Record Reviews: Incident reports, residentrecords and facilityrecords.

Allegation Summary:1. Text messages between the named resident's family member and the provider revealed the provider never texted the namedresident or family demanding cash. The named resident owed the provider $468 12 The named resident failed to pay theparticipation fees to the facility. The named resident moved from the facility on /16. The money owed to the provider wasnever paid. The text messages revealed the provider cordially attempted to work out an arrangement to pay the money owed tothe provider. This allegation was unsubstantiated. In review of facility records, it was found the provider failed to complete apreliminary care plan for two newly admitted residents. Failed practice was found.

Allegation Summary:1. Interviews of caregivers and the provider revealed the named resident was resistive to care and combative since admission tothe home on /18. The provider called the named resident's family and the family member came over to calm the namedresident on numerous occasions. The provider notified the case manager and physician regarding the uncontrollable behaviors.On /18, the named resident became increasingly violent and hit a caregiver and touched her inappropriately. The providercalled the family and 911. The provider gave the family member an immediate discharge notice due to fear of safety ofindividuals in the home and unable to care for the named resident. The discharge notice contained all the required information.The allegation of inappropriate discharge was unsubstantiated. In review of facility records it was found the provider failed tocomplete a preliminary care plan for 2 newly admitted residents. Failed practice was found.
