Reasonable and Customary:It means that the medical expenses are comparable with the level of fees charged by the majority of licensed doctors or hospitals in the member’s network in Saudi Arabia. It also means that the medical treatment does not di�er signi�cantly from what the licensed doctors consider acceptable for being the usual and customary treatment for any particular service.

Bene�ts:The costs of providing health services included in the insurance cover within the limits shown in the Table of Applicable

Healthcare Bene�ts:The mandatory bene�ts as speci�ed in the regulations set forth by CCHI provide comprehensive coverage. However, depending on your Saudi Enaya coverage you may receive higher bene�ts Please refer to your Table of Bene�ts.

Table of Bene�ts: The attached table of bene�t applicable to your plan shows the maximum bene�ts we will pay for each member.

Insured/Member:The person (employee or dependent) included under the scheme.

Card Number:A unique number provided to each Saudi Enaya insured member as an identi�cation.

Insurer:Your health insurance company Saudi Enaya Cooperative Insurance Co.

Policyholder:The employer or person in whose name the policy is issued.CCHI (Council for Cooperative Health Insurance): Regulator for Health Insurance in Saudi Arabia.

Provider:Physician, hospital, group practice, nursing home, pharmacy or any facility, individual or group of individuals that provides a health care service.

De�nitions

Chronic Condition:A medical condition or episode of ill health which persists for a long period.

Pre-existing & Chronic Diseases:All pre-existing and chronic diseases are covered except those listed in the Standard Exclusions.

Medical Report:A document �led by the doctor explaining the symptoms, diagnosis and treatment given.

Emergency:The medical treatment required for an insured person as a result of an accident, or an illness requiring prompt medical attention.

Emergency Treatment:You are covered up to 100% of your annual limit for covered emergency services available outside of your network, subject to the geographical coverage as per your enclosed Table of Bene�ts.

Out-patient Treatment:The Out-patient bene�t covers all eligible treatments, procedures and services other than those listed in the Standard Exclusions.

Coverage:The amount and extent of risk covered by Saudi Enaya.

Standard Exclusions:The health bene�ts and services excluded from your coverage.

Reimbursement:A method for compensation of expenses incurred by the insured member at a provider.

Direct Billing:The facility of non-payment granted to the insured at the provider appointed by Saudi Enaya whereby all such costs are directly billed to Saudi Enaya by the provider.

Network:All the primary physicians, specialists, hospitals and other providers who participate in this plan to provide your medical care. A list of your network providers is enclosed with this membership package.

Maternity:• Maternity bene�ts are automatically provided for all eligible females and covers the cost of pregnancy, normal deliver y, caesarean section and maternity complications including legal abortion and miscarriage. (Please refer to the enclosed Table of Bene�ts)• The cost of ear piercing for new born girls and circumcision for newborn boys is also covered under this bene�t. • Maternity coverage is speci�ed on your membership card with a sign indicating your coverage. “Y” means maternity is covered and “N” means maternity is not covered.

Dental:Dental bene�ts cover the cost of consultation, x-rays, normal and surgical extractions, root canal treatment, amalgam/composite �lling, gum treatment and prescribed medicines.

Optical:The optical bene�t includes consultation, vision tests and prescription glasses, including frames.

Co-Insurance:Your plan may include a Co-Insurance payment which is the amount that is required to be paid to the provider when you are being treated for an applicable out-patient service. This will be speci�ed on your membership card as a percentage % of the treatment cost with a maximum limit of SAR100.

In-patient Treatment:The In-patient bene�t covers all eligible treatments, procedures and services other than those listed in the Standard Exclusions.

Room Type:Depending on your level of coverage, your hospital room will be speci�ed on your membership card as a normal suite, private room, semi-private room or shared room.

Companion Cover:If you have a dependent child that is under the age of 12 or if medically necessary as assessed by the attending Physician, Saudi Enaya will pay for a companion to accompany the child during the hospital stay.

Programs for every family

Saudi Enaya o�ers a variety of medical insurance programs that are each compliant with the requirements mandated by the CCHI.

The programs vary in the levels of access to health care providers locally or internationally, room type and bene�t limits. Saudi Enaya additionally o�ers special coverage programs such as Lasik Surgery, pediatric or Congenital Disorder based on the speci�c needs of each customer.

The Diamond Program provides exceptional services and features that meet the requirement of our valued customers. Diamond program goes beyond the minimum requirements of CCHI and includes the following:

Direct Billing & Reimbursement:

Saudi Enaya guarantees its members that they can receive treatment on a direct billing basis for all the network service providers listed for their program level. This means that Saudi Enaya will pay for all services (in accordance to the bene�ts of that speci�c program’s coverage). Members will only be responsible to pay for their out-patient coinsurance (according to program level) directly to the service provider.

If services are required from any provider outside the network, (as per the table of bene�ts, members are allowed to proceed on cash basis and submit invoices for reimbursement, as explained below.

All Reimbursement Claims must be submitted with the following original documentations:

• Saudi Enaya Reimbursement form completed and signed.

• Detailed medical report �lled out by your treating doctor along with his signature, date and stamp.• Itemized invoices.

• Prescriptions, if applicable.

• Investigation results of any laboratory/diagnostic reports, if applicable.

Please note:

• Reimbursement claims must be received no longer than 30 days after the date of treatment for services available within KSA and 60 days for services available outside KSA.

• Processing and settlement of your reimbursement claim will take 14 working days upon the receipt of the claim along with required documentation.

Hospital Delegates:

Saudi Enaya hospital delegates are available in selected locations to assist and support our members in:

• Itemized invoices

• Supervising and resolving their issues

• Facilitate member’s appointments• Guide members on hospital process

• Send pre-authorization requests

• Immediate assistance when required

Members can utilize services outside of their designated Network Provider, by reimbursement. The amount will be reimbursed in accordance to the member’s program speci�cations. This is done to ensure that:

• You are eligible for the treatment or service under your policy benefits.• The proposed treatment or service is medically necessary.• The proposed treatment or service is within the reasonable and customary charges of comparable network Providers• There is enough remaining benefit limit on your membership to cover the treatment or service cost

Two international networks to consider, based on your needs of coverage, that include USA, Europe and the rest of the world

Medical Tourism: Saudi Enaya o�ers comprehensive medical service for medical services that will be provided outside the Kingdom. Members will have the bene�ts of traveling abroad to get the medical treatment they need, based on the case. The medical tourism covers the expenses of transportation, accommodation, �ights and treatments.

International Direct Billing:Saudi Enaya provides international direct billing to hospitals and medical centers so members do not have to concern themselves with payment of claims during their travel.

Home Country Treatment:Saudi Enaya provides home country treatment services for expatriate members living in KSA.

Global Emergency Evacuation and Repatriation Through Assist America:Our members bene�t from a unique global emergency services program from Assist America. This program immediately connects members to doctors, hospitals, pharmacies and other service providers in case of a medical emergency while travelling 150 KILOMETERS or more away from your permanent residence, or in another country. The Assist America Operations Center is sta�ed 24 hours

a day, 365 days a year by trained multilingual and medical personnel, (including nurses and doctors), to advise and assist in the event of an emergency.

Assist America completely arranges and pays for the assistance services it provides without limits on the covered cost. This alleviates many of the obstacles and potential expenses that can be caused by medical emergencies away from home. It is important that our members keep their membership card with them at all times so that they can call for services whenever needed. Assist America is not a travel or medical insurance program; rather it is a provider of global emergency services.

*Assist America’s services do not replace medical insurance during medical emergencies away from home. All medical costs incurred should be submitted according to the program a member is enrolled in and are subject to the policy limits of the coverage.

Key Services:

• Care of Minor Children: Assist America will arrange for the care of children left unattended as the result of a medical emergency and pay for any transportation costs involved in such arrangements.

• Return of Moral Remains: Assist America will assist with the logistics of returning a member’s remains home in the event of his or her death. (This service includes arranging the preparation of the remains for transport, procuring required documenta-tion, providing the necessary shipping container as well as paying for transport).

• Lost Luggage or Document Assistance: Assist America will help members locate lost luggage, documents, or personal belongings.

• Interpreter and Legal Referrals: Assist America will refer members to interpreters and / or legal personnel, as necessary.

• Pre-trip Information: Assist America o�ers members country pro�les that include visa requirements, immunization and inoculation recommendations, as well as security advisories for any travel destination.

• Medical Consultation, Evaluation & Referral: Calls to Assist America’s Operations Center are evaluated by medical personnel and referred to quali�ed doctors and / or hospitals.

• Hospital Admission Assistance: Assist America fosters prompt hospital admission by validating the member’s health coverage or by advancing funds to the hospitals as needed.

• Emergency Medical Evacuation: If adequate medical facilities are not available locally, Assist America will use whatever mode of transport, equipment and personnel necessary to evacuate a member to the nearest facility capable of providing a high standard of care.

• Medical Monitoring: Assist America’s medical personnel will maintain regular communication with the member’s attending physician and / or hospital and relay information to the family, as appropriate.

• Medical Repatriation: If a member still requires medical assistance upon being discharged from a hospital, Assist America will repatriate them home or to a rehabilitation facility with a medical or nonmedical escort, as necessary.

• Prescription Assistance: If a member needs a replacement prescription while travelling, Assist America will help in �lling that prescription.

• Emergency Message Transmission: Assist America will receive and transmit authorized emergency messages for members.

• Compassionate Visit: If a member is travelling alone and will be hospitalized for more than �ve days, Assist America will provide economy, roundtrip, common carrier transportation to the place of hospitalization for a designated family member or friend.

Conditions and Exclusions:

Conditions:

Assist America will not provide services in the following instances:• Travel undertaken speci�cally for securing medical treatment.• Injuries resulting from participation in acts of war or insurrection.• Commission of unlawful act(s)• Attempt at suicide.• Incident involving the use of drugs unless prescribed by a physician.• Transfer of member from one medical facility to another medical facility of similar capabilities and providing a similar level of care.

Assist America will not evacuate or repatriate a member:• Without medical authorization.• With mild lesions, simple injuries such as sprains, simple fractures, or mild sickness which can be treated by local doctors and do not prevent the member from continuing his/her trip or returning home.• With a pregnancy over six months.• With mental or nervous disorders unless hospitalized.

Exclusions:

Trips exceeding 90 days from legal residence without prior noti�cation to Assist America (separate purchase of Expatriate coverage is available). While assistance services are available worldwide, transportation response time is directly related to the location / jurisdiction where an event occurs. Assist America is not responsible for failing to provide services or for delays in the delivery of services caused by strikes or conditions beyond its control, including by way of example and not by limitation, weather conditions, availability of airports, �ight conditions, availability of hyperbaric chambers, communications system, or where rendering of service is limited or prohibited by local law or edict.All consulting physicians and attorneys are independent contractors and not under the control of Assist America.Assist America is not responsible or liable for any malpractice committed by professionals rendering services to a member.

08 Medically recognized venereal or sexually transmitted diseases.

09 Costs of treatment following diagnosis of HIV or any disease related to HIV, including AIDS and its derivatives, alternatives, or other forms.

10 All costs related to tooth implants, dentures, �xed or movable bridges or orthodontic treatment, unless resulting from violent external means.

11 Vision or hearing correction tests and visual or hearing aids, unless requested by a licensed physician.

12 The bene�ciary’s transportation expenses within and between cities in the Kingdom by means other than ambulances of the Saudi Red Crescent or licensed ambulances.

13 Hair loss, baldness or arti�cial hair.

14 Psychological, mental or nervous disorders.

01 Intentional self-in�icted injuries.

02 Sickness resulting from abuse of some medicines, stimulants or tranquilizers, or from use of alcohol, narcotics and similar substances.

03 Cosmetic treatment or surgery unless necessitated by a bodily injury not excluded under these exclusions.

04 General checkups, inoculations, drugs or preventive measures not required for medical treatment covered under this policy (excluding preventive measures determined by the Ministry of Health, such as vaccination, maternity and child care).

05 Treatment received by a bene�ciary free of charge.

06 Rest cures, general health cures and treatment in social welfare institutions.

07 Any illness or injury resulting directly from the bene�ciary’s profession.

20 Organ or marrow transplant, or implant of arti�cial organs to replace any organ of the body.

21 Any activities known to involve high risk of exposing a person to an illness or an accident, or expected to aggravate a previous illness or injury.

22 Alternative medicine.

23 Arti�cial and ancillary limbs except those required by the bene�ciary as per a medical decision issued by the health care facility approved by the council.

24 Natural changes related to menopause, including menstrual disorders.

15 Allergy tests of any nature, unless relating to medicines, diagnosis or treatment.

16 Equipment, means, drugs and procedures, or hormone treatment aimed at regulating reproduction, contraception, fertility, infertility, impotence, secondary sterility, in-vitro fertilization or any other method of arti�cial fertilization.

Any congenital weakness or deformity unless it is life threatening, except for cases requiring treatment in accordance with a medical report issued by the health facility approved by the council.

18 Any costs or additional expenses incurred by the bene�ciary’s companion during a hospital stay, except for the hospital room and board charges for one companion such as a mother companying her child aged up to twelve years or whenever medically necessary as assessed by the attending physician.

01 War, invasion, acts of foreign enemy, acts of aggression (whether or not war is declared) or civil war. 02 Ionizing radiations, pollution from radioactive activity of any nuclear fuel or waste resulting from the combustion of nuclear fuel.03 Radioactive, toxic, explosive or other hazardous properties of any nuclear plant or any of its nuclear components. 04 Bene�ciary’s service or participation in armed forces or police activities. 05 Riot, disorder, terrorism, and similar actions. 06 Riots, strike or terrorism.

أي تكاليف أو مصاريف إضافية يتكبدها المرافق للمؤمن له أثناء تنويمه أو إقامته بالمستشفى عدا نفقة ا�قامة وا�عاشة

في المستشفى لمرافق واحد للمؤمن له، كمرافقة ا�م لطفلها حتى سن الثانية عشرة، أو حيثما تقتضي الضرورة

الطبية ذلك حسب تقدير الطبيب المعالج.

معالجة البثور (حب الشباب)

حاالت زرع ا�عضاء المنقولة والنخاع العظمي أو زرع ا�عضاء االصطناعية البديلة التي تحل محل أي عضو بالجسم.

ا�خطار الشخصية الواردة في قسم التعريفات من هذه الوثيقة.

أدوية ووسائل العالج بالطب البديل.

ا�طراف الصناعية وا�طراف المساعدة، ماعدا تلك التي

تتطلبها حالة المؤمن له بموجب قرار طبي بالمرفق الصحي المعتمد من المجلس.

عناية السعودية لن تغطي المطالبات التي تنشأ عما يلي:

ا�صابة التي يسببها الشخص لنفسه متعمدا

ا�مراض التي تنشأ بفعل إساءة استعمال بعض ا�دوية أو ئات أو بفعل تعاطي المواد الكحولية أو المنشطات أو المهد

المخدرات أو ما شابه ذلك.

الجراحة أو المعالجة التجميلية إال إذا استدعتها إصابة جسدية عرضية غير مستثناة في هذا القسم.

الفحوصات الشاملة واللقاحات أو العقاقير أو الوسائل الوقائية التي ال تتطلبها معالجة طبية منصوص عليها في هذه الوثيقة (باستثناء ا�جراءات الوقائية التي تحددها وزارة

الصحة مثال لتطعيمات ورعاية ا�مومة والطفولة).

معالجة الحمل والوالدة للمرأة المتعاقد معها على أساس أنها غير متزوجة.

المعالجة التي يتلقاها الشخص المؤمن له بدون مقابل.

االستجمام وبرامج الصحة البدنية العامة والعالج في دور الرعاية االجتماعية.

أي علة أو إصابة تنشأ كنتيجة مباشرة لمهنة الشخص المؤمن له.

معالجة ا�مراض التناسلية أو التي تنتقل باالتصال الجنسي المتعارف عليها طبيا.

مصاريف عالج الفترة الالحقة لتشخيص فيروس نقص المناعة البشرية HIV أو العلل ذات الصلة بهذا المرض بما فيها مرض ا�يدز (نقص المناعة المكتسبة) أو مشتقاتها

أو مرادفاتها أو أشكالها ا�خرى.

جميع التكاليف المتعلقة بزرع ا�سنان أو تركيب ا�سنان االصطناعية أو الجسور الثابتة أو المتحركة أو التقويم

باستثناء تلك التي تنشأ عن وسائط خارجية عنيفة.

اختبارات تصحيح النظر أو السمع والوسائل البصرية أو السمعية المساعدة ما لم تكن مطلوبة بأمر الطبيب

المرخص.

مصاريف انتقال الشخص المؤمن له داخل وبين مدن المملكة بوسائل نقل غير سيارات ا�سعاف التابعة لجمعية الهالل ا�حمر السعودي وسيارات ا�سعاف

المرخص لها.

تساقط الشعر أو الصلع أو الشعر المستعار.

المعالجة النفسية أو االضطرابات العقلية أو العصبية.

اختبارات الحساسية مهما كانت طبيعتها بخالف تلك المتعلقة با�دوية أو التشخيص أو العالج.

ال تقدم (أسيست أمريكا) خدمات في الحاالت التالية: • السفر المؤدي خصيصا للحصول على العالج الطبي.

• ا�صابات الناجمة عن المشاركة في الحروب أو التمرد. • إرتكاب أعمال غير قانونية.

• محاوالت إنتحار. • الحوادث التي تشمل على تعاطي المخدرات مالم يصف الطبيب هذه الخدمات

• تحويل العضو من منشأة طبية �خرى ذات إمكانات مماثلة مع تقديم مستوى مماثل من الرعاية الصحية.

ال تقوم (أسيست أمريكا) بإخالء العضو وإعادته: • بدون تصريح طبي.

• في حاالت ا�ضرار الطفيفة أو ا�صابات البسيطة مثل إلتواء المفاصل أو القصور البسيطة أو ا�عتالل المرضي الطفيف الذي يمكن عالجه على أيدي أطباء محليين وال يمنع العضو من متابعة رحلته أو العودة إلى الوطن.

• الحمل الذي تجاوز ستة أشهر. • في حاالت االضطرابات العقلية أو العصبية مالم يتم تلقي العالج في المستشفى.

الرحالت التي تتجاوز مدتها 90 يوما من ا�قامة القانونية دون إرسال إخطار مسبق إلى (أسيست أمريكا) (يتوفر شراء غطاء تأميني منفصل للمغتربين (تتوفر خدمات المساعدة في جميع أنحاء العالم، إلا أن زمن االستجابة للنقل مرتبط بشكل مباشر بمكان

ونطاق السيطرة حيث وقع الحدث. (أسيست أمريكا) ليست مسؤولة عن عدم تمكنها من تقديم الخدمات أو التواني في تقديمها بسبب أعمال ا�ضراب أو ا�حوال الخارجة عن سيطرتها. بما يشمل على سبيل المثال ال الحصر، أحوال الطقس وصالحية المطارات للتشغيل وأحوال الرحالت الجوية والحجرات مفرطة الضغط وأنظمة االتصاالت أو في حالة قصر الخدمة أو حظرها بموجب أي قانون

أو مرسوم محلي.

جميع مقدمو الخدمة متعاقدون مستقلون وليسو خاضعين لرقابة (أسيست أمريكا) وال تكون (أسيست أمريكا) مسؤولة أو تخضع �ي مسائلة عن أي خطأ مهني يرتكبه مقدمو الخدمة لÏعضاء.

• خدمة الترجمة الفورية واالستشارات القانونية تقوم (أسيست أمريكا) بإحالة ا�عضاد إلى مترجمين فوريين و؟أو موظفيين قانونيين حسبما يتطلب ا�مر.

• معلومات ما قبل القيام برحلة: توفر (أسيست أمريكا �عضائها معلومات شاملة عن أي بلد وذلك علي الموقع ا�لكتروني www.assistamerica.com تتضمن شروط التأشيرة، وتوصيات التحصين والتلقيح إلى جانب المستشارين ا�منيين �ي وجة سفر.

• االستشارة والتقييم وا�حالة الطبية: مركز عمليات (أسيست أمريكا) مجهز بهيئة موظفين مرخصين طبيا، يعلمون على التقييم االتصاالت الواردة من ا�عضاء ويقدمون التوصيات الفورية بخصوص أي حالة طارئة بما فيه ا�حالة إلى أطباء و/أو

مستشفيات مؤهلة حسب الحاجة.

• ضمان تلقي العالج بالمستشفى: تضمن (أسيست أمريكا) تلقي العالج الفوري بالمستشفى عن طريق التحقق من صحة تغطية التأمين الصحي أو بتسديد ا�موال الالزمة إلى المستشفى بالقدر المطلوب.

• ا�خالء الطبي في حالة الطوارئ: في حالة عدم توفير منشآت طبية تتوفر فيها الرعاية المناسبة محليا: تستعين (أسيست أمريكا) بأي من وسائل النقل والمعدات وطاقم العمل الالزم �خالء العضو الذي يمر بحالة طارئة إلى أقرب منشأة طبية قادرة

على توفير مستوى عالي من الرعاية الصحية.

• المراقبة الطبية: يقوم الموظفون الطبيون لدى (أسيست أمريكا) باالتصال الدوري بالطبيب / المستشفى التي تقدم الرعاية الصحية للعضو مع إرسال المعلومات إلى أسرته.

• ا�عادة الطبية: إذا كان أحد ما يزال بحاجة إلى مساعدة طبية بعد الخروج من المستشفى تقوم (أسيست أمريكا) بإعادة العضو إلى موطنه أو منشأة التأهيل مسحوبا بمرافق طبي/غير طبي حسبما يتطلب ا�مر.

• المساعدة في تأمين ا�دوية: إذا نسي أحد ا�عضاء دوائه أو فقده وهو بحاجة إلى وصفة طبية للحصول عليه أثناء السفر، تقوم (أسيست أمريكا) بمساعدته في تأمين الوصفة الطبية الالزمة.

• نقل رسائل الطوارئ : تقوم (أسيست أمريكا) بتلقي وإرسال رسائل الطوارئ الخاصة با�عضاء

• الزيارة �سباب إنسانية: إذا كان العضو مسافرا بمفرده وسيتلقى العالج في مستشفى �كثر من خمسة أيام، تقوم (أسيست أمريكا) بتوفير تذكره ذهاب وعودة من الدرجة السياحية إلى مكان. المستشفى �حد أفراد ا�سرة أو ا�صدقاء

يعمل موظفو مراكز عمليات (أسيست أمريكا) على مدار 24 ساعة يوميا ولمدة 365 يوما في السنة، ويضم فريق العمل موظفين طبيين مدربين متضمنا الممرضات وا�طباء يجيدون العديد من اللغات لتقديم االستشارات والمساعدة بشكل سريع واحترافي

في حالة الطوارئ.

بمجرد اجراء اتصال واحد بسيط بالرقم المدون على بطاقة التعريف الشخصي لديك من (أسيست أمريكا) يتم توصيلك على الفور بما يلي:

• شبكة عالمية من مزودي الخدمات الطبية المؤهلين مسبقا.

• متخصصون متمرسون في إدارة ا�زمات.

• مركز عمليات بتجهيزات متقدمة وإمكانات استجابة عالمية.

• مزودي خدمات ا�سعاف ا�رضي والجوي تقوم (أسيست أمريكا) بإجراء جميع الترتيبات مع دفع قيمة خدمات المساعدة التي

تقدمها دون سقوف على تكلفة تلك الخدمات. وال شك أن ذلك من شأنه أن يخفف من الصعوبات ويحدد من النفقات المحتملة المرتبطة بالطوارئ الطبية خارج الوطن.

من المهم االحتفاظ ببطاقات التعريف الشخصي معك طول الوقت بحيث يمكن االتصال بنا للحصول على خدمات (أسيست أمريكا) من أي مكان تحتاج فيه إليها.

إن (أسيست أمريكا) ليست شركة تأمين سفر أو عالج، إنما هي مزود خدمات طوارئ عالمية.@ وال تعتبر خدمات (أسيست أمريكا) بديال عن التأمين الصحي أثناء حاالت الطوارئ الطبية خارج الوطن. تعود جميع التكاليف الطبية

المتكبدة على وثيقة العناية الصحية الخاصة بك، كما تخضع هذه التكاليف للحدود الواردة بوثيقة التأمين الصحي الخاصة بك.@ تلتزم (أسيست أمريكا) بإجراء جميع الخدمات وتوفيرها. ال تقبل أي مطالبات بالتعويض عن خدمات المساعدة.

• رعاية ا�طفال القاصرين: تقوم (أسيست أمريكا) بإجراء الترتيبات الالزمة لتقديم الرعاية الصحية لÏطفال غير المصحوبين

بمرافق نتيجة لحالة طوارئ طبية، وتدفع كذلك تكاليف االنتقال المرتبطة بهذه الترتيبات.

• إعادة رفات الموتى: تقدم (أسيست أمريكا) المساعدة الالزمة فيما يتعلق بترتيبات إعادة جثمان العضو إلى وطنه في حالة وفاته. ويشمل ذلك ترتيب إعداد الجثمان للنقل وإصدار المستندات الالزمة، وتوفير وسيلة النقل الالزمة، إضافة إلي دفع نفقات

النقل.

• المساعدة في تحديد موقع الحقائب والمستندات المفقودة: تساعد (أسيست أمريكا) أعضائها في تحديد موقع الحقائب و/أو المستندات و/أو المتعلقات وا�غراض الشخصية المفقودة.

ماهي الموافقة الطبية المسبقة؟

تطلب عناية السعودية من مقدم الخدمة الذي تتعامل معه االتصال بها للحصول على الموافقة الطبية قبل تقديم خدمة محددة أو عالج معين.

ويتم اتخاذ هذا ا�جراء لضمان اÖتي:

• أن تكون مستحقا للعالج أو الخدمة وفقا لوثيقة التأمين والمنافع المتعاقد عليها.

• أن يكون العالج المقترح أو الخدمة ضرورية من الناحية الطبية.

• أن تكون تكلفة العالج المقترح أو الخدمة في حدود المعقول أو المعتاد مقارنة بمقدمي خدمات الرعاية الصحية.

• أن يكون متبقي لديك في بطاقة العضوية ما يغطي تكلفة العالج أو الخدمة.

• تعبئة نموذج التعويض النقدي الخاص بـ عناية السعودية با�ضافة إلى التوقيع والختم.• تقرير طبي مفصل (يقوم طبيبك المعالج بتعبئته مع توقيعه، وكتابة التاريخ، ووضع الختم).

• أصول الفواتير والسندات المفصلة.• الوصفة الطبية (إن وجدت)

• نتائج الفحوصات في المعامل/ التقارير التشخيصية (إن وجدت).

• البد أن يتم تقديم المطالبات في موعد ال يتعدى 30 يوما من تاريخ العالج داخل المملكة العربية السعودية، و60 يوما للخدمات

المقدمة خارج المملكة العربية السعودية.• تستغرق إجراءات المطالبة والتسوية 14 يوم عمل بعد استالم المطالبة والمستندات المطلوبة.

تعيين مندوبي مستشفيات في مواقع محددة لتقديم المساعدة والدعم لعمالئنا الكرام فيما يتعلق با�مور التالية:• ا�شراف على قضايا العمالء وحل المشاكل.

• تيسير إجراءات مواعيد ا�عضاء.• تقديم ا�رشادات حول ا�جراءات في المستشفى.

• إرسال طلبات الموافقات المسبقة.• تقديم المساعدة الفورية عند الطلب.

ويعني ذلك أن تقوم عناية السعودية بدفع قيمة كافة الخدمات المقدمة لك، فيما عدا نسبة التحمل التي يطلب منك سدادها (إن كان ذلك ينطبق على وثيقة التأمين الخاص بك).

يقدم البرنامج الذهبي خدمات عالية الجودة ومميزات تتماشى مع متطلبات العمالء. يستطيع عمالئنا الكرام االستفادة من خدمات العالج خارج المملكة وزيادة حد التغطية. يقدم هذا البرنامج حدود

تغطية أكثر من الحد ا�على المطلوب من مجلس الضمان الصحي ويشمل المزايا التالية

عدة اختيارات متاحة لعمالئنا بدأ من قائمة المستشفيات ومرورا بإضافة خدمات تغطية إضافية مثل جراحة الليزك. البرنامج ا�لماسي

يقدم حدود تغطية أكثر من الحد ا�على المطلوب من مجلس الضمان الصحي ويشمل المزايا التالية:

تغطية المرافق:إذا كان لديك طفل تحت ١٢ سنة، فستتحمل عناية

السعودية تكاليف إقامة مرافق مع الطفل أثناء فترة إقامته في المستشفى.

تغطية الحمل والوالدة:• جميع الخدمات الخاصة بمتابعة الحمل والوالدة

للسيدات المستحقات لهذه المنفعة، ويتضمن ذلك تكاليف الحمل، والوالدة الطبيعية، والقيصرية،

والمضاعفات المرتبطة بالحمل والوالدة بما فيها ا�جهاض القانوني (يرجى مراجعة جدول المنافع المرفق).

• كما يتم تغطية تكاليف ثقب ا�ذن لحديثي الوالدة من البنات وعملية الختان لÏوالد.

• وتظهر تغطية متابعة الحمل والوالدة على بطاقة العضوية مع عالمة تشير إلى تغطيتها (Y) تعني أن متابعة

الحمل والوالدة مغطاة، و(N) تعني أنها غير مغطاة.

تغطية ا�سنان:وتشمل الكشف، وا�شعة السينية، والخلع العادي والجراحي، وعالج الجذور، والحشو المعدني البسيط

والمركب، وعالج اللثة البسيط، وا�دوية الموصوفة و تنظيف ا�سنان مرة واحدة خالل السنة التعاقدية.

تغطية النظارات الطبية:وتشمل التغطية، الكشف، واختبار النظر، وعمل النظارة

الطبية بما فيها ا�طار والعدسات الطبية العادية.

الدفع المباشر:ميزة عدم السداد الممنوحة للشخص المؤمن عليه في

مكان مقدم الخدمة المحدد من قبل عناية السعودية حيث يتم تحميل النفقات كلها على فاتورة عناية

السعودية.

الشبكة:كل ا�طباء ا�وليين وا�خصائيين أو المستشفيات أو

مقدمي خدمات الرعاية الصحية الذين يشاركون في خطة التأمين لتقديم الرعاية الطبية. وتوجد قائمة

بالمستشفيات والعيادات مرفقة بهذا الدليل وهي عبارة عن شبكة مقدمي الخدمة.

الحالة المزمنة:حالة طبية أو وضع مرضي يدوم لفترة طويلة.

ا�مراض السابقة وا�مراض المزمنة:يتم تغطية جميع هذه ا�مراض، فيما عدا تلك المدرجة

في قائمة االستثناءات.

التقرير الطبي:هو عبارة عن مستند يقوم بتعبئته الطبيب، يشرح فيه

ا�عراض والتشخيص والعالج الموصوف.

الطوارئ:العالج الطبي المطلوب �حد ا�شخاص المؤمن عليهم

نتيجة لحادثة أو حالة تستلزم الرعاية الطبية الفورية.

عالج الحاالت الطارئة:التمتع بتغطية تصل إلى ١٠٠ ٪ من الحد السنوي المخصص

لك لخدمات تغطية الطوارئ المتاحة خارج الشبكة، مع مراعاة التغطية ا�قليمية بحسب ما هو موضح في

الجدول المرفق الخاص بالمنافع.

العالج في العيادات الخارجية:تغطي جميع إجراءات العالج المستحقة في العيادات

الخارجية ماعدا تلك المذكورة في قائمة االستثناءات.

نسبة المشاركة التأمينية:تحديد القيمة المطلوب سدادها لمقدم الخدمة عند تلقي

العالج في العيادات الخارجية أو يتم توضيح هذه القيمة على بطاقة العضوية بنسبة مئوية من تكلفة العالج،

ولكنها مقيدة بأال تزيد القيمة عن ١٠٠ ريال سعودي.

العالج في العيادات الداخلية:تغطي جميع إجراءات العالج المستحقة في العيادات الداخلية غير تلك المذكورة في قائمة االستثناءات.

نوع الغرفة:يتم تحديد الغرفة على بطاقة العضوية: إما جناح عادي، أو

غرفة خاصة، أو غرفة شبه خاصة، أو غرفة مشتركة بحسب مستوى التغطية الذي ينطبق عليك.

المؤمن عليه/ العضو:

الشخص (الموظف أو من يعول) الذي تشمله خطة التأمين.

رقم العضوية:هو عبارة عن رقم خاص يتم منحه لكل عضو مؤمن

عليه في عناية السعودية للتعريف.

ن له: المؤمعناية السعودية للتأمين التعاوني.

صاحب الوثيقة التأمينية:جهة التوظيف أو الشخص الذي تصدر باسمه الوثيقة.

(مثل الشركة المنتمي إليها(.

مجلس الضمان الصحي التعاوني:منظم التأمين الصحي في المملكة العربية السعودية.

مقدم الخدمة:يعني الطبيب، أو المستشفى أو المجموعة الممارسة أو

التمريض المنزلي أو الصيدلية أو أية منشأة أو فرد أو مجموعة أفراد تقدم خدمة الرعاية الصحية.

ا�سعار السائدة والمتعارف عليها:يعني النفقات الطبية التي تتفق مع مستوى الرسوم

المحددة من قبل غالبية ا�طباء المرخصين أو المستشفيات في شبكة العضو في المملكة العربية

السعودية.

وعالوة على ذلك، يعني أال يختلف العالج الطبي بشكل كبير مما يعتبره ا�طباء المرخصون معقوال ليكون المعتاد

والمقبول �ي خدمة بعينها.

المنافع:تعني نفقات توفير الخدمة الصحية التي تشملها التغطية

التأمينية ضمن الحدود المبينة في جدول الوثيقة.

منافع الرعاية الصحية المقدمة:توفر عناية السعودية المنافع ا�لزامية التي حددتها القواعد

التنظيمية لمجلس الضمان الصحي با�ضافة إلى مزايا إضافية حسب جدول المنافع الخاص بعضويتك.

جدول المنافع:الجدول المرفق الذي يحتوي على كل المنافع المنطبقة

على خطتك والذي يعرض الحد ا�قصى للمزايا التي نتحمل نفقاتها لكل عضو.

التغطية:القيمة ومدى المخاطرة التي تتحملها عناية السعودية.

االستثناءات القياسية:المنافع الصحية والخدمات المستبعدة من التغطية.

المطالبة:هي طريقة لتعويض النفقات التي يقوم المؤمن عليه

بسدادها لمقدم الخدمة.
