There is hereby imposed an annual fixed tax for everydelivery vehicle, truck, van, or any motor vehicle, vessels orwatercraft used by manufacturers, producers, wholesalers,dealers or retailers in the delivery or distribution of distilledspirits, fermented liquors, soft drinks, cigars and cigarettesand other essential products and sales of goods as mayhereinafter be determined by the Sangguniang Panlungsod tosales outlets or consumers, whether directly. Or indirectlywithin the City, subject to the following rates:

- Motorcycle and Motorized Tricycle P 240.00

SECTION 72. IMPOSITION OF TAX.

The following worker or employees, non-holder of validlyissued Professional Tax Receipts whether working ontemporary and permanent basis, shall secure the individualOccupational Permit at the moment they are actually acceptedby the management of any business or industrialestablishment to start work and shall pay an annual permitfee at the rate of One Hundred Twenty-Five Pesos (P125.00).

Further, Section 75 of the same Ordinance provides that “anannual registration sticker shall be issued by the Office of theCity Treasurer upon payment of Two Hundred Pesos(P200.00) and the same shall be displayed at the rightwindshield of any delivery van, truck or vehicle.

TO BE A LEGIT RIDER / DELIVERY SERVICES CONTRACTOR YOU NEED TO HAVE:

1. Either a Mayor’s Permit (Business Permit) or Occupational Permit2. Delivery Sticker for the Vehicle use in Delivering Food & Non-Food

For Riders who DO NOT HAVE employers or riders who areFreelancers, a Mayor’s Permit is REQUIRED.

The following are the REQUIREMENTS:1. Unified Application Form (Notarized and completely filled-out)2. Proof of business registration, Incorporation, or legal personality

(i.e. DTI / SEC / CDA Registration)3. Basis for computing taxes, fees and charges (e.g. Business

Capitalization)4. Copy of Occupancy Permit5. Contract of Lease (if Lessee) or any proof of ownership (if owner or

property)6. Barangay Clearance7. Sketch8. Cedula

For Riders who DO NOT HAVE employers or riders who areFreelancers, a Mayor’s Permit is REQUIRED.

The following are the REQUIREMENTS:

9. Certified List of Employees10.Copy of OR/CR of vehicles use in business11.Special Power of Attorney for Single Proprietorship or Board

Resolution / Secretary’s Certificate for other types of ownership, ifrepresented

12.Copy of any government issued I.D. for owner and of authorizedrepresentative if there are any.

For Riders who DO NOT HAVE employers or riders who areFreelancers, a Mayor’s Permit is REQUIRED.

The following are the PROCEDURE:

1. FILE. Drop your duly accomplished and notarized Unified Form (UF)and other documentary Requirements at a designated section at theDavao City Recreation Center or Almendras Gym from Monday toFriday, from 8am to 5pm. The Unifed Form (UF) can be secured at theAlmendras Gym or can be downloaded atbusbureau.davaocity.gov.ph

For Riders who DO NOT HAVE employers or riders who areFreelancers, a Mayor’s Permit is REQUIRED.

The following are the PROCEDURE:

2. PAY. Proceed to the City Treasurer’s Office for payment of taxes,fees, and charges. Should you opt to pay online, go towww.davaocity.gov.ph and select online payment option for furtherdetails.

3. CLAIM. Present to the Business Bureau the proof of payment andwait for the schedule of the release of your Business Permit.

For Riders who HAVE employers, ask for the original copy ofoccupational permit and copy of the receipt for delivery vehicle fromyour employer or you may secure occupational permit as walk-in incase these documents are not yet secured by the employer with therequirements and procedure below:

Requirements:1. Duly filled-out Application Form2. Certified List of Employees (with the name of the applicant

indicated or highlighted from the list)3. Health Certificate and Health I.D.

For Riders who HAVE employers, ask for the original copy ofoccupational permit and copy of the receipt for delivery vehicle fromyour employer or you may secure occupational permit as walk-in incase these documents are not yet secured by the employer with therequirements and procedure below:

Procedure:1. FILE. The Application Form can be secured at the Almendras gym orcan be downloaded at busbureau.davaocity.gov.ph.
