RESOLUTION NO. __, Series of 2015

A RESOLUTION TO CHANGE ONE OF THE SIGNATORIES FROM SASHAH B. DIOSO TO PETER GLENN A. RAPIZ DUE TO RESIGNATION OF THE FORMER EFFECTIVE JUNE 30, 2015.

WHEREAS, Barangay Tigmarabo, Miagao, Iloilo is one of the recipients of Kapit-Bisig Laban sa Kahirapan Comprehensive and Integrated Delivery of Social Services National Community-Driven Development Program(KC-NCDDP) of the Department of Social Services and Development (DSWD), a community-driven development project with rehabilitation and recovery mechanism for the communities affected by Yolanda while aiming to empower communities and improve local governance through the enhanced participation in community projects that reduce poverty;

WHEREAS, as per further discussion with the community during the barangay consultation meeting, it has been determined that Peter Glenn A. Rapiz will be the new signatory for the account opened.

NOW, THEREFORE, on motion of Lea Endiable and duly seconded by the body, resolved as it is resolved to authorize Peter Glenn A. Rapiz, Area Coordinator, as the new signatory for the account opened to replace Sashah B. Dioso.

RESOLVED FURTHER, to furnish copies of this resolution to Landbank of the Philippines Miagao Iloilo Branch, Area Coordinating Team Miagao and offices concerned for their information and guidance.
