Referrals
Referrals to TLC’s residential services are made by regional center case managers, teachers, social service agencies, family members and any other concerned individual who has received consent to refer. Visit us! We encourage family members to visit TLC in advance of the need for residential services. If TLC is not the right fit, our professional staff is ready to guide you to other resources to find the one that is.

Residential Living at TLCAdults ages 18-75
TLC is a network of living centers offering residential programs for those with blindness and multiple disabilities. A talented and compassionate staff dedicates itself to helping individuals who have a variety of intellectual and/or physical disabilities. As those we serve begin to grow and thrive, they find the joy that comes from learning to help themselves. Because of the spirit of the people served, our centers have become “homes” in the truest sense of the word.
Service Delivery
TLC’s community homes offer a loving and secure environment for adults. An interdisciplinary team of professionals in diverse specialties (ranging from case management services to orientation / mobility instruction) ensures ongoing individualized assessment and effective program plans.
Intermediate Care Facilities
Ten nurturing homes for six residents
Two nurturing homes for twelve residents
Active participation in family style living
A variety of responsibilities for each resident
Staff supervision 24/7
Customized support according to written individual program plan
Residential Services Include Individual functional assessments
Speech and language therapy
Personalized dietary and menu planning
Health and medical care management
Occupational therapy
Community based recreational program
Coordination with transportation providers to various day programs
Physical therapy
Orientation and mobility training
Vision impairment specialist consultation

CRITERIA FOR ADMISSIONApplicants must:
Have a diagnosis of intellectual disability
and visual impairment or blindness
Be at least eighteen (18) years of age
Possess basic receptive language
Applicants seeking Intermediate Care in nursing placement requires eligibility
determination by Regional Center.
