LETTER FROM THE EDITOR Greetings Three Rivers AAHAM Members! This past March, I accepted the Communications Chair position for the Three Rivers Chapter. I am so excited to be working with such a wonderful organization and with some amazing professionals as well. So you have a little background, I am currently the Manager of the Outpatient Centers for St. Clair Hospital. Funny thing is that I used to fear finance and revenue cycle management. In fact, I would be comfortable in saying I hated it! But let’s face it, these areas are valuable pieces of the daily operations of any organization, and there is no escaping it! So I pushed myself, on purpose, to increase my understanding in these areas, and somehow I ended up loving it! The fall conference in Nemacolin was my first AAHAM chapter event and I have to say, I was extremely impressed by the content and quality of the speakers. From Social Networking to the 2016 Presidential Campaign, there was so much to learn and benefit from. However, I was surprised that not more members were present to enjoy the presentations. I learned more over these two days than I had since college and had fun doing it! My goal for this year is to increase awareness about the Three Rivers AAHAM chapter and promote the amazing educational events that it has to offer. Through newsletters, social networking and other promotion efforts, we hope to draw in a larger pool of members. I plan on working with Stephanie Snider, our Membership Chair to broaden our recruitment efforts to not only working revenue cycle managers and staff, but also the potential graduates and new professionals from local Universities that will soon be joining the healthcare arena. What can you do to help? GET INVOLVED! Attend chapter conferences. Refer a friend. Encourage your colleagues to attend an upcoming event! PRESIDENT’S LETTERBRENDA FRAAS CRCE-I | PRESIDENT OF THREE RIVERS AAHAMI cannot believe that I have been your Chapter President for almost a year already! I was very intimidated by the shoes I had to fill as Barbara Winwood had done a fantastic job growing our chapter and keeping it strong. I hope I can continue to grow our Chapter and do the same in the coming year with all of the fantastic members we have!!!! Three Rivers AAHAM celebrated our 30th anniversary this year. We have had some of the best Presidents and Board of Directors that a Chapter could have. They have led the way over the years in educating the industry on what is happening. Healthcare is constantly changing and AAHAM lets you connect with people on the issues. I remember years ago, our interim Director at UPMC told his management team we needed to get involved with a local healthcare organization and start networking to find out what is happening with other facilities. I chose AAHAM and a few years later when Lisa Hennigan asked me to become a member of the Board as Certification Chair, I was hesitant, but honored that she asked me. Even though I have not been involved as long as some of our members (Nancy Baker, Bob DeBiase, Kathy Sandora, Carla Fayak, and Deb Bizich just to name a few), I can honestly say I love being involved. I am hoping as Three Rivers AAHAM continues to grow, Kandi Housley, Stephanie Snider, Alyshia Inks, Sarah White, and Melissa Gazis have the love in their heart for AAHAM and learn the ropes so they can take over the realm.

I have worked in the healthcare industry for over 20 years and while it is always changing, it seems the mentality of the people does not. We are always looking for what we can do better to help care for our patients, to serve our communities and to survive in this ever changing environment! Regardless of whether we work in patient access, patient accounting, in a physician office, for a pharmacy or anywhere else in the healthcare arena, we all realize everyone needs to work together for the same purpose ~ our patients! AAHAM has the same philosophy and goals:

• How can we better serve our members (who serve the patients)?• How can we serve the communities our members work in?

• What are the burning and ever changing hot topics that we can provide education on so our members can survive in this ever-changing healthcare environment?

That is what makes AAHAM such a worthwhile organization to be a part of. Besides the education we receive at our meetings, we have an opportunity to network with people from all over our region who are dealing with the same issues you are dealing with every day in healthcare.

We continue to face healthcare challenges in the ever-changing environment, and we realize we need to continue to focus on Consumerism. But we are also finding a need to expand our horizon with our educational opportunities to help the Physician Division, as well as other areas, for example, Pharmacy. We are always open for suggestions for topics and speakers for our meetings, meetings, and newsletter articles. Three Rivers AAHAM is always interested in receiving valuable information on what is happening in our industry and how your organization is handling these changes. Do not hesitate to reach out to anyone on the Board if you have a topic you would like to learn more about, or if you know a great speaker who can educate our organization.

Three Rivers AAHAM has an AWESOME Board of Directors who are always ready, willing and able to pitch in to make this year a great one. We also have additional members who are willing to step forward and participate as Committee Chairs. Thank you to all of them! I would love to see our chapter grow this year so that our message and efforts can be spread throughout Western Pennsylvania. Tell your peers and co-workers about AAHAM & GET INVOLVED!!!

Did you attend the Fall Conference? If not, boy did you miss out on an awesome agenda, lots of networking, and an amazing venue.

Our Keynote speaker, Matt Jones gave us an inspirational and motivational talk to get us fired up and pumped up about working in Healthcare. Matt is a 3-time cancer conqueror that began running marathons and now is an inspirational/motivation speaker. Matt’s presentation got everyone on their toes and shouting “VICTORY”. Matt provided those who attended the presentation his most recent book “Life is a Marathon” with his autograph. Every topic within the agenda was a hot topic within the Healthcare industry. Don Paulson (The Revenue Group) provided a frightening presentation on the increase in BAD Debt. Don provided strategies including patient-friendly billing, pricing transparency, point of service collections and enhancing patient satisfaction. Anthony Catalano (Securestate) presented on Cyber Awareness which had each of us holding on to our seats as we begin thinking about how we can ensure that our daily passwords are unique to avoid hackers hacking into our personal and work accounts. Social Media is very popular within our current society. Matt Gentile (IMG Digital Group) presented on the value proposition of social media. He got several of us thinking and linking up to those sites! What an amazing presentation by Sara Emery and Kimberly Scaccia (KPMG) providing all with ways to improve the revenue cycle processes that allow you to increase efficiency, reduce cost and shore-up sagging margins. These two ladies had us thinking of ways that our departments can work more efficiently by identifying the root cause. Karen Shaffer-Platt (UPMC) provided us with an excellent presentation on Patient Consumerism. In today’s Healthcare, patients are shopping around for the most affordable pricing for healthcare. They want better experience, whether it is customer service, technology or convenience. Karen provided ways to make changes within our organization through training of customer service staff, providing the technology that suites the appropriate audience, as well as convenience for service delivery. Is your organization prepared for the Federal Regulations 501 R requirements? Jeff Porter (UPMC) provided a high level overview of the requirements and shared some of UPMC’s approaches that are meeting the guidelines. With the election in November, Paul Miller (Miller/Weinhold Capitol Strategies, LLC) presented a very interesting presentation on ‘The Year Washington was turned upside down’. Paul presented updates on what is happening in today’s election as well as other healthcare topics.

As we continue to work through the topics from the conference, let’s remember one thing that Matt Jones mentioned during his presentation, “I’m too BLESSED to be STRESSED”.

CERTIFICATION SCHEDULE

GOVERNMENT RELATIONS UPDATE LEGISLATIVE DAY 2016 | Ken Krieger, Government Relations ChairOur state and particularly our Chapter were well represented at this year’s Legislative Day on April 26. Monday evening April 25, Brenda Fraas, Bob DeBiase, Lisa Hennigan and I got to sit in with newcomers for some training and reminders about how to best utilize our time with the Representatives and Senators’ staff. It is rare to actually meet with the Congressmen and we’ve learned over the past ten years that it is the staffers who we really need to see anyway, as they are the ones who meet, consider, vet things out and make recommendations to our elected officials. On actual “Leg Day”, prior to “storming the Hill”, with our brethren from other Chapters across the country there are several speakers of note who provide their take on our proposed legislation, as well as offering additional slants on what is up in DC in general. Minnesota Rep. Tom Emmer spoke to us this year and we were also lucky enough to have the primary author of the HIP Act, Lisa Grabert of the House Ways and Means Committee, to get us pumped up for the short walk up to the Hill.

So up we go! The afternoon is spent visiting our local Representatives (by zip code, for example, I am 15243 so was assigned Tim Murphy) and our two Senators – Bob Casey and Pat Toomey. We usually split up the Pennsylvania group since some appointments overlap and there are so darn many of us, but we do catch ‘em all. As well as following up on the TCPA, our charge was to deliver position papers on and speak about two other pieces of legislation: The HIP Act and the Medicare Audit Improvement Act.

The HIP Act has been created in response to many issues that exist with the current Medicare payment systems such as the issues between payment systems (IPPS and OPPS), definition of a short stay, the two midnight rule, and reform to the RAC program. The Medicare Audit Improvement Act is specific toward making RAC auditors more accountable, including fewer requests for documentation as well as publicizing a scorecard for each RAC and how they are actually performing (or not!).

So, Bob and Lisa and I did such a great job in convincing Congressman Murphy’s Legislative Director, Scott Dziengelski, about the merits of these bills (oh, plus it’s an election year) that he got us a photo-opportunity with the Congressman! The last time that happened with our group was back when Rich Santorum was a Senator (also an election year but still pretty cool). Leg Day 2016 was a winner!

Any and all Local and National members are encouraged to attend Leg Day. If you cannot, please support those who do so on our behalf by reading and endorsing any papers we may share, expressing your views directly to our Leadership so they can pass them on, and/or broaching hot topics that you feel need brought to the attention of Congress. Below is a brief overview copied from the National AAHAM website that provides more detail on the Legislative Day event and what you and your Facility can expect to get out of it:

Represent your facility, your state and your industry at the one political event that directly impacts you, your facility and your bottom-line; AAHAM Legislative Day. Our strong grassroots program creates new opportunities for our industry but we need your continued support! We need you to make the trip to Washington to make a difference in our industry, attendance is limited, so register to attend today. It is all about strength in numbers. The more AAHAM members participate in this effort, the stronger the voice AAHAM will have. Remember, if you don’t speak up someone else will.

PENNSYLVANIA TRAUMA CENTERS AND WORKERS’ COMPENSATION REIMBURSEMENT UNDER THE PENNSYLVANIA WORKERS’ COMPENSATION ACTCharles J. Hilton, EsquireFor the last 23 years, specifically, since the 1993 Medical Cost Containment Regulations were promulgated, Level 1 and Level 2 trauma centers in Pennsylvania have been paid at 100% of billed charges for providing emergent trauma care to injured workers whose injuries met the threshold of life-threatening or urgent injuries. However, in 2016, two Commonwealth Court cases in Pennsylvania ruled that under the 1993 workers’ compensation regulations, Level 1 and Level 2 trauma centers are not entitled, carte blanche, to be paid their actual/ full billed charges, but to be paid at 100% of usual and customary charges. The definition of “usual and customary charges” in the workers’ compensation regulations is as follows: “The charge most often made by providers of similar training, experience and licensure for a specific treatment, accommodation, product, or service in the geographic area where the treatment, accommodation, product or service is provided.”

The Commonwealth Court, however, has gone to great lengths to state, unequivocally, that charges from trauma care providers are to be compared only to charges of other Level 1 and Level 2 trauma care providers. As such, any workers’ compensation insurer challenging the treating trauma provider’s charges will be facing an uphill battle because of the stringent standards set by the Commonwealth Court. In workers’ compensation fee review litigation, the burden is always on the insurance company to prove that they paid the treating provider the correct amount under the Pennsylvania Workers’ Compensation Act. So, in light of the workers’ compensation regulations definition of usual and customary charges, the insurance company will most likely have to elicit testimony and/or data from an accredited trauma center in the same geographical area that its charges for trauma care are significantly lower than the treating trauma center charges. Again, since the burden is on the insurer, the insurance company will have to move forward with this posture in any litigation before the treating provider will be required to respond. Accordingly, if a treating trauma center files an Application for Fee Review after receiving less than full payment for a strong trauma case, and the Bureau of Workers’

Compensation Fee Review Section does not award the treating trauma center any additional amounts, the treating trauma center should still file an appeal to Contest the Fee Review Determination and put the insurance company in the position of proving that the treating trauma center’s charges are significantly higher than other trauma centers for the same service in similar geographic areas. Again, it is important to note, with regard to Fee Review appeals, that the workers’ compensation insurance company always has the burden of proving that it paid the treating provider properly, no matter who wins or loses at the lower Fee Review level.

In conclusion, despite the recent decisions by the Commonwealth Court in 2016, going forward, treating trauma providers should continue to file Fee Reviews in the same manner. In the event that the workers’ compensation insurer does not pay full billed charges for strong trauma accounts, the treating trauma center should continue to file an Application for Fee Review, and if the Fee Review Section continues to award the treating trauma center full billed charges, I would expect various insurance companies to appeal based on these recent Commonwealth Court decisions. However, if the insurance companies do appeal an underlying Fee Review determination that the treating provider is due its full billed charges, the insurance company will not have an easy case, because the burden of proof in Fee Review cases is always on the insurer to prove it paid the treating provider correctly. Essentially, the burden will be on the insurer to prove that the treating provider’s trauma charges are above market price in Pennsylvania.

This is the current state of the law with respect to reimbursement for Level 1 or Level 2 trauma centers in Pennsylvania that treat injured workers with compensable claims under the Pennsylvania Workers’ Compensation Act. MACRA UPDATEDonna G. Trudgen, CRCE-P, Manager, Revenue Cycle, Children’s Community PediatricsHealthcare is bracing for a major overhaul in how physicians are paid for Medicare Part B services. In April 2015, legislation was passed to begin the transition from fee-for-service to fee-for-value. The legislation, Medicare Access and CHIP Reauthorization Act of 2015 (MACRA), ends the Sustainable Growth Rate formula.

The legislation requires reimbursement be tied to better clinical outcomes and costs reduction through patient engagement, use of technology and process improvement. All eligible providers will select a method of participation. Eligible providers are identified as:

• Physician• Physician Assistant• Nurse practitioner• Clinical nurse specialist • Certified registered nurse anesthetist.

The methods include an Advanced Alternative Payment Model (APMs) or a Merit-based Incentive Payment system (MIPS). APM requires enrollment in an ACO while MIPS participation closely resembles a traditional reimbursement process. Regardless of the path of participation, calendar year 2017 is the first reporting year. Data submission for 2017 is required by March 31, 2018. Medicare will provide feedback to providers based on data submitted and payment adjustments (+ or -) will take place effective January 1, 2019. If you do not submit data for calendar year 2017 you will automatically receive a 4% payment reduction.

There are exclusions for eligible providers that bill Medicare less than $30,000 in claims and provide care to fewer than 100 Medicare members.

MACRA also requires the removal of Social Security Numbers from all Medicare cards by April 2019. SSN identifiers will be replaced with Medicare Beneficiary Identifier (MBI). New cards will be mailed to members beginning April 2018. The transition period for issuing new cards is expected to run through December 31, 2019. The new MBI will be 11 characters in length and consist of numbers and uppercase letters. No special characters will be used in the MBI.

Although MACRA includes reporting quality measures similar to PQRS, Meaningful Use and the likes, it is important to research and learn as much as possible about these program to avoid the payment penalties for your providers/organization.

The Chinese Auction raised over $600 dollars for Children, Youth and Families (CYF) - Allegheny County. We sincerely appreciate everyone who donated a basket and participated in the auction! We also would like to thank all who provided toys for the children, it will be a nice Christmas for those in need! Happy Holidays!

CRCP ~ CERTIFIED REVENUE CYCLE PROFESSIONAL is intended for all supervisors and managers in the Revenue Cycle. It is a comprehensive 4- hour exam that covers Revenue Cycle subject matters including Patient Access, Billing, Credit/Collections and Revenue Cycle Management. The exam is comprised of true/false and multiple-choice questions. AAHAM offers two types of Professional certification: one for institutional (hospital, health system) and the other in a professional (physician, clinic) setting.

CRCS ~ CERTIFIED REVENUE CYCLE SPECIALIST is intended for Revenue Cycle staff with responsibilities in Patient Access, Billing, Account Resolution, Denial Management, Collections, Cash Posting, Customer Service and Self-Pay collections. It is a 2-hour exam that requires working knowledge of the Revenue cycle. The exam includes relevant regulations and acronyms are comprised of three multiple-choice sections. AAHAM offers two types of Specialist certification: one for institutional (hospital/health system) and the other in a professional (physician/clinic) setting.

CRIP ~ CERTIFIED REVENUE INTEGRITY PROFESSIONAL is intended for anyone in the Revenue Cycle industry to help ensure facilities effectively manage their charge master and bill and document appropriately for all services rendered to a patient. The exam is a 4-hour exam.

CCT ~ CERTIFIED COMPLIANCE TECHNICIAN is intended for all Revenue Cycle staff who must meet employer’s annual compliance training requirements. The compliance exam covers Fraud & Abuse, HIPAA, Administrative Sanctions, and RACs. The exam is 90 minutes.
