Our Scope of ServicesAdult Day Health Care – Health care, social activities and support services. Six days per week.
In-Home Care – Providing care for you or a loved one in the comfort, privacy and safety of your own home.
Mondello Wellness & Family Center – Health screenings, education programs and other wellness-related activities for senior adults and their families.
Nursing Home Care – 24/7 medical care in a beautiful, home-like setting. Our nursing home care also provides:
• Hospice and Palliative Care – Comforting and specialized care for those facing a serious or life-limiting illness.
• Rehab – Specialized, short-term care for those with recent illness, injury or surgery.
Traumatic Brain Injury – Several programs including waiver services to address the cognitive, physical and emotional aspects of individuals with TBI.
Veteran Care – Services available for veterans at home, those in need of adult day health care, short-term rehab, or skilled nursing care.

Schofield Care has been serving Western New York for 110 years. Initially known as The Wheel Chair Home, founded by Dr. Jennie Schofield, Schofield has been a resident in the KenTon area almost since its inception. However, our reach goes beyond the village and town borders whether it be with residents in our nursing home, rehab patients, those receiving home health care services, or those who have participated in Scofield’s Adult Day Health Care programs.Due to the limitations created by the COVID 19 virus, Schofield Care commemorated this special anniversary by “hosting” a non-event event gala in October 2020. We are especially grateful to our sponsors, donors, friends and family members who contributed to this celebration. Their partnership impacts Schofield’s very important work in caring for the most vulnerable, frail adults we serve and helps provide the necessities for their comfort.Because of their generous support, we are able to purchase a new bladder scanner, 20 wheelchairs and 10 bariatric chairs.

New Training ManikinDue to the generosity of Schofield’s Board of Directors member Darlene Maclay and donor Cecelia Kohlmeier, we were able to replace our dated training manikin with a state-of-the-art model. Darcee, much more realistic of a resident, replaces our former 20 year old manikin. Madhulika Topolski, LPN, Schofield’s In-Service Coordinator, is very excited to welcome Darcee to her department. This modern day manikin enables Madhu to test skill competencies, provide guidance on Schofield’s Policies and Procedures when assisting and caring for our residents both in long-term and short-term care. IV’s, injections and wound care are just a few of the necessary features Darcee brings to training. Our nursing staff is most grateful to Darlene and Cecelia. Thank you!
