ANTECEDENTES
• El estudioACRIN 6666 ‘08 documentó que u/s detecta 4.2 cánceres adicionales por c/1000 mujeres de alto riesgo.
• Un estudio de 2004 halló que 90% de los cánceres detectados con ultrasonido solo eran estadio 0 o 1
1. Ultrasonido puede detectar tumores ocultos a la mamografía.
2. Ultrasonido puede detectar cánceres de mama en estadios iniciales.

ESTUDIO ORIGINAL• PROPUESTA: determinar si el screening
con ecografía en mujeres con mamografías sin alteraciones pero con mamas densas es útil en la detección del ca. de mama.
• OBJETIVOS: Determinar el VPP, la tasa de detección de cáncer, el tipo ,tamaño y status axilar de los tumores hallados. Establecer la población ideal para este tipo de screening, riesgos y beneficios.

DISCUSION• El Screening Ecografico en mujeres con mamas
mamograficamente densas (> 50%) detecta cánceres ocultos.• Esto ha continuado al mismo ritmo / mil durante los primeros
cuatro años desde la promulgación de la ley que establece que las pacientes con alta densidad del tejido mamario deben ser informadas, permitiendo la utilización de imágenes adicionales como el ultrasonido.
• El VPP ha mejorado lo que indica que como era de esperar hay una curva de aprendizaje para decidir que lesiones biopsiar y cuales controlar. Los cánceres se encuentran en mujeres que tienen ECO anual.
• % general las mujeres elegibles que hicieron ECO se mantuvo estable en alrededor de 30 %, lo que puede deberse a la falta de educación, mas que a cuestiones de costos / seguros.

DISCUSION cont.• Los tumores de alto grado tuvieron más
probabilidades de tener ganglios centinelas positivos.
• No hubo asociacion con otros factores de riesgo mas allá de la “mama densa”.
• Hubo una disminución progresiva del tamaño de las lesiones detectadas, siendo las mayores las del año 1.
• El tamaño promedio fue inferior al cm.

LIMITANTESSólo cuatro años de datos - cuántos años necesitamos para demostrar el valor agregado de la ecografia de mama bilateral ?Tenemos muchos más años de la mamografía de screening y sabemos que se ha mejorado la mortalidad. Estos son los mismos tipos de cánceres.
La densidad mamaria absoluta no estaba consignada para cada cáncer es decir, sin especificar si era de 50 a 75 % o> 75 % Esto podría considerarse arbitrario , ya que no disponemos de los datos de densidad "absolutos" y no sabemos si eso es relevante.
No hubo analisis de costos para saber cuanto mas costo cada cancer diagnosticado. Dos estudios anteriores (ref . 6 y 7 ) realizaron un análisis de costos y no mostraron demasiado incremento . Después de todo , ¿cuál sería el costo si se compara con el hallazgo de un cáncer en una etapa posterior, que cuesta más tratar y con mayor mortalidad potencial ?! Claramente más datos y análisis son necesarios!

CONCLUSION
• El agregado de ecografia de screening en pacientes con mamas densas (>50%), aumenta la posibilidad de detección de cáncer.
• Se trata de tumores pequeños con GC negativos, excepto los de alto grado.
• En los casos en que se repitieron sucesivamente las ecografías, se detectaron mayor número de cánceres, lo que indicaría que en este grupo de pacientes debería realizarse ecografía anual.

• Solo en 4 estados, son cubiertos los estudios adicionales como tomosíntesis, RNM o ecografía.
CRITICAS…
• No grupo control.• No seguimiento a largo plazo.• El impacto clínico del hallazgo de estos canceres
adicionales es desconocido.• Podrían haberse detectado en la mamografía posterior y
ser aun pequeños y con axila negativa?• Se desconoce impacto en mortalidad.• Muchas biopsias, muchos falsos positivos.( con Mx 25-
40% de las bp. son positivas, con eco screening solo 5-10%).
• Alto costo, pocos beneficios.• Tomosíntesis, disminuye 15-30% los falsos (+) y aumenta
en 30-50% la detección de ca.
