Residential Care Services Investigation Summary Report

Unalleged Violation(s):1. The residents were left alone in the AFH. Upon entering the home the night shift caregiver (Caregiver D) was not in the home.There was no one in the home with the residents from the time the department staff member arrived at 06:25am until thedepartment staff member was let into the home at 06:40am. Interview with the night shift caregiver revealed she was the onewith the residents for night time and that she left to go next door to help another caregiver. The night shift caregiver also saidshe left to go make coffee for the morning.

2. The AFH failed to provide privacy for 1 of 5 residents. While knocking on the glass sliding door of the home waiting forsomeone to answer, a resident was observed sitting in the bathroom on the toilet. The bathroom door was wide open whereanyone who approached the glass sliding door, could see the resident exposed in the bathroom. Record review of the resident'snegotiated care plan (NCP), found the resident needs assistance with toileting and is a risk for falls.
