• To review the form and function of the ABHB Locality structures following the establishment of the Health Board in Oct 2009
• To maintain locality focus – working in partnership with Local Authorities, GPs, Third Sector and other stakeholders in developing Single Integrated Plans.

Neighbourhood Care Networks
Principle function: To Optimise and Improve Care to Local Communities:– Through Collaborative working – Finding local solutions to services delivery
issues– Maximising use of all resources– Whole system approach to patient centred
pathways (across all health, community and social care settings
– Delivery of single integrated plan
Partnership Principles
• Community strategy for Caerphilly still remains a key foundation for partnership activity
• Four Community Plan themes (Education for Life, HSCWB, Regeneration and Living Environment) remain unaffected by restructuring
• Effective working relationships continue to be developed through the Primary Care & Networks Division and linked through the Neighbourhood Care Networks
• Senior Partnership Staff to be based with Local Authority Corporate Teams to strengthen cross organisational working
• All planning for health related services to reflect the ABHB 5 year Plan, Divisional Operational Plans and NCN priorities.
• Key links for Partnership Planning (Single Plan and wider community engagement, third sector contracting etc) through Primary Care & Networks Division and Locality Heads of Partnerships & Networks.
