3. Fill in information requested in Item 11 of the OTR and note comments regarding samplecondition in the space provided.
4. Send the top sheet of the OTR (white copy) to the HWI Sample Management Office andretain the remaining yellow copy for your files.
5. Transfer to the Sample Number box of an Organics Laboratory Chronicle-Extractions (OLC-E) and an Organics Laboratory .Chronicle-Analysis (OLC-A) the sample identification numberpreprinted on the Organics Traffic Report (OTR). For each sample received, that sample,an OTR, an OLC-E and an OLC-A must all be identified by a common HWI Sample Number.
6. Proceed with extraction/preparation, recording all pertinent information on the corres-ponding Organics Laboratory Chronicle-Extraction (OLC-E).
7. If a sample is used to fulfill QC requirements for Duplicate or Matrix Spike analysis,complete the appropriate sections on both the Extraction and Analysis Organics LaboratoryChronicles.
