Bring The Following Documents with You On the Day of Your Interview:
• IL Driver’s License
• Certified Birth Certificate
• Social Security Card
• Court Purpose MVR [The most critical tool in the hiring decision]
We need to make you aware that your MVR (Motor Vehicle Report) is the most critical tool in the
hiring decision, as it confirms your qualifications and safe driving practices. Without it, no matter
how much we want to make you that on-the-spot offer, we will be unable to do so until your MVR is
reviewed. So please know that it is in your best interest to have that in hand on the day of your
interview.
To obtain a copy of your Illinois driving record, you can visit your local DMV in person. When you
appear in person, you’ll be asked for your ID and will have to fill out form DSD DC 164.
The cost is $12.00, and SCR reimburses you upon successful completion of SCR’s 4-week training
course.
MVR FAQ’s
What is an MVR & Why is SCR Requesting it?
MVR stands for Motor Vehicle Report. Your MVR, the single most important deciding factor in the hiring
decision, will confirm your qualifications and safe driving practices. In the same way that a Driver’s License is
required to drive a car, SCR requires an MVR in order to operate our vehicles. SCR will reimburse you upon
hire.
To ensure optimum safety for our clients as well as elite driver performance, you must meet the qualifying
requirements that are in place.
Can I Interview without my MVR and bring it back, I know it’s fine?
Even if you think that your MVR is perfect, we have seen so many candidates come in for the interview; we
fall in love with them and want to bring them on board ASAP. So they run down and get their MVR, only to
come back and find out that they had a violation that disqualified them. We just don’t want to see that
happen anymore. Not only does it break our candidate’s hearts, it breaks ours. So, in order to avoid any
set-backs or heart breaks, you really need to bring in your MVR.
Not Sure If You Have a Clean MVR?
Obtaining an Unofficial MVR to See If You Qualify to Be an SCR Elite Driver
If you’re just not sure what is on your MVR, you can try this FREE little trick. Your Insurance Agent. An easy
way to find out what is on your driving record is to ask your insurance agent. Agents have the ability to look
up your motor vehicle report and probably be willing to print you a copy. It may not be a certified copy, but
it will let you know the basics. Traffic violations, conviction dates, and accidents will all be available to
your insurance agent if they have access to your driver's license number. If you see anything on your
unofficial copy that you are not sure of, you can e-mail it to us and we would be happy to go over it and let
you know if we see anything that would potentially disqualify you.
How do I Obtain My MVR? In Person (Same Day) or Mail (10 days)
Unfortunately, the State of Illinois does not offer DMV driving records online. To obtain a copy of your
Illinois driving record, you can visit your local DMV in person or apply for a copy by mail. When you appear in
person, you’ll be asked for your ID and will have to fill out form DSD DC 164. The fee for an Illinois DMV
driving record is $12.00. You will be able to leave with a copy of the form that day. SCR will reimburse you
upon successful completion of the SCR 4-week training course.
To obtain your Illinois driving record through the mail, you must mail in a copy of form DSD DC 164 along
with your payment of $12. Instead of your local DMV, however, you’ll need to mail your request to:
IL Secretary of State Driver Analysis Section 2701 S. Dirksen Pkwy Springfield, Illinois 62723

Check for Errors on Your Driving Record
When you obtain your driving record, be sure to check it for errors. Errors may be present in a number of
ways. Points may not be reflected properly. There may be convictions noted on your record that were not
convictions. There could also be outdated information on your record. Any mistakes on your IL driving
record can affect your insurance rates. It’s essential to contact the DMV if you notice any incorrect
information on your record.
How Long Do Violations Remain on Your Record
Typically, violations like speeding will remain on your IL driving record for four to five years. If your license
was suspended or revoked for any reason, that information will be reflected on your record for about seven
years. Violations related to alcohol or drugs will remain on your Illinois driving record for life. If you have
specific questions about violations and how long they will stay on your record, you should contact the DMV.
Also, you may be eligible to reduce the points on your record by completing a defensive driving course.
Post Interview Guidance
Abstract Inquiry Guidance
Should there be an issue with your Driver’s License Renewal dates, you are going to have to obtain and
provide SCR with an abstract inquiry. You would need to supply an abstract if:
You did not renew your license before it expired
You may have held a license in another state
The abstract inquiry is free but can only be obtained at the below location.
Upon receipt of your abstract inquiry, please e-mail your abstract to: Lawrence Moore: [email protected].
Upon reviewing it, we can then reschedule your interview.
