How to Book

To ensure the program runs according to schedule, you are required to arrive at the centre by 9.00am. All information for an excursion is found on the excursion page within attached documents. The anticipated number of children on excursions are 50 unless stated otherwise. I understand that due care will be taken by YMCA employees and that the employee cannot be held responsible for any damage or injury occurring during travel. There are no refunds if you no longer need care on a day that you have booked your child/ren in if they are booked in for an excursion. Any changes to the program will be informed via direct contact from the centre. Excursion risk assessments are available for review at the centre. Children attending all YMCA Vacation Care programs must wear a broad brimmed hat and enclosed shoes.

Please pack the child/ren a healthy morning tea, healthy lunch, healthy afternoon tea, a drink bottle, a broad brimmed hat, a jumper and appropriate shoes for daily activities. Children should wear t-shirts with sleeves. Please do not bring hot food or food that contains nuts.

• Please note: One weeks notice in writing is required to cancel your booking.

To cover staff requirements a fee of $2 per minute will be charged to your account after 6.00pm.

Fees:

All children will be accepted into the Vacation Care Program; however children who have not been immunised will be excluded from the program in the event of an outbreak of an infectious disease. Families must provide evidence of immunisation or have an exemption to be eligible for the CCS allowance.

The child/ren will be transported by Queanbeyan YMCA’s bus. This bus does not contain seatbelts.

Breach of YMCA Policies and Procedures YMCA Policies and Procedures are established for the safety and enjoyment of all children. Breaches of these policies and procedures may result in exclusions from activities or the program following consultation between parents/families and YMCA management.

Refunds or credits are not given unless a program is cancelled by the YMCA of Queanbeyan.

Please indicate immunisation status in the space provided on the Enrolment Form.

The daily cost is $48.00 per child from 8am - 6pm. Excursions are an additional fee from $5 - $30. Payments can be made by Cash / Cheque / Eftpos at the office or by Direct Credit to the Account. Excursions are to be paid upon Enrolment.

At the commencement of each morning you are required to sign the child/ren in on KIOSK. The child/ren must also be signed out and will not be allowed to leave the centre with any person other than those nominated on this booking form. Written notification is required to allow another person to collect the child/ren. Identification will be required.

I authorise the YMCA staff to obtain, at my cost, medical or ambulance assistance in the case of an accident or emergency. I hereby agree that the YMCA staff shall be released from and shall not incur any responsibility or liability whatsoever for any accident or injury sustained by any persons included on this application or for any damage to or loss of property.

Information on which supervisors are working daily and attending excursions is displayed in our centre. Our ratios for excursions will be in accordance with National Education and Care Regulations
