Request for extension of leave

Sir,

With due respect I would like to inform you that my reporting date was 26/10/2015,but unfortunately,

I will not be able to report to duty as I am suffering from high fever and severe joint pains

(polyarthiritis) from last week.Doctor has advised me to take complete rest for three weeks( Doctor's

certificate is attached).

I will be highly obliged if you kindly sanction my above mentioned days leave.
