July 2012 Improving my assessment skills has ensured that I
have the knowledge to make the best care
decisions for my clients. Increasing my
understanding about the relationship between
assessment findings and disease process and
medications has helped me communicate with my
colleagues and physicians and recognize changes
in my clients that I may not have recognized
without this knowledge.
Increase my comfort
with venipuncture
1. Review the venipuncture
material that I received
in my venipuncture
class.
2. Meet with my manager
and set up a day for me
to work in the outpatient
lab to draw blood.

October 2012 When I was uncomfortable in drawing blood, the
client would have to wait until I found someone
who could either draw the blood or guide me
through it. Now that I have increased both my
comfort and competency, I am able to perform
this skill independently and swiftly. This means
lab results are returned sooner and I can make
care decisions more efficiently and effectively.
Samples of specific practice examples
1. In May, I was the preceptor for a newly hired LPN. During orientation, I noticed that she was struggling with the math
calculations. I offered her guidance and showed her how I did the necessary math. But even with the guidance, her struggles
continued until she made a medication error. I told her that I felt accountable to make sure that the educator and manager knew
about her issues. She was not happy but I was able to get the 4 of us together and we talked about her challenges. The educator
set up a learning plan and she improved. It was tough to do but it was the right thing to do. In doing this, I have become more
confident in my leadership abilities.
2. Whenever I can, I share new information with my colleagues. When I find an interesting article I will bring a copy to the unit. In
October I went to the Scope of Practice workshop. I copied the handouts and shared it with the staff. The topic of scope of
practice comes up frequently and sometimes we will dig out the handouts and discuss them over lunch.
3. I demonstrate integrity and honesty by doing what I say I am going to do. If something comes up and I cannot follow through with
a patient, I make sure that I tell them. I have learned it is important to make sure the patient understands when a plan changes
because it can cause anxiety if I do not keep them informed.
