Career summary I started my career in Obstetrics and Gynaecology as a postgraduate trainee at Madras Medical College, Dr MGR Medical University. It is one of the leading universities in India responsible for undergraduate and postgraduate training. My interest in research began during my tenure there while evaluating the diagnostic role of cardiotocograph in mothers with meconium stained liquor for my MD. I subsequently worked as a senior SHO at Christian Fellowship Hospital in Oddanchatram, India. In recognition of my work, the British Council sponsored me in 2001 to undertake further training in UK. After 2 years in the clinical arena as an SHO, in 2004, I pursued my research interests by accepting the job of Clinical Research Fellow at Keele University. I had the opportunity to undertake Health Technology Assessment projects, especially diagnostic and therapeutic systematic reviews. During this period I was involved in the development of diagnostic EBM course at Birmingham Women’s Hospital. In 2006 I joined University of Birmingham as a Clinical Lecturer in Obstetrics and Gynaecology and Clinical epidemiology with Prof Khan and registered for PhD. The job offered excellent research training in Health Technology Assessment projects involving systematic reviews, clinical trials and research methodology in addition to medical education projects. At the end of my term as Clinical Senior Lecturer, I was offered the Professorship at Queen Mary University of London (QMUL). I am currently based at the Women’s Health Research Unit at Barts and the London School of Medicine and Dentistry, QMUL. I hold an honorary appointment as Consultant in Obstetrics and Maternal Medicine at Barts Health NHS Foundation Trust. I am the R and D Director of the Women’s Health CAG (Clinical Academic Group) at Barts Health NHS Trust.

I completed my Advanced Training in Specialist Medicine (ATSM) in Maternal Medicine, Labour ward management and Medical Education and Completion of Core Training (CCT) in Obstetrics and Gynaecology in Aug 2010. My special interest is Maternal Medicine in Obstetrics. My training in maternal medicine was undertaken in the largest and busiest tertiary obstetric unit in West Midlands (Birmingham Women’s Hospital) with 9000 deliveries per year. This involves management of obstetric patients in the joint diabetic, epilepsy, endocrine, cardiac, haematology, substance misuse, mental health, HIV, renal and immunology clinics in partnership with clinicians at Queen Elizabeth Hospital. My academic work has complemented my interest in maternal medicine as evidenced by my work in pre eclampsia and epilepsy. My PhD in “Health Technology Assessments in maternal and perinatal medicine” has provided me the opportunity to evaluate the tests and treatments in this aarea. Currently I provide obsteric services in the delivery suite and run the joint obstetric diabetic clinic and the endocrinology clinics at Barts Health NHS Foundation Trust. The diabetic clinic provides services to high risk mothers who are predominantly of Asian origin. My work in this clinic is closely linked to the recently funded multinational FP7 research project (GIFTS) on nutrition and outcomes in Asian women. I spend 2-4 hours training the juniors every week, particularly in surgery, although this is not part of my formal job plan. My oncall commitment is for obstetrics (1:9). My clinical work objectives for the coming two years include further development of the quality and range of services within the obsteric unit, update the guidelines and supervise audits.

Academic work and publications

The following figure outlines my core research themes and the relevant projects. I have a strong interest in clinical trials, diagnostic research, evidence synthesis and evidence based medicine. My research work is largely collaborative with national and international partners.

My research strength is mainly in clinical trials, evidence synthesis and EBM (Evidence Based Medicine) projects. My work in pre-eclampsia is further expanded by studying the genetic effects on the development of early onset pre-eclampsia and its complications. I was awarded a PhD by the University of Birmingham for my work on Health Technology Assessment in Maternal and Perinatal Medicine. The output from the PhD directly led to 3 successful HTA grants (PREP, EMPIRE, and Pulse Ox) and NICE recommendations for tests in pre-eclampsia. I am part of the core team at Women’s Health Research Network at Barts and the London and involved in the recruitment of researchers, development of the department and establishing links with other units in North East London through the newly founded Katherine Twining Network. Evidence Synthesis research I have established a strong portfolio of research activities in systematic review projects. I co-lead the Multidisciplinary Evidence Synthesis Hub (mEsh) of QMUL, a unit dedicated to the development and publication of systematic review projects. I am currently funded by the NIHR to undertake IPD (Individual Patient Data) Meta analysis on dietary and lifestyle interventions in pregnancy. The core methodology of my PhD was evidence synthesis, and it served the dual purpose of collating the best available evidence to inform practice and identifying the gaps for further research. It led to eight peer reviewed publications and was instrumental in securing NIHR funding of £3 million in total for three clinical trials. I built on this work to applying for NIHR Commissioned calls on relevant systematic review projects and was successful. All three projects have now been completed, and the work on obesity has been published in BMJ and widely reported by the media. Portfolio studies

I lead two multicentre NIHR clinical trials involving 50 centres in UK. I am supported by a strong research team established by me and currently responsible for setting up five portfolio studies in the CEL Comprehensive Clinical Research Network (CLRN). I was instrumental in establishing the Katherine Twining Network as a multidisciplinary collaborative Network of seven Trusts in North East London. I

obtained support from the Central and East London (CEL) CLRN to allow us to effectively accrue large numbers needed into trials rapidly. The Network membership is composed of academics, R&D managers, professionals and consumers. Studies selected receive support for Trust R&D approvals; research ethics applications; service user involvement; honorary NHS contracts for researchers; project publicity; promotion of the study with the services; participant recruitment; liaising with patients; screening and assessment of patients; and dissemination of results. As Clinical Lecturer at Birmingham Women’s Hospital, I was part of the team that recruited over 9000 patients in 2008-10 and the specialty delivered comparatively the highest number of accruals. Impact of research My work has been instrumental in changing existing national and international guidelines and clinical practice. The following are some of the examples

- My systematic reviews on the role of uric acid and proteinuria in the prediction of complications in women with pre-eclampsia identified their limited role in clinical practice. The NICE ‘ Hypertensive Disorders in Pregnancy’ guideline directly referenced these papers in recommending not to test for proteinuria once established, and to avoid use of uric acid results alone in the management of women with pre-eclampsia.

- The meta analysis on the accuracy of ultrasound in diagnosing miscarriage in early pregnancy, highlighted the possibility of falsely diagnosing miscarriage with existing guidelines. The work was presented to the journalists from over 15 media, through the Science Media Centre, London. It was widely taken up by the mainstream media and I was interviewed by national media including ITN and Sky with invitations from Radio 3 and 4. The RCOG (Royal College of Obstetricians and Gynaecologists) responded by changing its guidelines in this field the following week (http://www.qmul.ac.uk/media/news/items/smd/57260.html).

- I led the NIHR HTA work evaluating the effect of dietary and lifestyle interventions in reducing or preventing obesity in pregnancy. The findings of this project, published in the BMJ were widely taken up by the mainstream media including BBC, Sky, ITN, The Times, Telegraph, Independent and Metro. The figures from the QMUL communication office estimate that the findings reached an estimated audience of 12 million readers.

Student supervision I derive immense pleasure in supporting the development of my junior researchers. I currently supervise two MD students (L Velauthar and F Fong). I am the educational supervisor for two postgraduate trainees, one of them an Academic Foundation Fellow.

I hold over £ 6 million in research grants. I was instrumental in the conception, design and development of the proposal reflected in my time more than 0.15fte in these HTA projects. The multinational Leonardo da Vinci European project Evidence Based Medicine: Training the Trainers across the Healthcare Sector was run by me as the project manager. The project involves collaboration with 7 EU countries (Germany, Netherlands, Switzerland, Italy, Poland, Hungary and UK) and is responsible for developing e modules. Through this project I have been involved in curriculum design and development and piloting of curriculum in 5 European countries (UK, Netherlands, Hungary, Poland, Switzerland) in addition to the development and validation of assessment tool for training the trainers in EBM.

- Training the Trainers e-curriculum: West Midlands Deanery The School of Obstetrics and Gynaecology of West Midlands Deanery has commissioned the development of Training the Trainers e modules for the regional educational supervisors. I am involved in its conception, development, packaging and dissemination of the modules.

Course Tutor

I regularly hold Critical Appraisal Workshops for junior and senior STs in the London region I was one of the core organisers and tutors in the two day national level Diagnostic Methods Workshop at the Education Resource Centre, Birmingham which was incorporated into the MSc programme in Health Technology Assessment at the University of Birmingham. I have been part of the team that has started a similar EBM workshop at University Hospital North Staffordshire and Keele University. I was involved in running the Basic Surgical Skills Course at the Birmingham Women’s Hospital for the past 3 years. I deliver lectures and demonstrate in the Third and Fourth Degree Perineal repair course at Birmingham Women’s Hospital and was part of the team involved in teaching at the Third Degree Perineal Repair Workshop at University Hospital North Staffordshire, Stoke-on-Trent. I have been involved in the development of part 1 and part 2 MRCOG online courses (www.acecourses.co.uk and www.aceonline.org.uk).

Journal Club

I established the bimonthly Critical Appraisal of Topic, CAT club, for junior and senior clinicians. It offers an interactive forum for learning and application of critical appraisal skills. I co-ordinated the running of the weekly Journal club at Birmingham Women’s Hospital. I have introduced new critical appraisal tool, the GATE (Graphic Appraisal Tool for Epidemiology) tool. The fortnightly CAT (Critical Appraisal of a Topic) meeting at University Hospital North Staffordshire was established and run by me during my tenure as Clinical Research Fellow. I also regularly teach junior and senior registrars critical appraisal of literature. The monthly ‘Low Apgar and CTG teaching’ meet run by me at the University Hospital North Staffordshire provided an opportunity for discussion and learning in a multidisciplinary setting.

Management and Leadership

My work as senior academic is multidisciplinary by nature and requires good interpersonal and leadership skills. As a clinical academic with over 7 years’ experience, I have taken a keen interest in research management, governance and training. I have appointed several research posts across both the Trust and University. I have substantial experience of chairing meetings effectively: I have been the lead for EU multinational projects and have chaired Trials Steering Committees and Trial Management Groups. Through experience of managing large trials and HTA projects, and contribution to independent supervision of large national multi-centre trials, I have acquired and developed research organisational and leadership qualities. membership of Executive Committee of the British Maternal Fetal Medicine Society (2011 onwards), RCOG member on the National Obesity Steering Group of the Academy of Medical Royal Colleges (2012 onwards), Katherine Twining Network (2010 onwards) and the Reproductive Health and Childbirth speciality group of the Birmingham and Black Country Comprehensive Local Research Network (2008-2011). As part of the core team at Women’s Health Research Network, I am actively involved in the recruitment of researchers, development of the department and establishing links with other units in North East London through the newly founded Katherine Twining Network. I co-ordinate and supervise a team that has 2 trial co-ordinators, 10 research nurses and midwives, a data assistant and an administrator. I work closely with the 50 principal investigators involved in the portfolio studies and manage the budget of research projects of around £3 million. I am adept in team working for provision of patient care within the NHS environment and research projects within the academic milieu. My 360 degree assessments carried out as part of my appraisals show that I have universally received high scores for the question on team working.

Personal qualities and aspirations

I am a passionate optimist who strongly believes that with motivation, will power and hard work most challenges can be overcome. I have a ‘can do’ attitude to life. I have always endeavoured to give the very best at all times in my professional and personal life.

My exposure to healthcare in rural India and my training in UK has enabled me to appreciate the stark contrasts in healthcare systems. I am a Board member of ELLY Appeal (East London International Women’s Health) of Barts Charity, an appeal dedicated to bring relevant ideas, clinical evidence, resources, projects and training to improve global health, with the aim of reducing maternal death and disability in the developing world.

I am motivated and enthusiastic in my work and thrive in an intellectually simulating workplace. I relax after a hectic day at work by indulging in pencil sketching and listening to music. I am skilled in ‘Rangoli’, an indigenous decoration art on the floor. I hold a red belt in Zenshin Karate.
