1.0 GENERAL EVACUATION INSTRUCTIONS - RAISING THE ALARM An emergency Incident is an event that arises internally, or from external sources, which may adversely affect the occupants or visitors in a facility, & which requires an immediate response. • If you see SMOKE, FLAMES or hear the fire ALARM alert one of the occupants

immediately • If safe, close any windows and doors to confine the fire • Follow the signs to locate and leave through the nearest emergency exit and

proceed to one of the assembly points. • Telephone 000 and notify the Fire Service • Calmly follow instructions given by staff or the attending Fire Officers. The fire

warden will account for all occupants and report persons missing to the Fire Officers. Do not re-enter the building until you are told it is safe to do so by the Chief Warden or Fire Officers.

Emergency response is the action/s taken at the site of a physical incident to preserve life, the environment and assets. It includes the actions of Company, Municipal Emergency Services and other authorities or agencies. Levels of Response

1. Local alert: An emergency incident that relates to one area of the site with an immediate response required by the most senior person in that area (usually the supervisor or area warden).

2. Site Alert: This level of alert requires a site evacuation with immediate response from the Emergency Response Team (ERT).

3. External alert: A site alert with assistance and notification to emergency services and other regulatory authorities as required.

Whenever an emergency incident occurs, it is essential that procedures and organisational arrangements are adopted for the orderly movement of occupants out

of the area of risk to an area of safety. A summary of Initial Response procedures is attached at Appendix A and is displayed in every company building with an emergency map. 1.1 Introductory Matter - Business Unit Description The business unit comprises three sites:

1. Site 29 – Office & general storage 2. Site 33 – Bituminous Plant & Dangerous Goods storage 3. Site 35 – Paint manufacturing plant & Dangerous Goods Storage

Products manufactured on site are for the road, building and quarry industries such as:

• Road sealing & rejuvenating products • Pavement surface treatments • Waterproofing & membranes • Heavy duty paints & sealants

Activities on site include:

• Manufacture of spray sealing products such as Polymer Modified Bitumen binders (PMB).

• Manufacture of pre-coat, cutbacks, crack sealants and adhesion agent

products.

• Manufacture of paints and sealing products (site 35) including toll blending.

• Dangerous Goods Storage and distribution of paints, solvents, sealants and adhesion agents.

• Laboratory

Site 33 & site 35 is separated by a stormwater canal. A hazmat box is located at the front of both sites 33 & 35. An asbestos register is located beside the visitor’s book located at site 29. Approximately 15 persons usually work on site during normal business hours which are from 8:00am to 5:00pm.

2.0 PLAN PURPOSE The Site Emergency Response Plan (SERP) is an outline of the procedures to be followed by all workers and visitors at this Business Unit in the event of an emergency incident which include: • Control or limit the effect of a range of potential emergency incidents that may

occur within the site or off site; • Ensure safe, efficient and orderly evacuation of all persons if deemed

necessary.

• Ensure the safety and well-being of all personnel on the premises at the time of an emergency incident which will include assigning responsibilities for specific actions;

• Outline the procedures to be followed including communication protocols to

ensure all vital information is communicated to relevant stakeholders as soon as possible;

• Contain and control an emergency incident, so as to minimise the effects on

people, property and the built and natural environment so that normal operations can be resumed;

• Define & record emergency training requirements and emergency plan review.

3.0 EMERENCY CONTROL 3.1 Emergency Controller The Emergency Controller is the leader of the Site Emergency Response Team and shall take charge of all emergency situations and is responsible for coordinating the safe response and evacuation in an emergency situation. If the Emergency Controller is not on site the Area Warden (from the ERT) will assume the role of the Emergency Controller. ACTIONS ON during Emergency

• Initial assessment - contain the hazard only if safe to do so; • Advise Area Wardens of location and nature of emergency; • Ensure all persons are accounted for - if not, notify Emergency Services; • If necessary initiate call outs to Emergency Services advising the following:

• Location • Type of emergency • Any casualties • What assistance is required; • Any hazards • Your name & telephone number

• Travel to incident site if possible and re-assess; • Rescue or assist injured persons only if safe to do so; • As the details become clear, liaise with the Technical Supervisor (if available)

the best course of action to manage the emergency; • Contact appropriate trained personnel in preparation of isolation of energy

sources - emergency external contact list; • Collect list of casualties, equipment damage and status of the ongoing

response and additional information; • Maintain regular updates to Business Unit Manager; • For each action on the log, contact the relevant person from the call-up lists,

advise them of the circumstances and request them to take the appropriate actions. Delegate the action required where practicable;

• If a fatality or serious injury is involved, secure area and leave undisturbed Restrict entry to the site from media in a polite but firm manner and, refer all media related enquiries to the Business Unit Manager;

The duties and responsibilities of the Emergency Controller include, but are not limited to: • Take control of emergency situation and liaise with Emergency Services; • Being familiar with the buildings on site and emergency exits; • Ensuring the Business Unit SERP is current and effective; • Making the key decisions in response to an emergency; • During an emergency, assume control and direct operations until appropriate

authorities arrive; • Ensuring Emergency Services know how quickly to get on site; • Ensuring all appointed persons are competent in their specific role; • Conducting regular training exercises – readiness of all emergencies, live

evacuation drills, fire-fighting equipment, spill kit use, environmental awareness, new workers trained in SERP and their specific duties, at a minimum frequency in line with the requirements of relevant regulatory authorities;

• Ensuring the evacuation alarm is activated and liaise with Area Wardens; • Coordinating the provision of adequate refreshments for personnel involved if

emergency continues for any length of time; • Having detailed knowledge of available fire-fighting equipment and their

location; • Assisting in the investigation into the emergency if required; and, • Composing a detailed summary of the incident and submit to the Business

Unit Manager as soon as possible. • Complete BP COR F 08 07 Emergency Response Action Log;

Post Incident • Authorise re-occupation of the buildings on site once it is safe to do so; • Conduct debrief at suitable on-site or off-site location, at the completion of the

emergency; • When the situation has stabilised, conduct an investigation into the

emergency response and prepare a report to the Business Unit Manager; • Ensure energy sources have been re-instated, once area is declared safe;

and,

3.2 Area Wardens Area Wardens are appointed by the Business Unit Manager. ACTIONS ON sounding of Evacuation Alarm • Activate evacuation alarm; • Direct personnel in their area to the exits and proceed to evacuate; • Carry out inspection of their area ensuring toilets, lunchrooms, etc. are

cleared; • Ensure all personnel in their area are accounted for; • Notify Emergency Controller that their area is all clear; • If all personnel are not accounted for, advise the Emergency Controller and

await further instructions; • Ensure persons do not re-enter the building, until advised by the Emergency

Controller it is safe to do so; and, • Attend debrief at the completion of the emergency at the nominated location. The duties and responsibilities of Area Wardens include, but are not limited to: • Take charge of all operations until the Emergency Controller arrives to direct

operations; • Be familiar with the area of responsibility; • Be aware of the location of manual evacuation alarm points, firefighting

equipment and obscure areas where people may be located; • Liaise with the Emergency Controller; and, • Commence immediate evacuation of the area if the circumstances warrant it. After Hours Emergency – Night Shift Personnel • Assess and direct operations until the arrival of Emergency Services; • Contact Emergency Controller and Business Unit Manager; • Control incident scene access and preserve the scene; • Collect list of casualties, equipment damage and status of the ongoing

response and additional information; • Make a full and detailed report including times and actions of the incident;

and, • Attend debrief at the completion of the emergency at the nominated location.

3.3 First Aiders The First Aiders shall undertake first aid treatment in accordance with their relevant training and include, but are not limited to: • Proceeding to the incident once instructed to do so; • Administer first aid as required; • If required, arrange for medical evacuation and other back up as needed; • Welfare of the injured, at all times whilst at the incident site until Emergency

Services arrive; • Keep an accurate log of all treatments given and times; and, • Attend debrief at the completion of the emergency. 3.4 Bitumen Burns Bitumen burns are not the same as heat burns and are to be always treated as per the bitumen burns card. All workers working with bituminous materials are to be aware of the information contained on bitumen burns cards. If a worker suffers a bitumen burn:

• Follow DRABCD • Cancel the source of the burn • Cool the injured area immediately with water (preferably running water)

until the bituminous material has hardened (cooling time will be affected by the temperature and thickness of the bitumen)

• Manage shock - the first aider is to stay with the injured worker until relieved by the ambulance officer

• Call for an ambulance 000

Do not remove solid bituminous materials that have adhered to the skin. Do not remove or cut away clothing over burn’t areas – it may be stuck Do not puncture blisters (could lead to infection) Do not apply lotions, ointments, oils, butter, solvents or other substances to the injury site under any circumstances Remember, even burns no larger than a 20 cent coin require immediate medical attention

When the ambulance arrives, the bitumen burns card is to be attached to the injured persons clothing (prominently displayed). The site manager/ supervisor is to attempt to contact the hospital in advance or alternatively request the ambulance officer to contact the hospital to convey the following:

1. When the injured burn victim will arrive 2. The extent of the bitumen burn (rule of nines) 3. When a limb is completely circled by bitumen 4. Convey information to both the hospital and burns unit regarding the

bitumen burns card. The bitumen burns card is to be shown to all medical staff attending to the injured worker. If a doctor tries to remove bitumen from the skin without referring to a specialist burns unit he/she is to be reminded about the scarring likely to occur as result of this course of action. 3.5 Technical Supervisor The primary role of the Technical Supervisor is to attend the scene and ensure that the incident is rendered safe and damaged equipment removed.

Actions Required During Emergency The duties and responsibilities of the Technical Supervisor on becoming aware of an emergency situation by the Emergency Controller include, but are not limited to:

• Liaise with the Emergency Controller at all times; • Be familiar with the buildings on site and emergency exits; • Proceed to the scene of the emergency without any delay; • Establish a line of communication with the incident scene; • Locate the Police Service Command Post, introduce yourself to the officer in

charge and ask to speak with the Fire and Rescue Service Incident Controller; • If the scene has not been secured ask the Police Service to do so; • Introduce yourself to the Fire and Rescue Service Incident Controller, assess

the situation and offer any information or advice that may seem appropriate; • Locate product information (including Emergency Procedures Guide); • Alert salvage crews to standby; • Contact the Environmental Officer once the site is safe and any immediate

threats are contained and/or controlled; • Do not allow untrained persons to attempt to salvage or move damaged

vehicles 3.6 Workers and Visitors The duties and responsibilities of all workers, visitors and contractors include, but are not limited to:

• Have sound knowledge of emergency procedures for the site; • The identity of their Area Warden; • During the emergency incident, follow instructions given by the Area Warden; • Comply with all directions given by Emergency Services; • Assemble initially at a safe distance from the situation, preferably near the

nearest fire exit; • Advise the Area Warden if there is anybody in the area who has not moved to

the exits; • If fire or smoke situation warrants immediate evacuation, proceed quickly and

quietly, and in an orderly manner; • Move clear of the building and to the Evacuation Assembly Area. • Do not re-enter the building until advised by the Emergency Controller it is

safe to do so. • Induction training of workers shall be conducted when a new worker

commences; and, • Refresher training every twelve (12) months to be conducted for all workers 3.7 Site Emergency Response Team (ERT) The ERT is an initial response team to any emergency incident either on or off site. The role of the ERT is to: • Provide immediate response with reference to appropriate action checklists. • Assess the alert level. • Make the initial notifications. • Secure and contain the site. • Co-ordinate with arriving off-site emergency response personnel. • Maintain the response until the situation is resolved. The ERT team including area warden responsibilities are to be also displayed on the SEQ noticeboard. Internal: Emergency Response Team

3.8 Emergency Services It is normal procedure at emergencies for the Police Service to be responsible for:

• Overall control of the area surrounding the incident; • Establishing a command point at the emergency zone edge; and, • Nominate an officer in command of the scene. At the emergency, the Fire and Rescue Service is normally responsible for: • Combating the incident; and, • Creating an emergency zone often by physically cordoning off the area

4.0 Emergency Evacuation Process Raise the alarm by activating a manual call point then evacuate in the following order: 1. Remove from immediate danger; 2. Remove to a safe area; 3. Full evacuation of the building and area. As well as the stages of evacuation, the following is a guideline on the order of evacuation: 1. Personnel who are able to walk; 2. Personnel who are able to walk with assistance; 3. Personnel who are unable to walk. Persons suffering from mobility impairment or other impairment that would impact on their safe evacuation have an obligation to communicate the nature of the impairment to their Area Warden prior to any event that may require their evacuation. The evacuation of persons with mobility impairments is the responsibility of the nominated Area Warden. The Area Warden shall:

• Obtain details, names and location, of any person who are permanent

occupants of their area; • Determine a pre-arranged evacuation point and guide the mobility impaired

person to this area; and, • If possible, assign a person to assist the mobility impaired person in an

emergency.

In the event of an emergency evacuation and the evacuation path consists of obstructions such as stairways, for mobility impaired persons:

• They shall be guided to the pre-arranged evacuation point, e.g. landing in the

fire stairs, or a wet space such as a bathroom; • Ensure that another person remains with the mobility impaired person, if safe

to do so, until further assistance is provided; • The Emergency Controller shall be advised of the location of the person; and, • The Emergency Controller shall arrange priority evacuation with the

5.0 Emergency Communication The alarm panel is located in building 29 with the siren located on site 33. The alarm is triggered by manual call points situated around the site which are displayed on site emergency maps. Communication Devices:

• Two-way radio

• Mobile phone. The use of any radio network at the site shall be minimised in the event of a Bomb threat.

Personnel shall not use the Business Unit telephone system to contact family or friends during an emergency, as overloaded lines can block out vital communication links with the Emergency Services.

6.0 Promulgation of SERP The Site Emergency Evacuation Plan shall be placed in prominent locations on site. The evacuation map shall be on display in each building of the site. The site evacuation plan details all buildings, plant, utilities mains, exit points, evacuation assembly area(s), first aid facilities, emergency alarm locations and fire extinguisher equipment location. All personnel shall be briefed on the site evacuation plan and become familiar with their location as part of site induction. Workers shall refer to BP COR F 0024 11 Site Emergency Procedures Flipchart which contains emergency contact numbers. 9.0 Administration Matters (Summary) The site emergency plan is to be reviewed at least annually or whenever changes adversely affect site operations. If an incident does occur BP COR F 08 07 Emergency Response Action Log is to be completed. BP COR F 09 1 Incident/Non Conformance Report Form is to be completed in accordance with BP COR SP 09 Incident Reporting and Investigation. BP COR F 08 13 Emergency Drill Debriefing form is to be completed for regular evacuation rehearsals. Staff training as per AS 3745: 2010 Planning for Emergencies in Facilities is to completed as per the Site Training matrix/TNA. Emergency management procedures are to be incorporated within employee & contractor inductions. Distribution List of SERP Organisation Date FRNSW 22/11/2013 File Copy 22/11/2013 10.0 Evacuation Exercise Drills Evacuation drills shall be arranged by the Emergency Controller, at a frequency of not less than one per year. They should be conducted when full staffing is on site, wherever practicable. All occupants of the floor(s) or area(s) involved in the evacuation exercises shall take part unless the Emergency Controller granted an exemption prior to conducting the exercise. At the earliest practicable time after the evacuation exercise, the Emergency Controller, Area Wardens and other nominated personnel shall attend a debriefing session to be chaired by the Emergency Controller.

The main purpose of debriefing is to identify and correct any deficiencies in the procedure or its implementation. All areas identified for improvement shall be documented on BP COR F 08 13 Emergency Drill Debriefing form. Appendix A Actions ON Emergency Evacuation Plan (Summary) Evacuation

• The fire Alarm is also the evacuation alarm. • Regardless of the situation if the fire alarm is activated, evacuation must

take place. • If a clear danger exists, emergency services will be called by the

emergency controller. • The Emergency Controller, Technical Supervisor or Area Warden

may reassess the situation for immediate actions to take place. • If the designated evacuation point becomes endangered, or the

evacuees need to remain outside the premises for an extended period, personnel will be relocated to a suitable point at the direction of the emergency coordinator.

Head Count

• After evacuation, all persons on site including visitors and contractors shall assemble at the evacuation point.

• The evacuation or assembly point is located at the gate of the front car park.

• The Emergency Response Team (ERT) will conduct a roll call to ensure all persons are accounted for including visitors and contractors.

• Missing persons shall be advised to the Emergency Controller, who will then advise the emergency services.

• No personnel shall undertake search and rescue for missing persons in endangered areas.

Adjacent Areas

• The occupants of adjacent premises are to be advised if endangered by the emergency.

• Evacuation of adjacent premises is the responsibility of the individual companies and the emergency services.

Re-Entry

• Re-entry to the site will not take place until advised by the emergency services to do so. This will be relayed to all personnel by the Emergency Controller.

Review • The evacuation plan shall be reviewed:

a. Following any emergency or training exercise that expose short comings.

b. Following any significant changes to the layout or operations. c. At least once annually.

Appendix B Portable Fire Extinguisher Guide How to use a fire extinguisher: USE THE PASS SYSTEM 1. Pull the pin at the top of the extinguisher

2. Aim the nozzle towards the base of the fire

3. Squeeze the handle to discharge the extinguisher –stand approximately

two metres from the fire

4. Sweep the nozzle back and forth at the base of the fire

*Limited indicates that the extinguish-ant is not the agent of choice for the class of fire, but that it will have a limited extinguishing capability. ** Class D fire (involving combustible metals). Use only special purpose extinguishers and seek expert advise. *** Solvents which may mix with water, e.g. alcohol and acetone, are known as polar solvents and require special foam. These solvents break down conventional AFFF.
