The Making of a SpecialistGoverner bestows ‘DM’ on Nephrologist Dr Jigar Shrimali

To become a doctor, then to spe-cialise and later to become a super specialist in a branch is the dream of many but fulfilled by few. Out of hundreds of thousands applicants, few enter medical colleges, fewer specialise and every year one or two super specialists qualify from one university.

Dr Jigar Shrimali, the young Ne-phrologist of Shalby, is the new entrant in the select world of su-per specialists. He joined Shalby a few months ago after passing his final exams leading to Doctorate of Medicine [DM] in Nephrology.

The path towards this goal is fraught with extreme hard work,

sleepless periods which extend for days when on emergency duty, constant studies despite a huge clinical work load, managing the guide’s scientific correspondence, attending medical conferences, networking with peers all around the world, searching for good hos-pitals placement and so on.

One of the memories he shared as a DM fellow is worth reading. As a resident physician [the doctor who lives in the hospital premises and is on first call], Dr Shrimali was the physician who coordinat-ed a cadaveric kidney transplants [kidneys transplanted from recent-ly dead donors].

‘The arrival of a dead donor body for removal of kidney, liver and cornea for transplants is like a hur-ricane striking. As soon as I was informed that a cadaver was on the way to our hospital, I knew I would not be able to sleep for the next 36 hours. I had to put in mo-tion the whole process for informing the next suitable patient on the kid-ney transplant waiting list, the sur-geons who would do the surgery, book enough cross matched blood from the blood bank, book opera-tion theatres, inform the ICU staff to expect a transplant patient and of course, inform my boss. Then the real work started...

ternational visiting faculty by the University of Dundee, United Kingdom, for its M.Ch. [Orth] teaching program by Prof R J Abboud BEng, MSc, PhD, ILTM, SMIEEE, Hon FRCS (Eng). Prof Abboud holds many teaching and clinical posts in the University of Dundee. Dr Marothi completed his M.Ch. studies in Dundee in the year 2008 and later took extensive training in the USA before joining Shalby Hospitals. A speeding auto rickshaw tried to overtake a car, but slammed into Snehil Seth’s bike. Luckily the po-lice were nearby, they called an am-bulance and Snehil by accompanied by friend Yash Shah to Shalby Hos-

pitals. Snehil was losing blood...On Arrival

In Shalby, doctors found that he was haemodynamically unstable, meaning his blood pressure was low and his pulse rate was very high, his blood oxygen was low.

On examination, his arm was found to be fractured, but that could not explain his critical condition. He had pain in the abdomen. He proba-bly had injury on the abdomen, but it needed further investigations.Investigations

His arm was X Rayed and an ultra-sonography [USG] study was made on his abdomen. The USG showed blood in the abdomen.

On hearing about this, Dr Avinash immediately ordered a CT Scan, as that would show if Sahil was bleed-ing internally.

Increasing HaemorrhageAlthough not much blood was

seen outside Snehil’s body, Snehil had profuse internal bleeding. He had lost almost 50-60% of his to-tal blood. His BP fell, his pulse was

rapid, his blood oxygen level was low. Such a condition is called being haemodynamically unstable.

Worsening ConditionThe CT Scan was done half an

hour after the USG. In that half hour the liver bled even more and his condition worsened. He had to be operated immediately...Timely surgery, life saved

Dr Avinash Tank operated on Sne-hil the same night. Snehil’s abdo-men was a bloody mess, Dr Tank painstakingly identified the bleed-ing vessels and bleeding areas and cauterised or sutured them to stop the bleeding. With decades of expe-rience in treating liver injuries, Dr Avinash was successful. Fracture Repair

Dr Pranav Shah repaired his frac-ture as soon as his life was safe, with plate and screws, a major sur-gery made more risky because of Snehil’s delicate condition due to blood loss.

I met Snehil Seth when he had come for a follow up examination. He was fully aware that he had got another lease of life, given by the surgeons, and he was very grate-ful to his friend, who had taken the lead to bring him to Shalby. Secret of Success

Saritaben [name changed] was miserable, she suffered heavy pe-riods and pain because of fibroids, and her uterus and bladder were slipping downward [a condition called prolapse of the uterus]. The surgeons counselled her about the newer methods of surgery and they told her about all treatment op-tions. They told her that the uterus can be removed in four ways:Types of Hysterectomy

Abdominal Hysterectomy• Vaginal Hysterectomy• Laparoscopic-assisted vaginal • hysterectomy [LAVH]

Total Laparoscopic Hysterectomy • [TLH]She could not be operated through

the vagina because the size of her uterus was bigger than what can be removed from the vagina. To have a quick recovery without a scar for her condition, the only option was... Total Laparoscopic Hysterectomy

It is a form of minimally invasive surgery. It is similar to the meth-ods used in gallbladder removal and and appendectomy surgeries. It is also used in cardiology, urology, and other fields.

But in gynaecology, the accept-ance of laparoscopy has come more slowly. Out of all the hysterecto-mies done, more than 80% are ab-dominal surgeries. But they could have been done as laparoscopic or vaginal hysterec-tomies.Today’s Need

Now that the number of working women is i n c r e a s -ing and the need for a good cos-metic look [meaning no scars] is de-sired, laparos-copy will surely be the way ahead for hysterectomies. Because laparotomy gives faster recovery with minimal scars.

Besides the patient gets all the ad-vantages of a laparoscopic surgery, which are:

transfusion Small scars• Early recovery• Each part can be visualised much • better, so far more precise sur-gery Very less surgical tissue injury, so • faster internal healing

High End EquipmentHowever, TLH needs special sur-

gical equipment called a morcela-tor and of course, a good quality laparoscope. Also, it needs a highly skilled team.Surgical Skills

Unless the surgical team is skilled, there is more risk to the patient than open surgery. The same applies for hospital infra structure, which in-

cludes well appointed op-eration theatres, ICUs

and well managed patient rooms.

RecoverySaritaben’s

exper ience on the sec-ond day af-ter her TLH shows the success of her surgery:“The next

morning, I walked out of

the hospital,” she said.

“I felt wonderful. I never took any of the

medicines prescribed for pain because I didn’t

have any pain!” She was back at work after only

four days, although she had under-gone a major surgery! That is the magic of Laparaoscopic Surgery!

Dear Sir,Sub: Sharing of experi-ence in Shalby HospitalsI have been receiving complimentary copies of Shalby Times for the last few months and on going through the last issue no. 3, I came across an invita-tion to share my experience in Shalby Hospitals. This has motivated me to share my experience on the occa-sion of my undergoing Spinal Surgery during August-2014. I was admitted to Shalby Hospitals on 20th August for pre-operative pathological and other tests and was operated for Spinal Impediment to which I was suffering for over a year. The surgery was per-formed by Hon. Dr. Niraj Vasavada, ably assisted by Dr. Pratik Lodha and their team of doctors. There has been remarkable recovery and relief from the Spinal Ailment, I can walk one km regularly. I do feel that such an achievement could be registered with the highly professional, skill, humanitarian approach, highly co-operative attitude and utmost concern for the patient displayed by Dr. Niraj Vasavada & also his assistant, Dr. Pratik Lodha, not only during sur-gery but also in the post surgery period extending over almost 3 months. I am thus greatly indebted to both these doctors and through them the Shalby Hospitals management, for my gradual recovery and achieving almost normalcy in my physical disposition.

64 year old Minaxiben was diagnosed with breast cancer in 2005. She was brought to Shalby Hospi-tals and operated. She was given chemotherapy. She responded to the treat-ment well.

But in September 2007 her cancer spread to the liver, lung and bone. She was again admitted in Shalby and Dr Kajal Shah started treatment.

The cancer had damaged a small portion of liver in September 2009. To treat that, Dr Kajal did Radio Frequency Ablation. In August 2010, the size of the tumour increased.

In March 2012, a part of the liver was removed due to spread of cancer. In august 2012 she developed recur-rence in left lobe of liver.

Hormonal treatment was given. In February 2013, the cancer progressed to both sides of her liver. But she did not have symp-toms. Tests of her liver function were also nor-mal.

Then she developed multiple well defined one cm sized lesions in both

lungs. Chemotherapy was be-

gun and she responded well.

Just at the time when it seemed that her condition had stabilised, she devel-oped double pneumonia, infection and shock.

Then her Left Ventricle [LV] of the heart began to fail. She had severe LV dysfunction and was ad-mitted in ICU from in July 2014.

She was put on more aggressive chemotherapy. She recovered after the se-ries of prolonged illnesses in September 2014.

Now she is being given maintenance chemother-apy.

She has undergone many surgeries and many doses of chemotherapy, she has been through pneumonia and heart dysfunction in the preceding nine years, and yet she has survived. Perhaps Minaxiben is stronger than Cancer!

TKR surgeries begin in Shalby, Jabalpur Shalby Hospitals, Jabalpur, is operational, many surgeons begin surgeriesWith literally hundreds of patients coming from Madhya Pradesh to Ahmedabad es-

pecially for joint replacement surgery, it was the vision of Dr Vikram Shah to be able to provide Joint Replacement Surgery in Jabalpur, the third largest city in the State of Madhya Pradesh. That has turned to reality. Dr Vikram also operated in the new unit.

5 The laying of the Foundation...Shalby organises many free medical check up

camps. The above is a news item about such a camp organised by Shalby in December, 2012. At that time the making of the hospital was en-visioned.

Today the connect with so many patients shows up in the response to the the hospital services. Already most services are active and the remaining are to be operational soon.
