Using Smart phone, kidney disease (ESRD) patient can manage personal health related activities - remind for patients to comply to procedures - capture body measurements easily and submit
to hospital from home - collaborate with care takers

Hospital Via patient dashboard, medical professionals can monitor conditions of patients at home through their body measurements. Early intervention is possible In case of infection, fluid leakage and other complications.
