On this page, you will check all the reasons why you are taking this course. In addition, you will need to answer if you are 18 or over by checking yes or no.
(If you are under 18, check no and click next page and the site will take you to another webpage to complete that form)
If your placement is off‐campus face to face experiential learning, you will need to complete the COVID‐19 Training.
Click “yes,” if you will be physically at the site during the semester. You will then be required to complete the COVID‐19 Disclosure and Training on the next 2 windows.
Click “no,” if you will complete your hours remotely.
