1. INTRODUCTION Working in workplaces entails exposure to risks. These risks can create dangers, if expertise is lacking and safety rules are ignored. Working in a safe and healthy manner is a top priority and in the interest of you and your colleagues. The purpose of these safety regulations is to assist you to reducing the risks as much as possible for you and your environment. It includes a number of tips and rules of conduct that contribute to carrying out work activities safely and to improving working conditions. Finally, these safety regulations indicate how authorizations and responsibilities are organised in our company. 2. POLICY DECLARATION The safety policy of Verolme Special Equipment (hereinafter: VSE) aims to guarantee the safety of people in workplaces by preventing accidents and avoiding situations that threaten health, welfare and the environment. This applies to everyone who is on the premises of VSE. It is an integral part of the entire company policy. The aspects of safety, health, welfare and the environment will always receive our utmost attention. The executive staff will improve these aspects as much as possible and if necessary, the policy will be assessed and adjusted. The safety policy is implemented by:

1. Taking measures that contribute to improvements in the area of health, welfare and the environment

2. Providing information and instructions about these areas 3. Focusing attention on these areas 4. Enforcing the related legislations and regulations

3. RESPONSIBILITIES The management has the ultimate responsibility for the entire realm of safety, health, welfare and the environment in the company. The primary task of the management is to implement a company policy that aims to achieve the highest level of safety, to protect everyone’s health to the best extent possible, to promote the welfare of the employees in VSE, and to prevent damage to possessions and harm to the environment. The management can set down responsibilities and authorizations needed for implementing the company policy or parts thereof. The Labour Inspectorate is authorized to fine an employee who has clearly violated the regulations, while at the same time it can be proven that VSE has complied with its obligations. The QA/QC manager should randomly assess the implementation of the safety policy and the compliance with the regulations or parts thereof, and report this to the management. A task of the QA/QC manager is also to register and to report any occupational accidents. The persons who have the ultimate responsibility for a department should make it possible for everyone to work safely by means of (to name just a few):

- Implementing the policy intended by the management - Systematically inspecting, checking and maintaining installations,

equipment, tools and safety equipment that fall under their responsibility

- Providing safety equipment - Supervising the compliance with the rules and regulations

Persons in charge are responsible for: - Carrying out work activities and/or having these carried out safely - Immediately eliminating any unsafe and unhealthy situation - Reporting any accident to the person who has the ultimate

responsibility for the department and to the HR department - Reporting any circumstance that threatens welfare to the person

who has the ultimate responsibility for the department - Reporting observed risks/dangers, damages, etc. to the person who

has the ultimate responsibility for the department - Supervising the compliance with the rules and regulations and the

proper use of tools - Supervising the use of the proper Personal Protective Equipment

(PPE) The HR department is responsible for:

- Providing the safety regulations to the employees, temp agencies and self-employed persons without employees

- Providing information and instructing new employees - Sickness registration in VSE

The Purchase department is responsible for:

- Providing the safety regulations to subcontractors that carry out work activities on the VSE premises or have them carries out; the subcontractors will have to sign for receipt and this receipt confirmation should be saved in the Purchase department.

4. GENERAL RULES AND REGULATIONS

4.1 The Working Conditions Act, the Work Conditions Decree and the Health & Safety Regulations apply at VSE as well as the Environmental Protection Act, the Pollution of Surface Waters Act and the Environmentally Hazardous Substances Act. Everyone on the VSE premises is obliged to observe necessary caution and care in order to prevent risks/dangers to the safety, health or welfare of themselves, other people or the environment. Personnel of subcontractors and contracted personnel must also comply with their own company rules and regulations besides the rules and regulations mentioned in these regulations. The management of VSE is responsible for your safety, health and welfare during work activities commissioned by VSE. We will take appropriate measures against you, if your behaviour and/or actions are of such a nature that we can no longer bear this responsibility.

4.2 A number of work activities or circumstances demand special work

regulations and/or procedures that these regulations do not cover. Consequently, these special regulations/procedures are included in the starting instruction/quality plan of the project concerned. The starting instruction/project’s quality plan will be available in all relevant workplaces. The person in charge will check the compliance with it.

4.3 There may be cases that are not covered by any regulation or

procedure. In such cases, you should consult the personnel in charge and take measures that will prevent unacceptable risks. In case of doubt, please refer to the Occupational Health & Safety Coordinator, the Quality & Environmental Coordinator and/or the Occupational Health & Safety Service.

4.4 Everyone is obliged: 1. To observe the applicable regulations 2. To avoid situations that are unsafe and unhealthy to yourself

and to others 3. To report immediately to the personnel in charge, if there are

situations that constitute a danger to the safety, health, welfare or the environment

4. To properly use PPE, source extraction, tools, machines, equipment, transport means and other devices

5. To properly use the safeguards installed on tools, machines and/or equipment and NOT to manipulate or to bypass these safeguards

6. To keep the workplace orderly and accessible 7. To keep marked crane gantries, passageways, side-paths, etc.

over the full width free of materials and tools 8. Always leave the fire extinguishing equipment standing and/or

hanging in the designated places, which should always be directly accessible, and after using a portable fire extinguisher, to report this to your supervisor

9. To store flammable or hazardous substances in the storage places intended for this purpose

10. To always mix / process environmentally hazardous substances above liquid-proof floors and/or drip-trays

11. In case of using gas cylinders, to affix these well, to leave the keys on the cocks of the cylinders and to close the cocks after use

12. To make electric devices and machines ‘dead’, to the extent possible, at the end of the working hours

13. To participate in instructional sessions organized by or on behalf of VSE, as referred to in the Work Conditions Act (e.g., toolboxes)

14. To report the use of heavy medication to the person in charge and/or the Occupational Health & Safety Service

4.5 It is prohibited: 1. To bring alcoholic beverages and/or drugs onto the company

premises and to consume/use them there; the premises may also not be accessed by persons under the influence of the aforementioned products and/or medication that affect consciousness.

2. To smoke inside the buildings 3. To eat or drink inside the workplace 4. To use a mobile telephone inside the workplace 5. To wear jewellery, such as necklaces, rings and watches, while

carrying out work activities in the workplace 6. To access areas where X-ray examinations are carried out 7. To use tools, machines, equipment, transport means and other

devices, if you are not authorized to use them 8. To operate machines unsupervised 9. To start burning or welding activities in spaces where there is

the risk of fire without the presence of sufficient extinguishing equipment at the site and/or firefighter

10. To move fire extinguishing equipment, First Aid material, etc. to other locations without the permission of supervising personnel

11. To be under or on top of loads suspended from hoisting equipment or other equipment

12. If you are not over the age of 18: • To work at heights higher than 2.5 m above ground level • To work in a work bin • To operate equipment and/or machines without

professional supervision

4.6 These safety regulations are in effect for anyone on the VSE premises and anyone who falls under the responsibility of VSE.

4.7 In any case, these safety regulations do NOT replace any legal

regulations.

5. PERSONAL PROTECTIVE EQUIPMENT

5.1 Personal Protective Equipment (PPE) should be used correctly, should be stored in the place intended for this purpose after use, and should be maintained.

5.2 Everyone is obliged to use safety shoes and other PPE in areas where

this is mandatory. 5.3 Except people who work in the offices, everyone should wear

working clothes; wearing shorts in the workplace is forbidden. 5.4 Wearing a safety helmet is mandatory in areas where this is indicated

or if there is a high risk of head injuries 5.5 Wearing effective safety goggles is always mandatory.

Wearing a face shield is mandatory when: - Tooling such as sawing, grinding, sanding, rolling, drilling, milling - Clearing workpieces by using compressed air, etcetera - Carrying out any welding and cutting activities - Using chemicals - Using a power washer inside barrels Wearing welding goggles (protection against fire, etc.) is mandatory for autogenous welding or cutting processes. Using a welding shield or helmet is mandatory for all other welding activities.

5.6 Wearing hearing protection equipment, such as earplugs, ear moulds

and earmuffs is always mandatory at the workplace.

5.7 Wearing respiratory protection equipment is mandatory, if there is a

risk of breathing toxic, corrosive or bothersome vapours, smoke, gasses, fumes or dust. In any case, wearing respiratory protection equipment is mandatory during welding and cutting activities. Available respiratory protection equipment is: - Dust masks (type P1, P2 or P3) - Masks with active carbon filters: these masks are used for

protection against gasses and vapours at low concentrations. - Fresh-air masks or hoods: these are used in case of high

concentrations of gas, vapour and/or dust. Note: Never connect masks to pure oxygen.

5.8 Long loose-hanging hair can easily get tangled in rotating shafts, etc.

If this is a risk in your work activities, you are obliged to tie your hair together using a hairnet, for example.

5.9 Wearing and using a fall harness is mandatory, if using a scaffold or

platform or installing barriers, railings and similar provisions are not possible while working at heights above 2.50 metres. The lanyard may not be longer than 1.5 metres. You must use approved fall harnesses (provided with manufacturing and permission dates). Fall harnesses should always be checked before use. Replace any fall harnesses that have been subject to loads. If subcontractors are working in our workplaces, VSE must make fall harnesses available to them for work at heights.

6. SPECIFIC REGULATIONS

6.1 Working in confined spaces Sometimes work activities on modules, pressure vessels, etc. are carried out in confined spaces. A confined space is ‘a space closed on all sides with one or more relatively small entrances and no natural ventilation’. Working in such spaces for long periods may affect our level of attention to the potential risks in such places. Examples of potential risks are: - The oxygen level of the air is too low or too high. - The vapour/air mixture is too explosive. - The generation of hazardous gasses or vapours due to work

activities such as welding vapours - Poor options for communication with other people - Too high noise levels To limit these potential risks, you must observe the following when working in a confined space: - Always check a confined space for safe working conditions. - Never work alone in a confined space without good options for

communication with other people and/or without direct supervision. A manhole observer (outside observer) must always be present.

- Always take care of good lighting and ventilation. In case of insufficient ventilation, use personal respiratory protection.

- For lighting and powering tools, use a voltage of maximum 42V AC or 110V DC.

6.2 Working at heights Working at heights entails the risk of falling or other risks. Therefore, the law stipulates that a safe scaffold, trestle, platform or scaffold floor must always be installed for working above 2.5 metres. If this is impossible, the risks must be countered by putting up effective barriers or railings. If these provisions are not available, you must use safety nets, lifebelts or fall harnesses. These measures do not apply to working on stairways, ladders of scaffolds. Below you will find the regulations that apply when using these means.

6.2.1 Stairways and ladders

- Always opt for a stairway or ladder of the right type and the correct length, so that it protrudes at least 1 metre above the place to which it provides access.

- Check stairways and ladders after receiving them and before using them (in particular if the ladder has ladder feet).

6.2.2 Mobile scaffold towers

Mobile scaffold towers may be used for work activities up to a maximum height of 6.5 metres and you must observe the following safety prerequisites: - The scaffold floors of a mobile scaffold tower must be

completely sealed. During use, any opening in the scaffold floor must be sealed or shielded in such a manner that there is no risk of falling through the opening.

- The scaffold floor should be fixed tightly to prevent it from shifting or being blown away.

- A scaffold floor must be surrounded by fencing that includes at least: • A railing at approx. 1 metre above the scaffold floor • A rail at approx. 0.5 metre above the scaffold floor • A toe plate adjoining the scaffold floor with a height of

minimum 0.15 metre - Scaffold floors must be reachable safely. - A climbing facility consisting of slanted ladders and stairways

must be installed on the inside of the mobile scaffold tower. - You must leave a mobile scaffold tower, if the wind force is

more than 6 on the scale of Beaufort. - Working from a ladder or auxiliary scaffold set up on the

scaffold floor is forbidden. - A mobile scaffold tower must be secured against toppling, if the

mobile scaffold tower • Is exposed to a wind force of more than 6 on the scale of

Beaufort; • Is covered entirely or partially with tarps.

- The wheels of the mobile scaffold tower must be locked during use.

- No persons or materials are allowed on the mobile scaffold tower during rolling.

- It is forbidden to remove supports or jibs used to safeguard the scaffold’s stability during rolling. Supports or jibs that do not have wheels must not come in contact with the ground during rolling.

- Hoisting materials from the scaffold floor must never endanger the scaffold’s stability.

6.3 Working with gas and oxygen

To prevent the risk of fire resulting from working with gas and oxygen, the following should be observed: - Before using burning equipment, carefully read the correct user

instructions.

- Reduce the pressure to a level that is not higher than necessary for the job.

- Do not switch the colour coding of the gas hose (red) and the oxygen hose (blue).

- Attach the hoses to the burner using hose clips or quick-action couplings.

- A flame extinguisher must be installed between the reducer and the burner.

- Never use acetylene from horizontally positioned cylinders. - Never blow off acetylene gas, because this can produce an

explosive gas mixture. - Before starting the work activities, check the oxy-fuel

equipment for deviations. - Keep hoses at a distance from glowing sparks. - Report leaks in the equipment immediately. - Use oxygen only for maintaining the flame. Blowing out with

oxygen is prohibited. - Avoid contact with grease or oil located on the oxygen cock or

valve.

6.4 Using compressed air - Air hoses may only be connected or disconnected only when the

air supply is closed off. - Leaking air hoses must be replaced immediately.

6.5 Electric welding processes

To prevent the risk of fire resulting from welding activities, the following should be observed: - Electric preheating elements must be attached properly to the

workpieces. - Combustible materials may not be placed on the preheating

elements. - If a welding transformer is used, it should have recognizable

tension-lowering relays.

- Power-supply and welding cables should be in proper condition. - Make sure that the fixture is connected to the workpiece in a

highly conductive manner. - Welding activities to workpieces suspended from a crane may

be carried out only if there is no other way. If this is the case, you must ensure good earthing between the workpiece and the crane hook.

- Flammable substances, such as paint, oil, insulation material and/or wood, should be removed before starting the welding activities.

- You must check whether the welding activities have not resulted in smouldering fire.

- The welding transformer must be switched off after use.

6.5.1 Ventilation systems and exhaustion When using electric welding and grinding, the following should be observed: - The roof ventilators must always be switched on during the

work activities. The grids in the sidewalls of the buildings, which suck in fresh air, should be open at all times.

- The recirculation units in halls W and Z should operate during the work activities.

- Depending on the welding process, the welding electrode/wire, the material that should be welded and the site of the welding activities, measures should be taken to exhaust the welding vapours by means of source exhaustion.

- If the exhaustion is insufficient, you should use respiratory protection equipment.

6.6 Tooling machines with moving components

It is strictly forbidden to wear gloves when working with tooling machines that have moving components.

6.7 Grinding tools and grinding - Always prevent other people from being harmed by grinding

rays. - The grinder must always have a protective cap during grinding. - When working with grinders, use the correct grinding discs or

grinding brushes for the specific grinder. 6.8 Using demarcation and/or markings

Hazardous areas must be demarcated and/or marked at all times. This applies, for example, to areas under all operating cranes.

6.9 Storage of materials and set-up of workpieces

- Materials must be stored in a way that prevents them from toppling. The same applies to the set-up of workpieces.

- Roads and walkways must be kept free of obstacles. - Round materials must be secured at both sides against rolling

away.

7. EUROPEAN SYMBOLS INDICATING HAZARDOUS SUBSTANCES AND PRODUCTS You may encounter the following hazard symbols: 7.1 Inflammable / highly inflammable substances

Measures - Using open fire is not permitted when working with

inflammable / highly inflammable substances. - Smoking, burning, welding or work with a risk of sparks is not

permitted. - Rooms must be adequately ventilated. - Rags or cleaning clothes soiled by inflammable / highly

inflammable substances must be deposited in closable flam-extinguishing waste bins.

- Fire extinguishing equipment must be available during work with inflammable / highly inflammable substances.

7.2 Toxic and harmful substances

Toxic Harmful

Measures - Rooms must be adequately ventilated. - Rags soiled by toxic or harmful substances must be deposited in

closable waste bins. - Fire extinguishing equipment must be available during work

with toxic or harmful substances. - When working with toxic or harmful substances, you must use

appropriate clothing, gloves, face protection and respiratory protection.

- Consult a doctor if you feel unwell after working with toxic or harmful substances. You must be able to show the label of the product.

- You are not permitted to flush toxic or harmful substances in the sink.

7.3 Corrosive, oxidizing and irritating substances

Corrosive Oxidizing Irritating

Measures - When working with corrosive, oxidizing or irritating substances,

you must use appropriate clothing, gloves, face protection and respiratory protection.

- If one of these substances comes into contact with your skin or eyes, it should be rinsed off with lots of water and a doctor should be consulted.

- You are not permitted to flush corrosive, oxidizing or irritating substances in the sink.

8. FIRST AID / EMERGENCY RESPONSE SERVICES An Emergency Response Team (ERT) member appointed by VSE must be present at all times. What to do in case of serious accidents and other situations in which First Aid must be provided?

- The casualty must remain in the same position on condition that there is no fire, risk of explosion or development of gas.

- One person must call 112. - You must warn the ERT member immediately. Names and

telephone numbers are publicised on the bulletin boards. - You must inform the management after warning the ERT

member.

It is important to call or to let another person call the emergency response services to report the following details:

- Your name and department - The place of the accident - If possible, the nature of the injury/injuries - The number of casualties - To which place the ambulance should come

The place of the accident must be repeated so that there can be no misunderstanding about this. Subsequently, you will have to wait for the ambulance. In case there are several casualties, you should call more than one ERT member. What do you do in all other cases of accidents?

- An ERT member must always be called up. - In cases of entrapment, you must free the casualty from the

object(s) that are on top of him/her. - In case of an accident with electricity, you must immediately

switch off the power supply. - In case of stunning in a confined space, you must take the

casualty out of this space and ventilate this space, while using respiratory protection equipment.

Rule of thumb for the ERT member:

- Pay attention to dangers for yourself and the casualty. - Check what has happened and what is wrong with the casualty. - Reassure the casualty.

The ERT member takes care of the contact with the ambulance personnel, the GP and the company medical officer. The ERT member is the leader until he/she transfers the casualty/casualties to the professionals concerned. As soon as that happened, the ERT member should inform the management.

The internal accident reporting and registration and any further guidance of the person(s) involved should occur according to the applicable procedures. Reporting accidents to the Labour Inspectorate occurs according to the applicable procedures. To facilitate a proper accident investigation, it is forbidden to change the circumstances / conditions / situations, unless these result in danger. 9. FIRE / EMERGENCY RESPONSE SERVICES Fire must be reported immediately to the ERT member or the emergency response services. It is important to call or to let another person call the emergency response services to report the following details:

- Your name and department - The place of the accident - If possible, the nature of the injury/injuries - The number of casualties - To which place the fire brigade and/or ambulance should come

In the event of a fire, the personnel that are present must be warned and the fire must be controlled/extinguished using the available small fire extinguishing equipment, while preventing any unnecessary risks. You should know where the fire extinguishing equipment is located. The fire extinguishing equipment must be accessible at all times. In the event of a fire without the risk of incidents, you should act as follows:

- Stay calm. - Evacuate the workplace. - Close the gas and electricity supply. - Start extinguishing using the appropriate fire extinguishing

equipment. - Report the fire to the person in charge.

Take the following into account: - Never spray water on the electrical installations or equipment. - Keep windows and doors closed as much as possible. - Switch off machines / equipment. - Follow the instructions of the ERT member or the fire brigade

personnel immediately. - Inform the person in charge or the fire brigade personnel, if

inflammable or hazardous substances are in the department / room concerned.

- It is forbidden to use elevators in case of fire. To prevent fire, everyone must pay extra attention to the following:

- Leaking oxygen - Leaking gas hoses - Splashing sparks of cutting torches - Flying grinding material - Gouging and welding in the vicinity of inflammable substances - Inflammable substances in the direct vicinity of preheating with

gas A number of tips to be able to act faster in case of fire:

- Before starting the work activities, make sure that you know where to sound the alarm.

- Make sure that you know how to operate the fire extinguishing equipment.

- Fire extinguishers are divided into application areas by means of letters. You will find these application areas in the manual: A = solid substances C = gasses E = electricity B = liquids D = metal

10. ENVIRONMENTAL MEASURES

- Hazardous waste, oil products, wood, paper, batteries and other operational waste must be deposited only in the bins or barrels intended for the specific purpose.

- Besides the mandatory safety measures, effective measures must be taken to prevent pollution of the environment, while loading / unloading / transporting / processing / storing oil or chemicals.

- In case of sampling / decanting oil or chemicals, using drip-trays is mandatory.

- It is forbidden to discharge and drain off contaminated wastewater.

- 11. VIOLATING THE WORK CONDITIONS OR ENVIRONMENTAL

REGULATIONS

VSE is first and foremost responsible for good work conditions for its employees and for preventing any harm to the environment. VSE is not only authorized, but also obliged, to impose punitive measures on anyone who intentionally endangers the safety, health, welfare or the environment. These punitive measures can consist of:

- An oral reprimand - A written reprimand - Disqualifying the person from driving transport equipment of

the company - Transfer to another function for a definite or indefinite period of

time with or without salary adjustment - Suspension for a definite period of time - Dismissal

12. THIRD PARTIES The following additional measures apply to contractors/subcontractors who carry out work activities commissioned by VSE:

- Tools in their possession must meet the legal requirements and regulations of the Labour Inspectorate. If requirements and regulations do not apply, these tools must comply with what can reasonably mean ‘safe tools’.

- While carrying out work activities, professional monitoring should be carried out on compliance with the rules stated in these regulations.

- Contractors/subcontractors should take care of their own PPE, small fire extinguishing equipment, protective and demarcation materials, etc., unless other agreements were made with VSE.

- VSE is not liable for any damage resulting from using VSE properties. VSE is indemnified against any claim to compensation.

- VSE reserves the right to recover from contractors / subcontractors damages and costs resulting from the actions of contractors / subcontractors.

- The contractors / subcontractors are obliged to insure themselves against the risk of legal liability. The must be able to submit an insurance policy upon the request of VSE.

- Waste products released during the work activities by the contractor / subcontractor must be discharged efficiently and entirely at the expense of the contractor / subcontractor in compliance with the Environmental Protection Act and the rules stated in the Industrial Waste Decree.

13. POINTS OF ATTENTION

1. Use the right PPE at all times. 2. Use the tools in the correct manner. 3. Keep your workplace and other operational areas neat. 4. Avoid situations that are unsafe or that can endanger your health. 5. Address other people in case of violating these safety regulations. 6. Immediately report situations that endanger the safety, health,

welfare or the environment to your manager. 7. Make sure that you know the evacuation plan and how to use

safety equipment correctly.
