Regarding registration of ANM Nursing students, the registration fees for each student is Rs. 300/- (Rupees three Hundred only) and the total amount should be submitted in shape of Bank Draft of State Bank of India only infavour of Registrar, Odisha Nurses and Midwives Council, Bhubaneswar, payable at Govt. Treasury Branch, Bhubaneswar along with following documents attested by the Principal (Application of the students should be in the alphabetical order):

i. Attested Photo copy of H.S.C Certificates.

ii. Attested Photo copy of Provisional/Pass Certificate issued by the concerned Affiliating/Examining Body for ANM. (The Original ONMEB Certificate should be signed by the candidate).

iii. Attested Photo copy of Course Completion/Training Leaving Certificate from the Principal Tutor of the concerned institution. (Specify the date, month & year)

iv. Recent three copies of passport size colour photograph self attested by the candidate.

Submit D.D. and the above documents of all pass out students along with the prescribed registration format duly filled in by the candidate and forwarded by the Principal Tutor/ Head of the institution.

Registration Renewal Particulars

1. Registration Renewal is done for once in every five years. 2. Presence of applicant/candidate is mandatory. 3. Applicant/candidate must bring the original Registration

Certificate along with him/her. 4. Submission of fee of Rs. 200/- towards Renewal of Registration

Certificate & Rs. 300/- for Part ‘A’ & Part ‘C’ Registration certificates.

5. Registration Renewal fee of Rs. 200/- (Rupees Two Hundred only) & Rs. 300/- (Rupees Three Hundred only) for Part ‘A’ & Part ‘C’ Registration certificates should be submitted in shape of Bank Draft of State Bank of India only infavour of Registrar, Odisha Nurses and Midwives Council, Bhubaneswar, payable at Main Branch, Bhubaneswar.

Late Fees Structure For Non Renewal

Upto three months from the date from which registration is due for renewal-No fine. Rs. 200/- (Rupess Two Hundred only) Rs. 300/- (Rupees three hundred only) for Part ‘A’ & ‘C’ Registration Certificates.

For period more than three months from which registration is due for renewal upto five years. Additional Rs. 500/- (Rupees Five Hundred only)

For period more than five years from which registration is due for renewal. Additional Rs. 1000/- (Rupees One thousand only)

Received of Renewal Certificate from 11.00am to 01.00pm.

Lunch time from 01.00pm to 02.00pm.

Official processing for renewal from 02.00pm to 03.00pm.

Issue of Renewal Certificate 03.00pm to 04.00pm.
