RELEASE: By signing below, I hereby authorize the staff of Artistic Motion Dance to act for me according to their

best judgment in any emergency requiring medical attention, and I hereby waive and release Artistic Motion Dance

and its staff from any and all liability for any injuries or illnesses while going to and from and while at Artistic Motion

Dance studio. Any and all medical expenses incurred will be my responsibility. I have no knowledge of any physical or

mental impairments that would be affected by the student’s participation in the Artistic Motion Dance Program.

2015-2016 Policies

Please read and initial below, indicating that you understand and agree to the following:

The dance year begins August 3, 2015 and classes run until May 28, 2016. All students will be enrolled in classes

Until May 28th unless withdrawal notice is given. Tuition is due the 1st of each month and will be

charged unless a WITHDRAWAL FORM has been received by AMD.

The cost of lessons is based on a 10 month tuition. Payment can either be made for the full season, or be

divided into equal payments for August through May. If you have classes that fall on a holiday, make-ups may be

made,

If a student misses a class, he/she may do an appropriate make-up class any time during the dance year, prior to

the end of the session in May. Credit is not given for absences.

If a student is going to be late to class, please call the front desk and they will attempt to get word to the

teacher. If a student is going to be more than 10 minutes late their admittance to class is at the discretion of

the teacher. The teacher will take into consideration whether or not the class is already warmed-up and if it

would be unsafe for the late dancer to just join class and also determine how disruptive the late arrival would be

to the rest of the class. If a teacher determines the student is unable to attend class, make-ups may be made.

If a student finds it necessary to drop a class, please notify the front desk. If a student needs to

withdraw from all classes, a withdrawal form must be completed, as tuition will continue to

accumulate until the Withdrawal Form has been completed and signed. The changes become effective the

first of the following month. Conversely, please advise the front desk if your dancer wishes to enroll in

additional classes after initial registration.

We do enforce a DRESS CODE. To “look like a dancer” encourages one to conduct oneself like a dancer.

Students not dressed according to the dress code will not be permitted to take class. Make-ups can be done

for missed classes. Please check the dress code(s) for your dancer’s class(es).

ARTISTIC MOTION DANCE is hereby granted permission to take photographs of students for use in

brochures, web sites, posters, advertisements and other promotional materials created by the school and/or

performing company. Permission is also hereby granted for the school and/or performing company to copyright

such photographs in its name.

Artistic Motion Dance reserves the right to suspend or remove any individual from class in the event of

behavioral problems, with an understanding that if the individual is removed, there will be no credit given. Artistic

Motion Dance is not responsible for material, injuries, or liabilities.

Tuition Payments are due each month on the 1st. Each family will be required to have a credit card on file. If

tuition payment is not received by the 10th of each month, a $10 late fee will be applied, and this card will be charged.

If you do not wish to provide a card number, we must have 6months tuition paid with registration.

Registration FAQs

When does registration for the 2015-16 season begin? Registration for the 2015-16 season begins on Saturday, May 23rd and continues until January 31st, 2016, or whenever classes have reached full enrollment.

Is it important to register early? Yes! The earlier you register, the more options there are available for your dancer. Classes will fill up quickly. Save your dancer’s spot by registering early and paying the registration fee. Should any class be at full enrollment, you can elect to be place on a waitlist in the event we have any dancers withdraw. We recommend having a few options when registering, just in case. Also, families registered before June 1st will be entered into our annual drawing to win a free month of tuition!

Do I need a new registration form if my dancer is a returning student? Yes, all students need a new registration form on file for the 2015-2016 season. Thank you! This is my dancer’s first dance class at AMD, how do I know which class would be appropriate? The AMD Staff would be more than happy to help guide you to a good fit for your dancer. Each new dancer is also more than welcome to try a class, free of charge, before registering. Be sure to pay close attention to ages and school grade levels that are listed on the class schedule. We use August 31st as our age cutoff, to ensure that dancers remain with other students in the age range. My dancer would like to audition to be a part of the STARS Performance Team or the AMD Company. Do I still need to register?

Yes Please! If you feel confident that your dancer will be selected for either our STARS Perfor-mance Team or Competitive Company, just write STARS or Company on their Registration Form, and we will enroll them in the appropriate classes after auditions. If you are unsure about our dancer’s chances, please speak with an AMD faculty member for a better assess-ment.

Please take some time to read through the following packet of information to get to know all about

AMD. Our amazing Faculty is dedicated to passing on their love of dance to your child. Classes are

offered in variety of styles, so there is a perfect fit for every dancer. We have open classes in:

combination classes, ballet, jazz, hip hop, lyrical, tumbling, and theater. We are also pleased to offer

both our STARS Performance Teams and Competitive Companies. All dancers are welcome to try

out a class to see what is the best fit for them. Please don’t hesitate to call or stop by the front desk

with any questions. Our knowledgeable staff can assist with everything from accounting to class

selection.

Our schedule begins August 3rd and concludes in June with our Annual Recital on June 4,

2016. This performance features all of our dancers at AMD. It is a great way for dancers to

showcase their hard work to family and friends. Additional detailed Recital information will be

available throughout the year!

Tuition is due on the 1st of the month. Each family will be required to have a credit card on file. You may elect to have us charge your card automatically each month, which will be done the 1st-3rd. If you are not set up for automatic monthly payments, a Late Fee of $10 WILL be added on the 10

th,

and your card will be charged the monthly tuition and late fee on the 11th.

All monthly rates are SPECIAL RATES. It is the same regardless of whether there are 3,4, or 5 weeks in a month. If you don’t pay monthly, the regular rate per class is $20 per hour.

No REFUNDS. No EXCEPTIONS!

Returned check fee is $25, plus the late fee.

Payments are applied to oldest balance first.

When enrolled, each student is enrolled in class through May 27th, until officially withdrawn.

Withdrawal from classes requires written notice before the first of the month. If withdrawn after the 1st, refund will not be prorated but payments will stop the beginning of the next month.

All classes are subject to minimum class size and teacher availability.

Students will not be allowed into class if tuition has not been paid. Dancers will not perform or compete if tuition is overdue.
