E������� W������� P������ + R������The Everyday Wellness program aims to support and encourage good health. You have theopportunity to earn up to $550 in wellness rewards for being in good health or working towardimproving your health. Earned rewards will automatically be deposited into your Health SavingsAccount (HSA) in January of each year for the prior year’s program. If you do not have an HSA through Schwab, the reward will be issued as a taxable cash payment in January.

PROGRAM ELIGIBILITY All full-time, part-time and seasonal Schwab employees hired by August 31, 2019 in the U.S. and Puerto Rico are eligible to participate in the program. Employees who are on a leave of absence are also eligible to participate. Contingent workers, interns, leased or temporary employees are not eligible to participate. To receive wellness rewards, you must be actively employed on January 1 following the program year and meet all program eligibility criteria. If you are unable to participate in the health screening due to a medical condition, please contact HealthCheck360 at 1.866.511.0360 or send an email to [email protected] to discuss alternative options.

*Puerto Rico employees can reach out to HealthCheck360 for participation details.*If you miss your screening due to an emergency (i.e. death in the family, sudden illness, natural disaster (hurricane), excused leave, last minute work travel), please check the online scheduler for an alternate screening location. If you are unable to reschedule, please contact HealthCheck360 support within 48 hours of your missed appointment. Please provide your original appointment date and time along with the reason for missing.

Earn $300 in Health Rewards when you complete a health screening, Health Risk Assessment (HRA) and meet the minimum health score requirements. NEW in 2019 for repeat participants, you must receive a minimum health score of 75 or improve your previous year score by 5 points.* Employees hired between September 1, 2018 - August 31, 2019 are provided a participation year and only need to complete the health screening and HRA to achieve the incentive.

The chart below outlines the steps to earn the reward.

* You are not required to complete the biometric screening or HRA; however, only employees who do so can earn the $300 Health Reward. Schwab is committed to helping you achieve your best health. If you participate in the program and are unable to meet the standard for a reward, you might qualify for an opportunity to earn the reward by different means. Contact HealthCheck360 at 1.866.511.0360 by October 31 and they will work with you (and, if you wish, with your doctor) to provide an alternative program to earn the reward.

Health Report + Health Score*

After you complete the HRA you can view your health report and health score. Your health report outlines the results of your health screening and provides you a health score based on your screening results. To earn the $300 reward, you will need to receive a minimum health score of 75 or greater or improve your most recent health score by five (5) points.

Biometric (Health) ScreeningA health screening is a medical assessment of your health. The first step consists of biometricmeasurements including height, weight, neck, waist, and blood pressure. The second stepincludes a venipuncture blood draw for HDL, LDL, Triglycerides, Total Cholesterol, Glucose, A1c, GGT, Cotinine, Creatinine, Albumin, ALP, Total Protein, ALT, ASP, GSL, Bilirubin, Globulin, and BUN. Onsite health screenings take place in all locations from August through October. Screeningregistration opens in late July.The information from your biometric screening and HRA will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as Health Coaching. You are also encouraged to share your results or concerns with your own doctor.

Health Risk Assessment SurveyThe HRA is a questionnaire that asks about lifestyle and certain medical conditions that can impactyour health. 7 - 10 days after you complete your health screening you will receive an email fromHealthCheck360 to complete your HRA. Your HRA must be completed by October 31, 2019.

If you participate in a health screening and HRA and don't earn the reward, you canparticipate in the Health Coaching Program to earn the $300 Health Reward. ContactHealthCheck360 at 866.511.0360 ext.5099 to register for the coaching program.

Health Coaching Program*

The Lifestyle Reward program allows you to focus on achieving your wellness outcomes for the HealthReward and take ownership of your health by deciding which activities best suit you and your lifestyle.Schwab will reward you for participating in eligible wellness activities by making a contribution to your HSA,or will issue a taxable cash payment if you do not have an HSA through Schwab. For a list of eligible activities, log in to your myHealthCheck360 account, locate "My Company" and click the link for additional information. Expenses incurred prior to your hire date will not be reimbursed. Expenses must be dated on or after hire date for all new hires.

Collect Proof of PaymentCollect receipts, statements, or other proof of payment for eligible wellness activity expenses incurred from November 1, 2018 - October 31, 2019.

New hires may submit eligible receipts for expenses incurred on or after their hire date within the program period listed. Receipts submitted for expenses incurred prior to your hire date are not eligible for reimbursement.

Sign into myHealthCheck360Sign into your account on the myhealthCheck360 app or at myhealthcheck360.com; Locate Lifestyle Rewards and upload receipts/proof of payment by October 31 , 2019*.

Receive Your RewardFor every eligible receipt you submit, Schwab willmake a contribution to your HSA or make a taxablecash payment for those who do not have an HSAthrough Schwab, up to a maximum of $250 forthe program year.

Protections from Disclosure of Medical Information

Schwab’s voluntary wellness program, also known as Everyday Wellness, is administered in accordance with federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990, the Genetic Information Nondiscrimination Act of 2008, and the Health Insurance Portability and Accountability Act (HIPAA), as applicable, among others.

Schwab, our group health plan, and its vendors, including HealthCheck360, Premise Health Holding Corp, and eHealthScreenings, are required by law to maintain the privacy and security of your personally identifiable health information, which is known as “protected health information” or “PHI”. You may provide PHI when participating in Everyday Wellness during your screening and when you complete the Health Risk Assessment (HRA), as described above. Your PHI will be used in compliance with applicable laws. Among other things, your PHI may be used to provide you with information to help you understand your current health and potential risks, to offer you services such as health coaching through Everyday Wellness, and to respond to your request for a reasonable accommodation to participate in the wellness program. Your PHI also may be disclosed to your group health plan and its vendors for healthcare operations, such as understanding the overall impact of the clinical programs offered through the group health plan and Everyday Wellness. In addition, Schwab may receive aggregate data for purposes of designing future wellness initiatives. Schwab will also receive a list of participants who earn the health reward so the health rewards may be paid. PHI will not be provided to Schwab and may never be used to make decisions regarding your employment.

Your PHI will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry out specific activities related to the group health plan or Everyday Wellness, and you will not be asked or required to waive the confidentiality of your PHI as a condition of participating in the wellness program or receiving the health reward. Anyone who receives your information as part of the wellness program will abide by the same confidentiality requirements.

In addition, PHI obtained through the Everyday Wellness will be maintained separate from your personnel records, information stored electronically will be encrypted, and no information you provide as part of Everyday Wellness will be used in making any employment decision. Appropriate precautions will be taken to avoid any data breach, and in the event a data breach occurs involving information you provide in connection with Everyday Wellness, we’ll notify you as soon as practicable.
