SCHEDULE – A (See Clause 10.1)

SITE OF THE PROJECT

1 The Site

1.1 Site of the Project shall include the land, buildings, structures and road works as described in Annex-I of this Schedule-A.

1.2 An inventory of the Site including the land, buildings, structures, road works, trees and any other immovable property on, or attached to, the Site shall be prepared jointly by the Authority Representative and the Concessionaire, and such inventory shall form part of the memorandum referred to in Clause 10.3.1 of the Agreement.

1.3 Additional land required for construction of works specified in the Change of Scope order issued under Clause 16.2.3 of this Agreement shall be acquired in accordance with the provisions of Clause 10.3.6 of this Agreement. Upon acquisition, such land shall form part of the Site and vest in the Authority.

Annex - I (Schedule-A)

SCHEDULE – A .

Site of the Project .

1.1 General Introduction

The Government of Madhya Pradesh is poised for rapid growth of infrastructure in the state. Medical Education Dept. through Madhya Pradesh Road Development Corporation (the Authority) intends for development of 150 seats Medical College of Graduate Level, associated 750 bedded Hospital Complex at Ratlam in the state of Madhya Pradesh on BOT (Annuity) basis. Ratlam is the administrative headquarters of Ratlam District. Adjacents to the District Ratlam are the boarding districts Mandsaur in north, Ujjain in East and Dhar and Jhabua in South. The western boundary of the district is shared with Rajasthan.

1.2 Topography of District

The District Ratlam is located in the north-eastern part of the Malwa Plateau.

1.3 Site & Project

The term “Medical Campus” used in this project broadly means that, Whole Project 150 seats Medical College of Graduate Level, associated 750 bedded Hospital Including Hostel complex, Residential complex, Guest House, Recreational Facilities and Site infrastructure development work which include road, sewer line, Drainage work, Parking Area, Landscaping, electrification, water supply, waste disposal systems etc. as per standard laid down by Medical Council of India (MCI) and minimum area requirement as laid in Schedule B, C & H, whichever is higher.

An index map showing location of Proposed Site of Medical Campus is enclosed as

Annexure A-1

1.4 Details of Site

a. There is not any Existing Building on Site. The project site measuring 14.70

Hectares has been identified for the Medical College and the teaching Hospital.

The site for the Proposed Govt. Medical College at Ratlam is located in Village

Banjali in Ratlam District. It is approximately 8km from Collectorate, Ratlam and

approx. 4km from air strip., Ratlam.

b. An inception Report of the Site including the land, buildings, trees and any other

immovable property on or attached to the Site shall be prepared jointly by the

Authority Representative and the Concessionaire and such inventory shall form

part of the memorandum.

The Concessionaire has to assess the Development of The infrastructure and requirements to meet the Specification and Standards proposed for the project.

c. Location & Categories of proposed Project

Proposed Medical Campus shall be constructed at location given in the Index

Map. This Project has been divided in 6 categories of development:

Hospital Block * Medical College block

Residential Block * Hostel Block

Commercial Centre * Infrastructure Development

2. Existing Condition Survey of Site, and other Features The land for the proposed medical college belongs to Government of Madhya Pradesh Medical Education Department and is made up of adjacent land parcels, the details of which are as below: Gram - Banjali Tehsil – Ratlam Jila - Ratlam Khasra Number - 225/1/1 Gram - Banjali Tehsil – Ratlam Jila - Ratlam Khasra Number – 46,47,48,49 The access to the site is via Ratlam – which is currently 6m wide road. Proposal for road widening is also under process. The topography of the land is plain and is devoid of any natural features like drain, water body or large trees except a few on the South boundary.

3.0 Disclaimer

The data presented in this schedule is for an initial understanding and guidance

of the Concessionaire. The data is based on Feasibility Report prepared. The

Authority will not be responsible for any inaccuracy in the information provided

and shall not be liable for or be bound by the data used by the Concessionaire

in building the Project. The Concessionaire will carry out his own Independent

Survey for assessing actual position of the Project Corridor.

The Concessionaire acknowledges that prior to execution of this agreement;

the Concessionaire has satisfied himself (based on his own Independent

Assessment) of survey data, co-ordinates, site and all information provided by

the Authority. The Concessionaire acknowledges and accepts the difficulties,

risks and hazards likely to arise during the course of performance of his

obligation hereunder.

Annex - I (Schedule-B)

SCHEDULE – B .

Development of Project .

1.0 General

Construction & Development of 150 Seat Medical College & 750 bedded Hospital

complex ‘Project’ proposed for development. The description of the requirements

for the various elements of Medical Campus Project given here in under are the

bare minimum for the ‘Project’. The ‘Project’ has the same meaning as defined in

RFP (Request for Proposal) . In planning, design and execution of the works in

connection with the development of this Project, the Concessionaire shall take all

such actions and do all such things as will enable the Authority to provide an

acceptably safe, Convenient in respect of its condition (structural safety).

Notwithstanding anything contained in this document, the Concessionaire shall

himself carryout and be responsible for engineering surveys, investigations and

detailed architectural & engineering designs and prepare the designs, working

drawings for all the components relevant for construction of 150 Seat Medical

College & 750 bedded Hospital complex and to fulfill the scope of the project as

envisaged herein under. These shall comply with manual of design specifications

and standards given in Schedule ‘D’. The designs for different project facilities

shall follow the locations given in Schedule ‘C’ and shall comply with design

specifications and standards outlined in Schedule ‘D’. All the designs and

drawings shall be reviewed by the Independent Engineer prior to execution.

2.0 CONCEPT OF MEDICAL CAMPUS

2.1 Concessionaire shall construct Medical Campus at proposed location indicated in Schedule A. Security gates shall be constructed to facilitate road users and to cut short time required for interconnectivity. Project shall have the combined facilities for Medical Campus in Blocks as mentioned below:- (1) Hospital Block (2) Medical College (3) Residential Block (4) Hostel Block (5) Commercial Complex (6) Infrastructure Development a. The project consists of entire site Area, parking area, Landscaping, Road, exit and entry area and the building like Hospital Building, Medical College, Residential Complex, Hostel, Rain Basera etc. b. The facilities centre consists of project facilities described in Schedule ‘C’.

2.2 BROAD SCOPE OF WORK

The scope of work for Project includes but not limited to the following:

2.2.1 Project Scope includes the following

Design, engineering, procurement, construction, Operation and maintenance

during the concession period of required building complexes, as mentioned in

Section 3. 0 of Schedule B.

Design, Engineering, construction and maintenance of project infrastructure

facilities of entire medical college campus area as mentioned in Section 3.0 & 4.0

of

Schedule B.

Construction of compound wall along periphery of the premises of the

Construction Site along with aesthetical entrance gate to Hospital, College and

Residential complex, road signage, and road furniture’s, etc as required for the

successful execution of the project as mentioned in Section 4.0 of Schedule B.

All Building shall be constructed in accordance with standards & Specifications

defined in Schedule D.

All buildings shall have following

o Internal & external Water Supply

o Internal & external Plumbing & Sanitary work

o Water proofing treatment

o Internal & external Electrification

o Internal & external Communication between all Booths & Office Building

o Provision for complete internal furnishing i.e. furniture & fixtures**

o Fire fighting system shall be installed in medical college, hospital building & hostels. Rest of the buildings will have fire fighting equipments.

o Provision for hot water supply in all residential buildings, through Solar Hot Water systems.

o Anti-termite & anti-fungal treatment for the whole site & all building blocks

o Air-conditioning in Hospital building & college building in areas such as OPD/ OT-complex/ ICU/ Laboratories/ Auditorium/ Conferences/ Library etc and area should not be less than 40 % of total building area.

o Medical college & hospital complex should have all necessary provisions & cables/ connections for electrical lines, gas lines, ducts , piping , cables for various electronic equipments and systems in operation theatres , ICUs , wards etc

The specifications are prescribed in Schedule D.

(** Furnitures & Furnishings: Concessionaire’s scope doesn’t include furniture items (wooden etc). However, he shall fully be responsible for making necessary provisions in civil works to receive the furniture items, medical equipment, devices and other support services equipment brought out later by college or hospital authority. Moreover, the electrical provisions (within scope of concessionaire) will have to be made such that they are compatible with appropriate furniture & equipment layout. For this, the concessionaire shall submit the detailed furniture & equipment layout for approval from the Authority.

2.2.2 Buildings & Building Management System

Concessionaire shall undertake Construction, Operation and Maintenance of all buildings like Medical College, Hospital, Hostels, Residential complex, Commercial complex, Raen-basera, Sulabh etc at the Project site and Construction, Operation and Maintenance of project infrastructural facilities through BMS.

2.2.3 Electrical Works

The scope of work for electrical works includes, Supply, installation, testing, commissioning, replacement, Operation, maintenance and payment of deposits, charges, all required consumables during the concession period of required electrical equipments as mentioned in Section 6.0 of Schedule B.

2.2.4 Operation & Maintenance: Concessionaire will keep the building neat & tidy & properly maintained in terms of repairs/ painting/ rectification works etc. & replacement of damaged/ defective items, during the entire concession period, as per schedule-K. For this purpose, he will submit detailed Operation and maintenance plan to the Authority for approval. & Concessionaire shall depute (at his cost) adequate team of people OR expertise agency for these works. The credentials & experience of these would be verified by the Authority before approval.

2.2.5 Project Facilities The concessionaire shall construct the project facilities prescribed in Schedule-C; briefly described below to form part of college campus Project. The project facilities shall include:

Toilet block (SULABH)

Tree plantation / Arboriculture

Water supply.

One number hard top four wheeler Vehicle to the Authority during

the Development and Construction Period.

Drinking Water [as per norms laid by Health department]

Security of site : round the clock

The facilities and amenities will be comparable to global standards. It is extremely important that the facilities erected are architecturally beautiful,

aesthetically pleasing, environmentally friendly, functionally efficient and operationally convenient as well as safe, in accordance to schedule-D.

3.0 DETAILED SCOPE OF WORKS

The proposed 150 seated medical college & 750 bedded hospital complex shall be constructed by Concessionaire as per layout plan given in Schedule H including all the detailed facilities, amenities & infrastructure. The campus shall include all the building blocks as per design brief & the infrastructure facilities which shall be constructed as per Standards & Specification of Schedule D.

3.1 Proposed Area for Project

Area Specifications for the building blocks of the project shall be in accordance

with the detail drawings, designed by concessionaire & approved by Independent

Engineer / the Authority, and in accordance with the drawings given in schedule-

H, subjected to a minimum of proposed area given in tables below.

Concessionaire shall construct & maintain building as per proposed area given

below:

3.2 General Layout Plan of Medical college campus

The proposed 150 seated medical college & 750 bedded hospital complex shall be constructed by Concessionaire as per General layout Plan with all the detailed facilities, amenities & infrastructure. The drawing layout plan showing buildings & facilities are prescribed in Drawings-Sch`H’ of Concession Agreement. The road works, buildings, Structures & Facilities shall be constructed as per Standards & Specification and minimum design Criteria mentioned in Schedule D. However, the Concessionaire will prepare the specific detailed layout plan based upon site conditions and availability of land, prior to execution and seek approval of Independent Engineer on specific layout plan, as per the provision of Concession agreement. Concessionaire shall construct, housekeep and maintain following building complexes as per MCI norms and minimum area requirement as laid Schedule B, C & H, whichever is higher according to standard & specifications mentioned in Sch`D’.

4 INTERNAL ROAD WORK & RELATED INFRASTRUCTURAL WORKS 4.1 General

It is proposed to design and provide medical college campus on NH-86 at Ratlam. The site for medical college campus is large enough in size (12.45 Hect.) depending upon the traffic flow and requirements of user departments. The interlinking roads have been divided into 4 categories varying in road-widths according to traffic volume.

4.2 Proposed Infra Construction for the Project

The proposed civil construction for road shall be in rigid pavement for all category of inter-linking connectivity roads as mentioned below:- The design criteria of Geometric & Pavements design for rigid pavement shall be adopted as per Schedule D. The layout drawings of each categories of road network for these components shall be followed.

4.3 Internal Roads

Road construction for internal roads to residential blocks and pavement shall be in flexible pavement. The carriageway of road shall be at-least 7.0 Mtr. minimum where the width is less than that, width for internal roads in residential area and its connectivity to other facility shall be 3.75 Mtr. The width of main road with rigid pavements have been proposed for construction as per the layout drawings approved:

6m wide CC roads : 2066m length

9m wide CC roads : 450 m length

12m wide CC roads : 230 m length

18m wide cc roads : 128m length The layout drawings for above components shall be as mentioned in Schedule H. The design criteria of Geometric & Pavements design will be as per Schedule D.

4.4 Parking area will be as per drawings in Schedule H.

Surface parking for all vehicles , ambulances, 2 wheelers, cars all buildinga and stilt parking for residences and hostel blocks shall be constructed.

4.5 Cross Drainage Structure

A.) Inside Campus:

CD works will necessarily be designed as RCC Boxes in entire medical college campus project area. The Concessionaire will have freedom to train the streams and seek approval of Independent Engineer on design of such CD works, following the provision of Concession Agreement. The design of CD works will follow the standards described in Schedule D.

B.) Outside & around Campus:

Existing CD structures outside & around the college campus site, including pipe/box culverts shall be widened & new CD works shall be constructed as required. This shall be within the scope of concessionaire & work shall be carried under approval of Independent Engineer.

4.6 Traffic Signage & Overhead Gantry For safety & guidance of traffic signage & overhead gantry boards shall be provided by the Concessionaire for different complexes of Medical college campus buildings as mentioned in Tables A(i) to A(v). Road devices like blind-corner glass shall be provided on all the intersections as per estimated traffic on the roads.

4.7 Safety Devices Concessionaire shall provide kerbs, Parapet, Railing, guard stones, crash

barriers where ever require to ensure safety & orderly behavior in the medical college premises. The design of the safety devices will be approved by Independent Engineer as per provision of Concession Agreement.

4.8 Storm Water Drainage

Storm water & road side drainage shall be discharged through Concrete Drains of properly designed size as per Standards & Specifications of Schedule D. Proper drainage arrangement in the entire premises of medical college campus & discharge of entire drainage in nearby stream through concrete drains shall be the responsibility of Concessionaire.

4.9 Landscaping & Boundary Wall

Concessionaire shall provide & develop adequate landscaping of aesthetically pleasing view. The Concessionaire shall construct RCC boundary wall of at least 1.5mt height in civil finish & an additional height of 0.6m in Barbed wire fencing along the entire perimeter of the project. Length of boundary wall is 2500 m. Details and list of area of landscaping shall be developed as given in schedule-H.

4.10 Green Building Systems

The concessionaire has to achieve a minimum of 3-star Rating from GRIHA (Green Rating for Integrated Habitat Assessment) in appropriate category of buildings. For this purpose, the concessionaire will have to follow building techniques / material selection in accordance with the norms laid down by the Green-building rating systems.

5 Electronic Equipments

The scope of work for electronic equipments works includes but not limited to procurement, installation, integration, any related civil works , cables ,wires , poles/brackets etc, testing , approval from competent authority , commissioning, replacement ,Operation, maintenance and consumables during the concession period of required electronic equipments for the project as required for the successful operation of medical college & hospital during entire concession period.

5.1 Additional Electronic Equipments for safety and Maintainance shall include the Items listed in Table below:

6.1 Electrical Equipments:

a. Internal & External Electrification of all buildings.

b. Electrification/ provision for all hospital equipments & task lightings

c. Electrification – of internal / external / area lighting of the project, parking areas,

and all buildings.

d. Electrical substation / transformer of required capacity.

e. The street light arrangement inside the entire complex and area lighting around

the Building / complex.

f. Diesel Generator set of 80% capacity requirement of entire medical college

campus.

g. Air conditioners in administrative block, Hospital areas library & auditorium etc.

h. EPBAX Telephone for internal communication between administrative buildings,

Hospital building, Hostel blocks, Residential blocks & security gates etc.

6.2 All charges shall be paid by the concessionaire to the power supply company for obtaining supply from existing system, security deposits for meters etc.

7 Human Resource Requirement

7.1 Concessionaire will be responsible for assessment & deployment of the qualified manpower as required for the Operation & maintenance of the project during the concession period. Concessionaire will be responsible for the employees deputed on project during construction as well as concession period and compliance of the employment acts and provisions. All claims of the employees will be borne by the concessionaire and it is clarified that authority will not be accountable for any claims/liability on accounts of human resource deployed for the project. The list of qualified staff shall be submitted by Concessionaire & to Independent Engineer approved by the Authority

7.2 Concessionaire will need staff like Computer Engineer, Operator, Security Staff ,

Building Maintenance Staff , Landscaping Maintenance Staff , Maintenance staff for all Services , Waste Management Staff and Vehicle Regulation Staff etc. or any other requirement the actual strength shall be assessed by Concessionaire and will be maintained at the Project site after approval from IE/the Authority.

2.0 The concessionaire shall construct the project facilities described below to form part of Project. The project facilities shall include:

f) The facilities and amenities will be comparable to global standards. The

amenities shall offer the users of the Medical Campus necessary and suitable

facilities etc. It is extremely important that the facilities erected are

architecturally beautiful, aesthetically pleasing, environmentally friendly,

functionally efficient and operationally convenient as well as safe.

Development of internal roads on the campus, as per traffic volume & requirement. Major components of the work/scope of work are as under: 1. Preparation of detailed conceptual layout plan and detailed architectural and

structural design , services drawings and detailed working(execution)drawings , shop drawings , drawings and documents required for various approvals .

2. Submission , liasoning and procurement of all statuary approvals , as required .

3. Construction and establishing of wayside amenities including construction of Parking, Rest Area, park (entry/exit) to the facility for the Project Area.

4. Operating and maintaining the above facilities during the Concession period.

1.1 FACILITIES

The concessionaire has to provide basic facility centre at the prescribed location marked as facility area only. The facility centre should have provision for:

a. Shopping Facilities

a 1 Shoping Complex,

a 2 Medical Shops

a 3 STD/PCO/ Cybercafe

b. Raen basera.(For Guest)

c. Sulabh Intnl

d. Sub Station

e. Incinerator Building

f. Banks/ATMS

g. Guest house

h. Canteen

Operation & maintenance for such supporting facilities shall be under the scope of the concessionaire except the Operation and maintenance of clinical equipment and its auxiliaries, clinical supplies, hospital linen, upholstery, loose and fix furniture, all waste management excluding sewage system, dietary services, laundry services, clinical services, para-clinical services and housekeeping.

1.2 LANDSCAPING (AUTO-IRRIGATED) & TREE PLANTATION.

3.1 The Concessionaire shall Develop the landscaping, plantation trees of required number and type at the appropriate location within the land of the Project. The Independent Engineer, shall specify the number of trees to be planted with species, the Concessionaire shall also maintain the trees and shrubs in good condition during the concession period as per the maintenance schedule. The setback distance of trees and other plantation, setback of trees on curves, vertical clearance of trees and other plantation, choice of trees will be as per approval of Independent Engineer. 3.2 The Concessionaire will use atleast 15% of the Project area for plantation and landscaping. The plantation scheme proposed by the Concessionaire should be prepared keeping in mind the local conditions and climate. The plantation scheme will be approved by Independent Engineer.

1.3 PLAYGROUND & JOG-TRACKS

The concessionaire shall provide for playground & amenities as per building norms as per master plan layout in schedule H . he shall be solely responsible for their maintenance and caretaking throughout the concession period.

1.4 PARKING AREA DEVELOPMENT

The concessionaire shall provide for surface , plinth & basement parking as per building norms wherever given in detailed layout plan in schedule H . He shall be solely responsible for their maintenance and caretaking throuout the concession period.

1.5 WATER SUPPLY INCLUDING DRINKING WATER

Specific arrangements will be made for round the clock running potable water supply and water for other facilities in adequate quantities. Concessionaire shall make suitable arrangement for source & storage of water. 1.6 SEWAGE SYSTEM & SEWAGE TREATMENT PLANT

STP & Sewage system of required capacity shall be assessed by the concessionaire and the entire system shall be procured , installed , integrated and kept in a running condition and well maintained by the concessionaire. The design for proposed STP shall be detailed by concessionaire as per relevant code of practice and approved by IE / the Authority.

1.7 STORM WATER DRAIN

Storm water & road side drainage shall be discharged through Concrete Drains of properly designed size as per Standards & Specifications of Schedule D. Proper drainage arrangement in the entire premises of medical college campus & discharge of entire drainage in nearby stream through concrete drains shall be the responsibility of Concessionaire. 1.8 RAIN WATER HARVESTING

Rain water harvesting shall be provided in Entire Block & Facility Centre by the concessionaire. Required number of discharge pits shall be arrested by the concessionaire so as to keep the rain water harvesting system running properly & well maintained throughout the concession period. 1.9 ELECTRICFICATION

The scope of work for electronic equipments works includes but not limited to procurement, installation, integration, any related civil works , cables ,wires , poles/brackets etc, testing , approval from competent authority , commissioning, replacement ,Operation, maintenance and consumables during the concession period of required electronic equipments for safety & maintenance of the site as required for the successful operation of medical college & hospital during entire concession period 1.10 SOLAR WATER HEATER SYSTEM

All hot water requirement shall be assessed by concessionaire & solar water heater system shall be procured , installed , integrated & maintained by the concessionaire on roof top of each building .

1.11 SOLAR PV SYSTEM

The concessionaire with procure , install & integrate for at least 3% of total connected load on solar PV. Concessionaire shall make suitable arrangement for storage , source and utilization of electricity generated and the whole system shall be maintained and kept in running condition by the concessionaire.

2.2 Central Control Facilities.

(i) Concessionaire shall design , construct and maintain a Control Facility for all infrastructure including building , services and amenities. The built-up area to be constructed by the concessionaire for the above mentioned facilities centre shall be indicated on drawings by the concessionaire and got approved by the IE/the Authority.

(ii) The Concessionaire can create additional facility in the Project area as per provision of Concession Agreement and cost should be borne by concessionaire. No additional cost will be reimbursed by the Authority.
