Annexure – 01
ELIGIBILITY CRITERIA AND SPECIAL TERMS AND CONDITIONS
A. Eligibility Criteria for Tenders
1. The bidder / OEM/ OEM authorized suppliers should be in existence for the last 5 Years as
on bid calling date. Bidder should submit Manufacturer’s Authorization form specific to
this tender.
2. The Bidder shall submit Document fees of Rs.1000/- (Non-Refundable) infavour of
Chief operating Officer, APDMP, Guntur in the shape of DD.
3. The Bidder shall submit the EMD of Rs. 25000/- infavour of Chief operating Officer,
APDMP, Guntur in the shape of DD/BG.
4. The Bidder/OEM should have earlier supplied the same quantity of the offered brand/ items
being in the current tender in any of the last two years to State/Central Govt.
Organizations/Nationalized Banks/Public Sector Undertakings/ Reputed Organizations.
5. The Bidder/OEM should have at least three Service Center across the state of Andhra
Pradesh.
6. The Bidder should be a Manufacturer or the OEM Authorized representative of the
equipment in the schedule.
7. The Bidder should have turnover of Rs.20crore in each of the last three Financial
Yearsi.e.,2014-15, 2015-16 & 2016-17.
8. The bidder should be a profit making company and shall have a net worth of 5 crores in the
last 2 financial years.
9. The bidder or OEM should not have been debarred / blacklisted by any State Government /
Central Government / PSU / Reputed Organizations for any reason in the last three financial
years as on bid calling date. A letter of undertaking on the letter heads of the Bidder/
Manufacturer should be submitted separately stating that they have not been blacklisted by
any State Government / Central Government / PSU for any reason in the last three years and
should be signed by an Officer competent and having the power of attorney (with Company
stamp) to bind the bidding Firm / Manufacturer(s).
10. The Bidder shall file the price bid inclusive of all taxes and all other costs.
B. SPECIAL TERMS AND CONDITIONS:
Delivery Period: The proposed activity of supplying equipment at the institution should be made within 30 days from the date of receipt of purchase order by the Bidder. Warranty: All items supplied by the Bidder shall be guaranteed against any defects and the Bidder should provide time-to-time operational maintenance support for a period of 3 years (On Site Comprehensive Warranty). Bidder shall be liable to rectify any defects that may be found in the equipment supplied free of cost. Installation: The installation should be done at as stated in the Annexure – 03 enclosed, free of cost. Resolution Time: The response time of the Bidder to resolve any complaint upon receipt of the complaint/information from the user should not be more than 3 days. The successful bidder shall submit the Performance Bank Guarantee @5% on Purchase order value pledged in the name of Chief Operating Officer, Guntur Validity of the Bid: 60 days from closing date of the bid The prices offered in the price bid shall be inclusive of all taxes and all other costs. Payment terms: a)85% of the invoice value shall be paid against submission of delivery challans duly acknowledged by the respective recipients and 5% of the PBG of the total value of the invoice b)The remaining 15% of the invoice value shall be paid against submission of satisfactory installation reports from the respective recipients. C. Bid criteria: The Bids shall be submitted as three part bid as mentioned below a)Pre-Qualification bid (The cover should contain all the necessary documents mentioned in the point A of Annexure-I) b)Technical bid(The cover should contain Technical compliance statements and data sheets of the offered products, no lower deviations accepted) c) Price Bid: The cover should contain only prices of the all the items inclusive of all the taxes and all other costs. All the above three covers (a,b,c) should be properly sealed and labeled mentioning all the particulars and put them in another big cover which shall be sealed and labeled with all the particulars . D. Documents to be furnished The following documents should be submitted along with the bid, otherwise the bidders are liable to be disqualified.
1. All the supporting documents in respect of Eligibility Criteria. i.e.,
a. Registration/Incorporation Certificate in support of the Existence of the company for
required number of years as per the tender schedule eligibility criteria.
b. Copies of Purchase Orders in support regarding the supply of required quantities of
items in any of the last two years as per the tender schedule eligibility criteria.
c. List mentioning the addresses and contact persons with phone numbers of the Service
Centers present throughout A.P.
d. Supporting Documents indicating that the Bidder is the Manufacturer or the OEM
Authorized representative of the corresponding items/products.
e. Audited financial statements that are balance sheet, income & expenditure and profit &
loss accounts in support of having required turnover in each of the last three Financial
Yearsi.e.,2014-15 ,2015-16 & 2016-17.
f. The bidder should be a profit making company and shall have a net worth of 5 crores in
the last 2 financial years
2. Annexure – 02 duly signed & office Seal affixed as a token of acceptance of Special
Terms and Conditions.
3. Documents in support of Technical Specifications for equipment as mentioned in
Annexure – 05 clearly mentioning the make, modeland Specifications.
4. The latest Income Tax Clearance Certificate.
5. The list of customers, to whom the bidder had supplied identical materials in the past
along with P.O. details and performance report, if any.
6. The letter of undertaking should be enclosed

1. The organization (APDMP) will not hold any risk and responsibility for non-visibility of the
scanned document or non-receipt of hardcopies or loss in transit. 2. The Bidder will be disqualified at any stage of the tender process, if found to have misled or
