Where Can I Get More Information about These Programs or Providers?EligibilityThese programs have different eligibility rules. To join SCO or the Frail Elder Waiver, you must meet all eligibility rules. This includes being eligible for MassHealth. To enroll in PACE or to get services as part of the Massachusetts Home Care Program, you must meet all eligibility rules, but you do not need to be eligible for MassHealth. You may be eligible for one or all of these programs. If you would like to learn more about these programs, please call the phone numbers listed in this brochure. Participation in these programs is voluntary. You may stop participating at any time.

Senior Care Options (SCO)

If you join the SCO program, your SCO plan gives you comprehensive, medically necessary health care services. Nurses and other SCO staff will visit you at home. This helps your care providers find out what you need to stay healthy and remain at home. If you join this program, you must go to doctors and other providers in the SCO plan’s provider network (except in special circumstances).

Program Features You keep all your MassHealth benefits. You keep all your Medicare benefits, if

applicable. You get a care plan developed for you by a

team of health professionals who specialize in the care of older adults.

Your care is coordinated with health and social service workers who visit you at home when needed.

You get dental coverage including fillings, dentures, and crowns.

You get vision and hearing-aid coverage. You get free presciption drugs. You may get personal care attendants to help

you at home. You may get transportation from your home to

medical appointments. You may get home-delivered meals.

The Program of All-inclusive Care for the Elderly (PACE) If you join PACE, your PACE program gives you comprehensive, medically necessary health care services. PACE is designed to help older adults stay in the community instead of getting nursing home care. If you join PACE, your primary care is coordinated by doctors and nurses at a PACE day center. At the PACE day center, you can have a meal and participate in activities. You can also see social workers, physical therapists, and other professionals who specialize in the care of older adults. Many services are also provided at home. Most PACE primary care providers work only with people in the PACE program. If you need a specialist, PACE will help you to choose one from a network of providers. If you join this program, you must go to doctors and other providers in the PACE program’s provider network (except in special circumstances).

Frail Elder Waiver and Massachusetts Home Care ProgramThe Frail Elder Waiver pays for home- and community-based services for eligible people on MassHealth. The Frail Elder Waiver is provided through Aging Services Access Points (ASAPs). ASAPs also provide other services as part of the Massachusetts Home Care Program. These programs help people served by ASAPs to keep their independence, safety, and dignity and to continue to live at home or in a community setting.

Services Provided Through ASAPs Include Case management Care coordination Information and referral Home care services, including: • homemaker • personal care • home health aide • skilled nursing
