  • We would l ike to acknow ledge the integr al r ole of Pr ofessor Antony Kidman who establ ished the H ealth Psychology Unit (H PU) in 1985 and r emained in the r ole unt i l his passing in September 2014. Dur ing his l i fet ime he was at the for efr ont of br inging evidence-based psychology to Aust r al ia and he left a st r ong legacy for psychology and phi lanthr opy in Aust r al ia.

    Pr ofessor Kidman t r ained under the pioneer s of Cognit ive Behaviour Ther apy (CBT), Pr ofessor s El l is and Beck, and was inst r umental in the disseminat ion of evidence-based ther apy acr oss Aust r al ia. H e del iver ed thousands of lectur es on CBT, pr esented at nat ional and inter nat ional confer ences, publ ished over 150 ar t icles and t r anslated psychological science into accessible, sel f-help books for the community.

    Pr ofessor Kidman sustained the cl inical and r esear ch output of the Unit at a high level of qual i t y thr oughout his dir ector ship. In 2005 he was awar ded a M ember of the Or der for Aust r al ia for his cont r ibut ions to cl inical psychology. H e also r eceived a posthumous Alber t El l is H umanitar ian Awar d for his outstanding cont r ibut ion to the f ield of Rat ional-Emot ive & Cognit ive Behavior Ther apy.

    To aid planning for this thr ee year t r ial , this year our cl inical psychologists r an a pi lot t r ial t r aining school staff in Dubbo which was a gr eat success.

    Secondly, we need to equip our school counsel lor s and teacher s w ith the addit ional sk i l ls and r esour ces to manage these pr oblems. We desper ately need to be wor k ing together to pr ovide a coor dinated appr oach for helping these chi ldr en and their fami l ies.

    With this in mind, in 2017 we moved towar ds br inging together the mental heal th sector and schools. Our 32nd year set off an exci t ing shi f t towar ds incr easing pr eventat ive wor k in schools. The number

    of pr eventat ive mental heal th talks that we del iver ed doubled and over 4000 students wer e r eached in the Sydney r egion.

    Towar ds the end of the year we star ted wor k ing on a new thr ee year pr oject t r aining pr imar y school staff in the r egional, r ur al and r emote ar eas of N SW. These r egions have been ident i f ied as having dispr opor t ionately high r ates of mental heal th issues, compar ed to their met r opol i tan

    nat ional r epor ts, r esear ch, and cl inical exper ience to deter mine how best to dir ect r esour ces to tackle the gr ow ing mental heal th pr oblem.

    In the past thr ee year s, as par t of a r andomised cont r ol t r ial , we have seen many famil ies in our t r eatment cl inic who have chi ldr en w ith emot ional and behaviour al pr oblems. What has become clear to us in wor k ing w ith these chi ldr en - and ther e ar e 3 - 4 in ever y classr oom acr oss N SW - is that our wor k needs to be

    tak ing place not just in our t r eatment cl inics, but also in the classr oom. This is essent ial for bet ter outcomes to occur.

    Ther e ar e effect ive appr oaches avai lable for these chi ldr en. H owever , the par tner ships needed between school staff and mental heal th special ists to enact them ar e not happening. This is why chi ldr en ar e fal l ing thr ough the cr acks.

    Fir st ly, we need to impr ove our systems so that school staff and mental heal th special ists ar e wor k ing together.

    The H er ald Later al Economic Index is a useful measur e of col lect ive wel l -being for Aust r al ians. I t goes beyond nar r ow economic data, adjust ing GDP to factor in changes such as health, wor k l i fe and social inequal i t y, amongst other s.

    This index shows us that two major factor s impact on wel l -being in our count r y: mental i l lness and obesi t y. Indeed, the impact of mental i l lness on society is tw ice that of obesi t y - at 207 bi l l ion dol lar s. These f igur es give the r eal

    pictur e of impair ment and cost to society of mental i l lness.

    I t is wel l accepted that the r esour ces avai lable for mental heal th fal l wel l shor t of demand and an unacceptably high number of young people and their fami l ies ar e st r uggl ing.

    This is wher e not -for -pr of i t or ganisat ions l ike H PU play an impor tant r ole. In our appr oach we synthesize infor mat ion fr om

  • AB OU T H PUThe H PU is a depar tment w ithin the Faculty

    of Science at the Univer si t y of Technology, Sydney.

    I t is a r esear ch and t r eatment cent r e dedicated to the under standing, pr event ion and r educt ion of mental heal th pr oblems in young people aged 5 to 25 year s, and their fami l ies.

    Our wor k includes:

    - pr eventat ive mental heal th talks for schools;

    - evidence-based psychological t r eatment cl inics for young people and their fami l ies;

    - evaluat ing evidence-based t r eatments thr ough the appl icat ion of qual i t y r esear ch; and

    - disseminat ing this r esear ch into the "r eal wor ld" thr ough t r aining health pr act i t ioner s, school staff and welfar e pr ofessionals.

    In addit ion to pr oviding t r eatment , the object ive of our r esear ch cl inics is to impr ove avai lable evidence-based t r eatments. We del iver t r eatment , assess the effect iveness of the t r eatment model, and this infor ms t r eatment to be del iver ed in futur e r esear ch t r ials.

    To enable young people to l ive their l ives ful ly and thr ive thr ough their for mat ive year s by the pr ovision of evidence-based psychological ther apies and r esear ch aimed at decr easing the impact of mental i l lness.

    The H PU aims to del iver evidence-based inter vent ions to pr event the development of mental heal th pr oblems in young people aged 5 - 25 year s, and t r eat those w ith establ ished psychological pr oblems. In pr oviding t r eatment , staff acknow ledge the impor tance of the young per son's suppor t system and include their fami l ies and school community in this appr oach.   • Thr ough the Side by Side cl inic we r un a r esear ch t r eatment pr ogr am for fami l ies of chi ldr en aged fr om 7 - 14 year s w ith emot ional and behaviour al pr oblems. The aim of t r eatment is to impr ove the chi ld's funct ioning and simultaneously t r ial an innovat ive t r eatment for this populat ion.

    At the end of 2017 we had completed t r eatment for 110 famil ies w ithin this r esear ch cl inic.

    Each family at tending the cl inic is al located to one of two t r eatment condit ions, ei ther Par ent M anagement Tr aining (PM T) or Col labor at ive and Pr oact ive Solut ions (CPS). PM T has been ar ound for 50 year s and is consider ed the gold standar d of t r eatment for chi ldhood behaviour pr oblems. H owever , up to 20-40% of chi ldr en to do not impr ove to a sat isfactor y level by the end of PM T t r eatment . We ar e explor ing al ter nat ive t r eatments for the thousands of fami l ies who do not r espond to PM T. This includes CPS, which is a t r eatment pr ogr am that was

  • developed at H ar var d M edical School and appr oaches chi ldhood behaviour al pr oblems in a unique way.

    Chi ldr en in the t r ial have had a r ange of pr esentat ions, w ith most exper iencing anxiety, depr ession and At tent ion Defici t H yper act ivi t y Disor der , in addit ion to behaviour al pr oblems. M any have also exper ienced addit ional di f f icul t ies including bul lying, suicidal ideat ion, poor peer r elat ionships, school avoidance, and technology addict ion. Whi lst the t r ial is st i l l in pr ogr ess r esul t s so far show that :

    M ost fami l ies have exper ienced cl inical ly signi f icant impr ovement in their chi ld's day-to-day funct ioning.

    The feedback fr om par ents has been over whelmingly posi t ive (see box on the left ). Chi ldr en have also found the pr ogr am to be useful , w i th one chi ld (7 year s) saying:

    Thoughout 2017 we r eceived a steady st r eam of r efer r als to the pr ogr am via wor d of mouth, schools and health pr ofessionals. This includes publ ic, pr ivate and Cathol ic schools, the N or th Shor e Chi ld & Youth M ental H ealth Ser vice, GPs, paediat r icians, and psychiat r ists. We w i l l cont inue to r ecr ui t thr oughout ear ly 2018, w ith a plan to f inish r ecr ui tment and t r eatment of fami l ies by September next year. Fol low-up assessments to deter mine whether t r eatment gains cont inue over t ime w i l l cont inue unt i l mid-2019.

    H er e is what one M um had to say about our pr ogr am:

    "Our daughter (13 year s) used to have massive meltdowns at least tw ice a day. She was hi t t ing and k ick ing k ids at school and they wer e thr eatening to suspend her. She just couldn't manage her own emot ions i f something upset her. Yet after i t happened she would cr y and cr y: 'M um, why am I l ike this?' Our l ives wer e incr edibly har d when my sister showed me the f lyer for Side by Side and we wer e accepted in to the pr ogr am.

    She has become mor e expr essive since star t ing the pr ogr am. She doesn't lose her temper to the same intensi t y as she did befor e and i t occur s less fr equent ly. I feel in cont r ol again! We ar e much mor e awar e of how our r eact ions can al ter her s and we ar e much less r eact ive than we used to be.

    H er teacher is happier and the over al l st r ess levels in our fami ly ar e much, much bet ter than when we f i r st came..."

    " .....I didn't want to come to the cl inic and wouldn't get out of the car for a whi le. I t sounded l ike I was going to get into t r ouble, and I didn't know what they would be doing. I f i t was l ike the doctor last t ime, we had to wait for ages and i t was so bor ing.

    I t wasn't so bad though. The lady Yul i was r eal ly nice and k ind. When I f i r st got ther e we played Jenga and Connect 4, whichever one I picked. I also made fr iends w ith another gir l in the k id's r oom and we dr ew pictur es together.

    I got to talk about things that we wer e f ight ing about about and I k inda l ike i t when we ar e al l together w ith Yul i . I 'm bet ter at talk ing and expr essing myself when I get angr y and my par ents don't get fr ust r ated and yel l as much as they used to. I was a bi t sad when we had to stop going..."

    Thanks to the gener osi t y of the M ar y Al ice Foundat ion, we wer e able to offer fr ee talks to schools acr oss Sydney on the topic of M anaging H SC St r ess. These talks wer e del iver ed to students in Year 11 and Year 12 at high schools acr oss the Sydney r egion.

    This is a signi f icant incr ease compar ed to the pr evious year , and sever al schools have alr eady r e-booked talks for 2018.

    These seminar s ar e designed to educate students about st r ess and teach them cognit ive and behaviour al coping st r ategies to assist them thr ough the demands of the H SC year. Our talks ar e pr act ical and inter act ive, and they have been developed in r esponse to teacher and student feedback. In r esponse, 100% of teacher s have said they ar e sat isf ied or ver y sat isf ied w ith the talks.

    We have also del iver ed H SC St r ess M anagement talks for par ents who ar e along for the r ide of the H SC year. These talks offer evidence-based st r ategies to help par ents under stand adolescent mental heal th and manage their own psychological wel l -being.

    In 2017 we also del iver ed teacher t r aining on the topic of adolescent mental heal th.

    In 2017 we welcomed two addit ional cl inicians to the Gener al Psychology Cl inic, Dr L isa Gomes and Dianne Fi t zger ald. The cl inic cont inues to gr ow as we see chi ldr en, adolescents and adults for a w ide r ange of pr esent ing pr oblems. These include anxiety, mood disor der s, schizophr enia, r elat ionship pr oblems, t r auma, st r ess, and par ent ing issues.

    The Col labor at ive and Pr oact ive Solut ions (CPS) Cl inic is expanding as mental heal th pr ofessionals and member s of the community become awar e of the ser vices that we pr ovide. Famil ies of chi ldr en w ith behaviour al issues, who ar e unable to par t icipate in the Side by Side pr ogr am, ar e able to r eceive t r eatment thr ough the CPS cl inic.

    Cl inicians in both the Gener al Psychology and CPS Cl inics use evidence-based, pr act ical st r ategies to help their cl ients. These include cognit ive behaviour ther apy, mindfulness and acceptance st r ategies, schema ther apy, and at tachment ther apy. They r egular ly l iaise w ith r efer r er s to pr ovide an opt imal level of car e. Our staff also r egular ly at tend pr ofessional development seminar s on a r ange of topics to cont inue to develop their cl inical sk i l l set .

    We pr ovide bulk bi l l ing ser vices to cl ients who ar e in f inancial st r ess.

    Appr oximately 50% of our cl ients r eceived

    evidence-based ther apy at no cost .

    One of the faci l i t ator s r epor ted the fol low ing: "One guy stated that just one session in the Gr ow th Room was far mor e empower ing than al l pr evious help he had r eceived. This guy?s l ight bulb moment was open communicat ion. H e is on cloud nine! H is r elat ionships have taken off and he feels amazing."

    Alongside the Gr ow th Rooms, The Banksia Pr oject also r an thr ee lar ge community for ums aimed at r aising mental heal th awar eness for the gener al publ ic. These for ums wer e held in univer si t ies acr oss Sydney and ar e cal led the Gar den Rooms. The Gar den Rooms have at t r acted many young men seeking help as wel l as fami ly member s and fr iends who ar e looking for guidance as to how best to help their loved ones. Our dir ector , Dr Rachael M ur r ihy,

    chal lenges and r esponsibi l i t ies of adult l i fe.

    Banksia has a number of inter vent ions. The main component is the suppor t gr oups (Gr ow th Rooms) for young men w ith mental heal th issues acr oss Sydney.

    Our cl inical psychologist , Louise Rémond, t r ained lay faci l i t ator s to lead the pr ogr am. She developed the for mat and content of the pr ogr am sessions, each of which highl ighted di f fer ent evidence-based st r ategies.

    These wer e del iver ed to the gr oups by the faci l i t ator s and M s Rémond pr ovided cl inical super vision thr oughout the r ol l out of the pr ogr am.

    This year we have been heavi ly involved in The Banksia Pr oject (TBP), an ini t iat ive of the St . Ignat ius Col lege Alumni.

    The Banksia Pr oject was set up by member s of the community to addr ess the r ising r ates of mental heal th pr oblems and suicide in young men. This is a gr oup that t ypical ly r ejects the t r adi t ional face-to-face counsel l ing model.

    Banksia is innovat ive because i t offer s new ways of suppor t ing men?s mental heal th that r epr esents an al ter nat ive to the t r adi t ional counsel l ing model.

    We wer e par t icular ly inter ested in engaging in a pr oject that ser viced men in the 18-25 year age gr oup who wer e at r isk as they st r uggled to negot iate the

    With al l t r aining completed, M s Rémond w i l l now step away fr om this r ole and the faci l i t ator s w i l l move ahead independent ly, ful ly t r ained and equipped to r un the gr oups sustainably into the futur e. M oving for war d, we w i l l cont inue to be involved in the Gar den Rooms in an advisor y capaci t y, ther eby helping to r aise awar eness of mental heal th issues.

    The f inal event for the year

    was The Banksia Bal l , which was held at the H i l ton Sydney to help fund mental heal th and wel lness pr ogr ams r un by The Banksia Pr oject in the community. Tim Webster , television and r adio pr esenter and spor ts br oadcaster acted as the M aster of Cer emonies, whi le r enowned businessman M ar k Bour is AM was the keynote speaker. H e spoke about the impor tance of sel f-car e and how tak ing car e of his physical health

    was involved at each for um as their r esident psychologist on a quest ion and answer panel.

    At the f i r st for um in Febr uar y, Dr M ur r ihy was joined by eminent individuals including Pr ofessor Gor don Par ker (Scient ia Pr ofessor of Psychiat r y, Univer si t y of N SW), M itchel l Pear ce (Rooster s captain), and M ar gar et Cunneen SC (Dir ector of Publ ic Pr osecut ions). At this event , M r Pear ce spoke openly about his own mental heal th issues and st r ategies that helped him get his l i fe back on t r ack. Of mental i l lness amongst r ugby player s, he said, "The mor e someone who has gone thr ough a bi t of stuff talks, ther e is hope i t gets easier " .

    The next Gar den Room in M ay was an event focused on 'M ental H ealth in Spor t ' held at the Univer si t y of Sydney. We hear d fr om leading spor tsmen, including M at t Shir vington (athlete) and Wayne Schwass (for mer AFL player ) r egar ding mental heal th issues athletes ar e facing and how this affects the br oader community.

    This was fol lowed up by an event in N ovember on 'M ental H ealth in the Wor kplace', which was held

    M s Rémond and Dr Sar ah Edelman appear ed on Channel 9's 'Today Ext r a' to discuss their book 'Good Think ing: A Teenager s' Guide to M anaging St r ess and Emot ions Using CBT'. In this segment they out l ined pr inciples of cognit ive behaviour ther apy and st r ategies for managing anger. They also pr ovided psychoeducat ion r egar ding anxiety and depr ession, and st r ategies for r educing st r ess and impr oving mood. M s Rémond and Dr Edelman's appear ance on this segment t r igger ed conver sat ion on Today Ext r a's social media accounts r egar ding mental heal th issues in young people.

    M s Rémond also conducted sever al r adio inter views on the topic of managing H SC st r ess and teenage mental heal th. This included inter views w ith ABC Radio AM , ABC Radio N ight l i fe, ABC Radio M or nings w ith Wendy H ar mer , and 2ser. In these inter views she discussed issues r elat ing to whether mental heal th disor der s wer e wor sening in young people, or whether as a society we wer e just hear ing mor e about i t . She also discussed ways to r educe st r ess amongst students complet ing the H SC.

    Dr M ur r ihy appear ed on the Think:H ealth segment of r adio stat ion 2ser to discuss the topic "When your chi ld doesn't r espond to gold star s and scolding". In this segment she out l ined featur es of PM T and discussed how CPS is an al ter nat ive st r ategy that par ents can use to help inst i l l behaviour s in their chi ldr en that they want to see.

    Best sel l ing author of 'Change Your Think ing', Dr Edelmand, and H PU's M s Rémond, publ ished a sel f-help book for young people exper iencing negat ive emot ions, st r ess, and sel f-defeat ing behaviour s. The book is based on the pr inciples of cognit ive-behaviour al ther apy (CBT) and i t explains how to use CBT ski l ls in a pr act ical , clear way.

    This two volume handbook addr esses the state and pr act ice of school psychology w ith a focus on standar ds unique to Aust r al ia. I t is consider ed to be a major r efer ence text for school counsel l ing staff in Aust r al ia.

    Dr M ur r ihy co-author ed a chapter r eview ing the theor et ical under pinnings of Disr upt ive Behaviour Disor der s (DBD's). This chapter out l ined evidence-based inter vent ions for chi ldr en w ith behaviour al disor der s.

    The book chapter is a vi tal r esour ce that is being ut i l ised by psychologists Aust r al ia-w ide who ar e wor k ing w ith t r oubled youth.

    Staff member s ser ved as commit tee volunteer s for the 38th N at ional Confer ence of the Aust r al ian Associat ion of Cognit ive Behaviour al Ther apy. The theme of the confer ence was 'Innovat ions in cognit ive and behaviour al ther apies acr oss the l i fespan'.   • I t is of vi tal impor tance that the effor t that goes into conduct ing r esear ch into new t r eatments is matched by a simi lar effor t in ensur ing these t r eatments ar e disseminated to school and mental heal th pr act i t ioner s. This way evidence-based t r eatments can r each the community. With this in mind, we have commenced our plan to disseminate CPS to the community.

    Our pr imar y focus w i l l be TH RIVE: A M ental H ealth Pr oject for Chi ldr en in Regional, Rur al and Remote N SW. M ental heal th pr oblems ar e higher for young people outside of met r opol i tan r egions and ther e ar e also some unique bar r ier s to accessing car e. Values of stoicism, so helpful in many ways in the bush, can be unhelpful in ter ms of w i l l ingness to seek car e. Concer ns about a possible lack of pr ivacy can also be a bar r ier i f one ant icipates that a visi t to a mental heal th pr ofessional is l ikely to be mor e visible than i t might be for people seeking assistance in ci t y ar eas.

    With this in mind and in an at tempt to avoid any potent ial ly pr oblemat ic st igma, we decided that to help chi ldr en in non-met r opol i tan ar eas, we would need to del iver these inter vent ions into their ever yday l ives. H ence, teacher s and school counsel lor s w i l l be t r ained in the CPS model and later suppor ted thr ough case consultat ion via Skype to lear n social-emot ional competencies that w i l l then be passed on to their students. They w i l l use the CPS st r ategies univer sal ly in the classr oom, but also speci f ical ly w ith tar geted chi ldr en. In the past six months we have conducted a pi lot t r ial at a school in Dubbo that has infor med the lar ger thr ee year r ol l out of TH RIVE in the school set t ing.

    Over thr ee year s we w i l l t r ain teacher s in 192 schools w ith a potent ial r each of 12,000 students. The planning for this pr oject has commenced and we expect to be out in the r egional cent r es by August 2018.We also hope to impar t CPS ski l ls to mental heal th pr act i t ioner s acr oss Sydney and, in keeping w ith this aim, we have secur ed an ini t ial gr ant to del iver CPS t r aining in M ar ch next year to community mental heal th and hospital staff at the South East Ar ea H ealth Ser vice based at the Pr ince of Wales H ospital Randw ick.
