemical, including cement or concrete

SharpsAction to be taken if needles or syringes are found

Contact your Health and Safety Advisor immediately. Protective clothing in the form of overall and PVC gloves must

be worn A “Sharps Pack” should be available and if not the work area

should be closed until it is made available. Using the disposable tweezers, place any (or all) needles into

the sharps container When you are satisfied all discarded needles/syringes have

been dealt with, place the tweezers into the container and seal the lid

If needles/syringes are found to be embedded in seals/gaskets etc, these can be remove by using pliers and placed into the Sharps Box

The pliers must be sterilized by passing them through a naked flame. Three slow passes through the flame with the jaws of the pliers open will be sufficient.

Working Over Water

Warning signs/notices (deep water) must be displayed

A fence or rigid barrier must be provided where there is a risk of people falling into water where possible. If a fence or barrier cannot be provided all personnel must wear a buoyancy aid

A buoyancy aid, must be worn by all personnel working over water

Suitable rescue equipment, for example a boat, boathook, or lifeline must be in position when people are working over water

All personnel are to be instructed as to the means of raising the alarm and rescue drills

Hygiene

When working with water pipes, ensure you have adequate supply of diluted chlorus solution to do each job, at the start of every day

Kept in a suitable and clearly labelled container

Now you have it, you must use it!

Ensure pipes, fittings and tools are sprayed and free from germs prior to fitting

Remember germs passed into the water supply are passed onto the users. Which could include YOU and YOUR family

Clancy Docwra expect the highest standards of behaviour and compliance against site rules, risk assessments, method statements and the safe use of plant and equipment, all of which have been expressed clearly in this induction

Any person found to be in breach of these standards may be immediately removed from this Clancy Docwra site and disciplinary issues formally escalated to the persons employer

Drugs and Alcohol

Clancy Docwra has a comprehensive Drugs and Alcohol Policy and adopts a zero tolerance approach

Mandatory Drugs an Alcohol testing will be carried out on all employees, and a random testing program is in place

Any persons caught in possession of or under the influence of drugs or alcohol will be removed from site

If you take prescription drugs for any medical reason, please inform your supervisor at once
