5. Cash Your Check… GAF will send your rebate check upon approval!By submitting a photo, you represent and warrant that you are the sole author and owner of the photo and you assign to GAF all right, title, and interest in the photo for use in any and all media including, but not limited to, advertising, promotion, publicity, trade, merchandising, packaging, public relations, and on any GAF website.

TERMS AND CONDITIONS:• Size of the showcase home’s roof must be a minimum of 25 squares and a

maximum of 60 squares.• This offer applies to a maximum of 1 showcase home per builder/contractor.• One rebate per showcase home; original distributor invoices must accompany

rebate request form.• Builder or Contractor must submit both a before and after photo of a completed

showcase home roofed with Woodland™ or Monaco™ Shingles as well as original distributor invoices to support rebate request to GAF.

• Shingles must be purchased and installed on the showcase home between June 1, 2012 and October 31, 2012. This offer cannot be combined with any other GAF offer.

• Requests must be postmarked no later than November 30, 2012.• Allow 6 to 8 weeks from receipt of rebate form for your rebate check to arrive.• Valid for U.S. Contractors/Builders only. Showcase home must be located in the U.S.
