Clinical Research

An Open Label Pilot Study Testing The Role Of Classical Homeopathy In Chronic Allergic Rhinitis

Shubhamoy Ghosh*, Satadal Das, Malay Mundle,

Dishari Sengupta, Sk. Intaj Hossain, Munmun

Koley and Subhranil Saha

IJPSR, 2013; Vol. 4(4): 1475-1484.

Free Text Available

Purpose: The prevalence of allergic rhinitis (AR)

is increasing at an alarming rate throughout the

world. India has an estimated number of 15-20

million patients with allergic bronchial asthma

and 30-80% of these suffer from AR. So, AR is

considered as a major chronic respiratory disease

due to its prevalence, impact on quality of life

(QoL), work/school performance and

productivity, economic burden and links with

asthma. This research aims at testing the role of

classical homeopathy in bringing changes in

serum immunoglobulin E (IgE) level and absolute

eosinophil count (primary outcome measures)

and symptoms score and WHOQOL-BREF score

related to AR (secondary outcome measures) by

comparing the pre-trial and post-trial data.

Method: An open label, single arm, experimental,

prospective, non-randomized, non-controlled,

before and after comparison pilot study was

carried on 34 participants suffering from chronic

AR. Institutional ethical clearance was obtained;

then thirty four consenting patients were

enrolled after screening of 58 patients by

eligibility criteria and were allocated to classical

homeopathic treatment. Four cases were drop-

outs; thirty cases were regular. Outcome

measures were assessed and analyzed after one

year.

Results: After one year of homeopathic

treatment, lowering of serum IgE level

(1006.83±395.17 vs 336.5±126.96; P = 0.0000),

absolute eosinophil count of blood

(600.33±103.61 vs 302.5±82.21; P = 0.0000) and

AR symptoms score (30.27±5.12 vs 12.83±2.72; P

= 0.0000) and increase in WHOQOL-BREF scores

of AR (58±7.01 vs 87.7±6.18; P = 0.0000) were

statistically highly significant. No adverse effects

and/or complications were observed. Most

commonly used constitutional and acute

remedies were Natrum muriaticum and

Histamine hydrochloride respectively in different

centesimal potencies.

Conclusion: Data suggest that classical

homeopathic treatment may be a useful measure

for the patients suffering from chronic AR.

However, multi-centric randomized controlled

trials with larger sample size should be

undertaken for making firm recommendations.

Efficacy of individualized homeopathic treatment and fluoxetine for moderate to severe depression in peri- and postmenopausal women (HOMDEP-MENOP): study protocol for a randomized, double-dummy, double-blind, placebo-controlled trial. Trials 2013, 14:105

Background

The perimenopausal period refers to the interval

when women's menstrual cycles become

irregular and is characterized by an increased

Page 4: Research Updates-Homeopathy-Vol-2, Issue 5

38

risk of depressive symptoms. Use of homeopathy

to treat depression is widespread but there is a

lack of clinical trials about its efficacy in

depression in peri- and postmenopausal women.

Previous trials suggest that individualized

homeopathic treatments improve depression. In

classical homeopathy, an individually selected

homeopathic remedy is prescribed after a

complete case history of the patient. The aim of

this study is to assess the efficacy and safety of

the homeopathic individualized treatment versus

placebo or fluoxetine in peri- and

postmenopausal women with moderate to severe

depression.

Methods/design

A randomized, placebo-controlled, double-blind,

double-dummy, three-arm trial with a six week

follow-up study was designed. The study will be

conducted in a public research hospital in Mexico

City (Juárez de México Hospital) in the outpatient

service of homeopathy. One hundred eighty nine

peri- and postmenopausal women diagnosed

with major depression according to the

Diagnostic and Statistical Manual of Mental

Disorders, 4th edition (moderate to severe

intensity) will be included. The primary outcome

is change in the mean total score among groups

on the 17-item Hamilton Rating Scale for

Depression after the fourth and sixth week of

treatment. Secondary outcomes are: Beck

Depression Inventory change in mean score,

Greene's Scale change in mean score, response

and remission rates and safety. Efficacy data will

be analyzed in the intention-to-treat population.

To determine differences in the primary and

secondary outcomes among groups at baseline

and weeks four and six, data will be analyzed by

analysis of variance for independent measures

with the Bonferroni post-hoc test.

Discussion

This study is the first trial of classical

homeopathy that will evaluate the efficacy of

homeopathic individualized treatment using C-

potencies versus placebo or fluoxetine in periand

postmenopausal women with moderate to severe

depression. It is an attempt to deal with the

obstacles of homeopathic research due to the

need for individual prescriptions in one of the

most common psychiatric diseases.

Accelerating the healing of bone fracture using homeopathy: a prospective, randomized double-blind controlled study

BMC Complementary and Alternative Medicine

2012, 12(Suppl 1):O61

S Sharma*, N Sharma, R Sharma

Free Text Available

Purpose

In clinical practice, homeopathy is widely used in

the fracture-repair process, which accelerates the

healing of fractures, enhances callus formation

and reduces pain. But there is no anatomical or

scientific evidence yet to prove that. Therefore,

the current study was undertaken to test the

efficacy of homoeopathy in bone fracture healing.

Methods

The study was conducted as a double blind

randomized controlled study with 67patients

with acute non-displaced lateral malleolar

fracture. Patients were recruited from the

Emergency Orthopaedic department, SMS

Hospital, Jaipur, India during May 2007 to May

2009. Patients were randomized to either a

homoeopathy treatment (n=34) or a control

group (n=33). All the patients received standard

orthopaedic care through 12 weeks following

injury. The treatment group received

Page 5: Research Updates-Homeopathy-Vol-2, Issue 5

39

homoeopathic medicine on the basis of totality of

symptoms and individualisation. Outcome

measures include radiological assessments and

functional tests for healing.Assessments were

taken on 3, 6, 9 and 12 weeks.

Results

Faster healing was reported in the homeopathy

group by week 9 following injury, including

significant improvement

in fracture line (p < 0.0001), fracture edge

(p<0.0001), callous formation (p< 0.05) and

fracture union (p< 0.0001) in comparison to

placebo. There was also lower use of analgesics

and less self-reported pain in the homeopathy

group.

Conclusion

The study suggests that homoeopathy could

enhance anatomical and functional fracture

healing.

The Pharmacopoeia is the official compendium of

a country related to regulatory standards of

drugs manufactured and practiced in the political

zone of that country. The pharmacopoeial

standards are experimentally established and

work as yardstick to ensure the quality safety and

efficacy of a drug. These are approved standards

and vetted by duly constituted bodies to

incorporate in a pharmacopoeia. In broad term,

the pharmacopoeia is a book of standards

applicable to drug substance and their dosage

forms of common usage in a country.

Pharmacopoeia is prepared by recognized

authority appointed by the Government of a

particular country with a few exceptions eg. USP.

In India, Homoeopathic System of Medicines has

its own pharmacopoeia titled as Homoeopathic

Pharmacopoeia of India (HPI). So far, nine

volumes of HPI are published from 1971 to 2006

which covers total 919 monographs of

Homoeopathic single drugs.

Background

Homeopathic drug proving is a basic concept in

homeopathy. This study aimed to record

symptoms produced by a homeopathic drug

compared with placebo.

Methods

This multicentre, randomised, double-blind,

placebo-controlled phase 1 trial consisted of a 7-

day run-in period, a 5 day intervention period

and a 16-day post-intervention observation

period. Subjects, investigators and statisticians

were blinded for intervention groups and identity

of the homeopathic drug. Subjects in the

intervention group received Okoubaka

aubrevillei (potency C12) and subjects in the

placebo group received the optically identical

sucrose globules. Dosage in both groups was five

globules taken five times per day over a

maximum period of 5 days. Subjects documented

the symptoms they experienced in a

semistructured online diary. The primary

outcome parameter was the number of

characteristic proving symptoms compared with

placebo after a period of 3 weeks. Characteristic

symptoms were categorised using content

analysis. Secondary outcome parameters were

the qualitative differences in profiles of

characteristic and proving symptoms and the

total number of all proving symptoms. The

number of symptoms was quantitatively analysed

on an intention-to-treat basis using analyses of

covariance with the subject's expectation and

baseline values as covariates.

Results

Thirty-one subjects were included (19 Okoubaka

and 12 placebo). Data for 29 participants could be

analysed. No significant differences in number of

characteristic symptoms in both groups were

observed between Okoubaka (mean ± standard

deviation 5.4 ± 6.0) and placebo (4.9 ± 5.6). The

odds ratio for observation of a characteristic

symptom was 1.11 (95% confidence interval 0.4

to 3.05, P = 0.843). Females and subjects

expecting a higher number of symptoms at

Page 7: Research Updates-Homeopathy-Vol-2, Issue 5

41

baseline or feeling more sensitive to homeopathic

drugs experienced more characteristic symptoms

regardless of allocation. The qualitative analysis

showed an intercoder reliability of 0.69 (95%

confidence interval 0.62 to 0.76). The qualitative

comparison of symptom profiles was

inconclusive.

Conclusions

Combined results of qualitative and quantitative

methods did not result in a significant difference

of characteristic proving symptoms between O.

aubrevillei C12 and placebo. The qualitative

comparison of the symptom profiles leaves some

open questions. The nocebo effect might be a

plausible explanation for most of the phenomena

observed in this trial.

Chelidonium majus l. - a review on pharmacological activities and clinical effects Biswas Surjyo Jyoti

Free Text Available

Chelidonium majus L. (Papaveraceae) is a plant

that has been used for centuries in treating many

diseases in European and Asian countries. Crude

extracts from various parts of the plant contain

isoquinoline alkaloids. The alkaloids derived from

C. majus have not yet much studied; however,

some reports are available on toxicity studies of

alkaloids of this plant. In such a scenario there is

need for understanding its therapeutic potential

and its toxic actions. This review summarizes

scientific findings and suggests areas where

further research is needed.

Unification of Indian Systems of Medicine and Homoeopathy Policy and National Health Policy in India-Stakeholders' Perspective

Balpreet Singh*, Amarjeet Singh and Manoj

Kumar

Free Text Available

AIM: India has designed National Policy on Indian

systems of Medicine and Homeopathy (ISM&H

Policy) in 2002 to emphasize the development of

Ayurveda, Siddha, Unani, Yoga, Naturopathy and

Homoeopathy due to public patronage of these

systems. Currently India has two health related

policies, National Health Policy and ISM&H Policy.

This study proposes to look into the need of

eventual fusion of these policies for better

healthcare.

METHODS: An online questionnaire was

designed to assess the views of Public Health

experts and mailed to 100 experts. The tool was

tested for content validity and a pilot study was

done to find out the feasibility of the study. Study

period was September 2010 to May 2011. The

data was analysed with the help of Microsoft

Excel 2007 represented as percentages.

Page 8: Research Updates-Homeopathy-Vol-2, Issue 5

42

RESULTS: 58 out of 100 experts responded to

questionnaire. 67% of respondents favored

formation of unified health policy. Most

mentioned reason for unified policy was

'common goal of health' of all system followed by

'strengthening of health system' and 'equal status

and growth of all health systems'. Respondents

disfavoring the concept of unified health policy

opined that all health systems have different

principles which can be dealt by separate policy.

CONCLUSION: Majority of respondents opined

that ISM& H Policy should be merged to form

unified health policy as health is a complete

concept in itself and should be viewed in

multidisciplinary approach.

Key words: India, medicine, homoeopathy policy.
