• If you have a change of address, we will require for you to fill out a new Residency Documentation Form and provide us with a copy of a current document which lists the parent/legal guardian’s full name and residential address. Documents accepted as proof of address are: current utility bill, current payroll stub, 2020 W2, current property tax bill, mortgage documents, residential rental or lease agreement, current documentation from a state, tribal or federal government agency or state issued ID or driver’s license showing current address.
• We will need a copy the parent/guardian state issued ID or driver’s license.

Each statement below should be read and initialed by either a parent or the child’s guardian. Signature by a parent or the child’s guardian is required.

I understand that this procedure change is for the safety of all persons present at the facility/school and to limit to the extent possible everyone’s risk of exposure.

Una biblioteca escolar es un lugar muy especial. Es aqui donde compartimos y gozamos de los libros y formamos el habito de leer que estara con nosotros de por vida. Les permitimos a los estudiantes sacar libros, no solo para reforzar sus estudios, sino tambien para que los disfruten en casa. Su estudiante traera libros a casa regularmente. Ustedes pueden ayudar a su estudiante mostrandole interes en los libros que trae a casa y pasando tiempo escuchandolo leerlos en voz alta.
Favor de ayudarnos protegiendo y devolviendo los libros a tiempo. Si algun libro es robado, maltratado o extraviado mientras esta a su cargo, ustedes tendran que pagarlo. Ustedes nos pueden ayudar cuidando los libros, tanto en su casa como en el camino de la escuela a casa y de casa a la escuela. Protejan los libros, por ejemplo, con una mochila o bolsa de plastico. Vamos a hablar con su estudiante sobre estos reglamentos y ayudenlo a obedecerlos en casa. Santa Maria Middle School Dress Code 2021-2022
All students attending kindergarten through eighth grade in the Fowler District will be required to wear school uniforms. The clothing may not be altered in a manner that includes, but is not limited to, slits of shredded hems, logos, other than the school mascot (when allowed), or is disruptive to the educational process. Bare midriffs are not allowed. Some exceptions may be allowed due to religious reasons. The standard school clothing is:
TOPS:« Colors must be white, light blue or navy» No denim may be worn® Shirts must have sleeves• Polo shirts with buttons, or regular shirts and must have collar and
sleeves® Shirts may be un tucked but must be no longer than the top of the
hand when placed to the side» Must not have any logos larger than a quarter or writing on them
other than school mascot when allowed• Must be size appropriate• Undershirts worn beneath tops must be white and not visible
below the outer shirt.• A uniform shirt is to be worn under sweaters, jackets, or other
outerwear.e Uniform shirts must be worn at all times, unless otherwise
approved by the principal.BOTTOMS:• Must be navy blue® No denim may be worn® No cargo pants nor cargo shorts• Students may wear shorts, skirts, skorts, or slacks that conform to
the 3-4 pocket tailored uniform style• All shorts, skirts and skorts must be no shorter than two inches
above the knee• All shorts must be no longer than two inches below the knee• Belts must have no logos® Belts may not hang down below the waist line.• No logos larger than a quarter or writing on bottoms® Must be size appropriate• Pants must fit at the waist and may not drag the ground - NO
SAGGING or BAGGING• Sweatpants, spandex, joggers, and tight fitting clothes are not
permissible• Leggings worn under skirts or skorts must be white, black, or navy
blue• Gym shorts may not be worn under uniform pants.• Uniform bottoms must be worn at all times, unless otherwise
approved by the principal.FOOTWEAR:• Shoes are:
• To be closed toes• To have laces (if applicable)• To have same laces on both shoes.• Not to have writing on them.• To have tongues that are under the tied laces• To be the same on both feet• Socks are to be the same on both feet.
OUTERWEAR:• Must be solid white, light blue, or navy no logos larger than a
quarter• Sweaters and sweat shirts may be worn in the building• Sweater hoods may not be worn on the head while inside the
buildings• Windbreakers, jackets, coats, and any item with a zipper are to be
worn outside of the building only. It must be removed once the student is in the building.
OTHER UNIFORM ISSUES:• No piercing in nose, brow, tongue, or other visible parts of the body
(other than ears).• Tattoos are not to be visible at any time.• Hoop earrings shall be no larger than a fifty-cent coin• Earrings shall be no longer than 1 inch long. (i.e. tapers)• Make-up products shall be left at home• Hats, visors, bandanas, hairnets, knit stocking, and/or caps are not
to be brought to or worn at school.
Dress Code ViolationsFootwear must be worn at all times. Safety requirements for specific classes such as industrial technology, life management, physical education and science shall be followed.
Gang clothing/paraphernalia is prohibited on school property, in school buses and/or at school activities. Gang paraphernalia is defined as any apparel, altered apparel, jewelry, accessory, notebook, or manner of grooming which, by virtue of its color, arrangement, trademark, or any other attribute which may denote membership in a gang.
The school reserves the right to further define appropriate dress for the students. Santa Maria Middle School has established the following site guidelines for student dress.
Students that are out of uniform will be directed to bring their attire into compliance with the dress code, and the following steps will be taken.
Students will be allowed to call parents for a change of clothing. If the parent cannot come, the student will be required to change into 'loaner' attire if it is available. The student will not be allowed back in class until the clothing is changed.
Dress code violations 1- warning and parent call
Dress code violations 2-3 will result in a referral for non-compliance with disciplinary points and the corresponding consequence.
Dress code violation four (4) will result in a referral, disciplinary points and a two-day off campus suspension.
Dress code violations five (5) and above will result in a referral, disciplinary points and a five-day off campus suspension.

CAMISAS:• Puede ser blanca, azul claro, o azul marino• Deberan tener mangas• Camisas estilo polo y camisas con botones deberan tener
cuello y mangas• Las camisas pueden estar desfajadas, pero el largo no puede
estar debajo de la muneca cuando se pongan las manos en los costados
• No logotipo mas grande de una moneda de 25 centavos o escritura en las camisas a no ser la mascota de la escuela cuando se les permita
• Debe sertalla apropiada• No mezclilla o nada que se le parezca• Las camisas por debajo de la camisa de uniforme deben ser de
color bianco y no visibles• Camisa de uniforme debe ser usada bajo sueteres, chamarras,
abrigos, sudaderas, o prendas de invierno• Camisas de uniforme deben ser usadas en todo momento, a
menos que de otro modo aprobado por el director.PANTALONES:

Violaciones al Codigo de Uniforme
Zapatos se deben usar siempre. Deben ser de tai estilo que los estudiantes puedan moverse seguros por sus clases. Son requeridos para seguridad del estudiante en clases especiales tales como educacion fisica y ciencias naturales.
Articulos relacionados con o usados por pandillas son prohibidos en la propiedad, en los camiones y en las actividades de la escuela. Articulos usados por pandillas estan definidos como ropa, ropa alterada, joyeria, accesorios, libro de notas o maneras de actuar, los cuales, por virtud de su color, forma de usarlo o marca, o cualquier otro atributo que denota la membresia en una pandilla.
La escuela se reserva el derecho de definir que es la vestimenta apropiada para los estudiantes. La escuela Santa Maria ha establecido las siguientes pautas de vestimenta:
Los estudiantes que no esten en conformidad con el codigo de uniforme seran dirigidos a cambiarse y estar en conformidad con el codigo de vestimenta, y se tomaran las siguientes medidas.
Se le permitira al estudiante llamar a sus padres/tutor para traer su uniforme. Si el padre no puede venir, se requerira que el estudiante se ponga un uniforme de 'prestamo', si esta disponible. No se le permitira al estudiante volver a clase hasta que este en conformidad con el codigo de uniforme.

Immunization Information
Dear Parent of Students Entering 2021-2022 school year:
The Arizona State Department of Public Health Immunization laws directly affect any child who will be 11 years of age and who are entering the six grade 2021-2022. This letter is an effort to notified parents of the required vaccines prior to enrollment.
Please consult your Primary Care Doctor, Pediatrician, or refer to the MCDPH clinic list attached to obtain required doses. Please bring in your updated immunization record to our office after shots are received. The vaccine requirement will need to be met to allow your child to remain at school.
Immunization Rules for Arizona
Tdap Vaccine (Tetanus/Diphtheria/Pertussis)Students eleven (11) years of age will be required to have the Tdap vaccine. This law will affect student who are 11 years of age at the time of enrollment/ school entry.
Meningococcal Vaccine (MCV4)Students ten (10) years of age will be required to have the Meningococcal vaccine (MCV4). Many colleges require this vaccine also. It protects the students against meningitis.
You are encouraged to take the time now to review your child’s immunization record and make arrangements to get any of these vaccines you might need. If you have any questions of need further assistance, please feel free to contact our school health office at 623-707-1055. Thank you.
