How to use this guide

This guide describes the services and programmes offered by every clinical department within the Aga Khan University Hospital (Nairobi). For your convenience and ease of location, the table of contents demonstrates how the guide and the departments are organised. The appropriate page for each department contains a description of the services available and areas of particular expertise.

How to make a referral• The patient, referring physician or provider should call the Patient Referrals Coordinator (PRC),

currently based at the Chief of staff office on Tel. number 0203662462/3 or by email at [email protected].

• The Patient Referral Form should be completed & sent to the PRC.• On receipt of the form, and with collaboration of the required personnel, a decision will be

made on whether Out or Inpatient treatment is recommended. If the patient has any form of medical coverage then the necessary information will be processed & advice given on any credit arrangements.

• The PRC will assist the patient with arrangements such as airport pick up, accommodation & other necessary information.

Inpatient• If no prior arrangements have been made, then the patient will have to come to the hospital

A&E.• If the PRC has been informed of the admission prior to the arrival of the patient, then ambulance

service or transport facility may be arranged to pick up the patient & accompanying attendant.• Admission formalities & process will be done and the patient admitted.• The PRC will assist with post-discharge travel & accommodation arrangements for the patient

& accompanying attendant.

The Aga Khan University Hospital (Nairobi) is the primary teaching affiliate ofthe Aga Khan University (AKU). It provides a broad range of the latest and technically advanced diagnostic and therapeutic services backed by a team of well trained professionals. To ensure patient safety, our clinical staff is fully credentialed and clinical privileges are granted with great care.

Currently, the hospital offers training for residency programmes in Internal Medicine, Surgery, Clinical Pathology, Anatomical Pathology, Anaesthesia, Radiology Paediatrics and Gynaecology.

The Department of Anaesthesia is staffed by an experiencd team of anaesthetists committed to providing high quality services to patients. the department provides 24hr anaesthesia management for all surgical procedures as well as continuing critical care services through the Intensive Care Unit (ICU). Comprehensive 24hour Obstetric and Paediatric analegisia services as well as acute and chronic Pain Management services are alsooffered.

The department also provides Postgraduate Medical Education and at present 2 students are undergoing training.

The Obstetrics and Gynaecology department of the Aga Khan University Hospital (Nairobi) is accredited to the Royal College of Obstetricians and Gynaecologists, England. The department is staffed by a dedicated team of consultants, registrars, senior house officers and midwives offering quality care to our clients.

The Maternity package provides: certainty of cost, comfortable accommodation, a well equipped nursery for newborn babies, continued antenatal care, delivery, postnatal care and well baby care. The Lamaze Programme is in place to address the needs of women and their families during the challenging child bearing years. We encourage partners to attend lessons together. The ultimate goal of this programme is to provide a positive birth experience for the expectant mother and her partner.

The department offers full range of high quality gynaecological services including both outpatient and inpatient care. The Well Woman Clinic offers an opportunity for gynaecological and general medical check ups to well women with the aim of early identification, intervention and prevention of various medical conditions.

The Department of Medicine offers comprehensive 24 hour services in adult medicine and is run by a dedicated team of specialists with medical subspecialties represented by: Rheumatology, Cardiology, Endocrinology, Pulmonology, Gastroenterology, Oncology, Internal Medicine, Nephrology, Neurology and Dermatology.

The department is also involved in post-graduate medical training . Currently, there are 5 postgraduate students and 22 faculty teaching staff. Interns, residents and registrars form part of the team offering 24 hour coverage.

The department is also involved in Quality Assurance systems such Risk Management, Mortality Reviews and Continuing Medical Education.

The Department of Paediatrics is made up of a Children’s Ward; a Newborn Unit comprising a Level II Unit for sick, low-birth weight and premature Neonates, a Neonatal Intensive Care Unit, and a New Born Nursery.

The department also provides Emergency and Ambulatory Services, a Well Baby Clinic; vaccination center; and outpatient specialist clinics.

Specialist consultants are available in the fields of Neonatology, Neurology, Nephrology, Gastroenterology, Haemato-Oncology, Cardiology, Pulmonology and Child Psychiatry.

The Department of Pathology offers comprehensive laboratory diagnostic services of the highest quality. Both routine and specialized laboratory tests are undertaken by highly trained and skilled staff in the fields of; Haematology, Chemical Pathology, Medical Microbiology, Endocrinology, Histopathology and Cytology. Diagnostic services are constantly reviewed as new tests are developed and introduced including tests relating to Molecular Pathology.

The department has introduced modern, high-end equipment inorder to increase its participation in International Quality Assurance Programmes and position itself for international accreditation. The department is also actively involved in the development of outreach centres country-wide.

The change of status to a University Hospital ushered in new impetus and a significantly expanded mission which now includes teaching and research. The first cohort of postgraduate students was admitted in January 2006 to undertake specialist training in the fields of Clinical Pathology and Anatomic Pathology. A full complement of specialist consultants representing all disciplines of Pathology are fully committed to service provision and teaching and are actively engaged in research.

The Department of Radiology and Imaging is one of the most modern facilities in East and Central Africa. It is manned by five consultant Radiologists each one an expert in a sub-specialty, radiographers and technologists. Sub-specialties include:• General radiology• Fluoroscopy • Mammography• Ultrasonography• CT scanning• MRI scanning

The equipment is state of the art and of the latest technology. This includes: the 16 slice CT Scanner which has the capacity of reconstructing scans into 3D images and CT Angiography; the MRI is a 1.5 Tesla magnet with fully loaded software and coils for a wide range of applications and ultrasound machines which have capability of 4D scanning.

The mammography and ultrasonography provide imaging backup to the popular ‘One Stop Breast Clinic’ of the hospital.

The department is part of the Aga Khan University East Africa, which offers a four year post graduate training programme leading to a Master of Medicine in Radiology (M.Med. Rad) Degree. Currently seven residents are undergoing training.

The department is committed to efficiency and quality. It has well trained ancillary staff and trained nurses to ensure optimum patient-care.

The Department of Surgery is an academic department which endeavors to excel in quality of care, education and scholarship. It has 10 different sections which include: General Surgery, Orthopeadics, Urology, Neurosurgery, Pediatric Surgery, Plastic Surgery, Cardiothoracic Surgery, ENT, Ophthalmology & Maxillofacial Surgery. We have highly qualified, trained and dedicated faculty members in every surgical specialty with a complement of 8 resident junior doctors.The department provides 24 hours high quality tertiary care services.

Besides performing all routine procedures in every surgical discipline, we excel in providing; specialised cancer surgery, minimally invasive (laparoscopic) surgery, minimally invasive cataract surgery (phacoemulcification), complex neurosurgical, orthopeadic and maxillofacial procedures. We believe in a multidisciplinary approach to patient care and have infrastructures and expertise of highly qualified & experienced Surgeons, Anesthetists, Radiologists, Pathologists and Nursing Staff to support our complex and major elective and emergency surgical workload. We have trained and certified trauma team with a 24 hour activation system to deal with any kind of major surgical trauma.

We regularly audit our outcomes and are very conscious of quality assurance in the department. We have credentialing systems; weekly morbidity and mortality meetings and we engage in continuous professional development. The clinic offers expert advice on the various available options for contraception, and helps couples choose the right contraceptive method. AKUH (N) provides comprehensive inpatient services in almost all major disciplines of medicine and surgery. There are 7 inpatients units with a total capacity of 254 beds. This includes 16 critical care beds in the ICU, HDU and NICU. The average length of inpatient stay at AKUH (N) is the lowest in the city. All inpatients are routed either through the Accident and Emergency Medicine Section or outpatient services. Other admissions are routed through the admissions desk.

Facilities offered in the nursing units are: general wards, private rooms and VIP private rooms in the Princess Zahra Pavilion. Most nursing units are able to take care of patients who need close monitoring and supervision. The major strength of the inpatient service lies in its highly dedicated team of professionals comprising medical consultants, residents, trained and competent nurses, and other paramedical staff. They provide high-quality health care through a collaborative effort together with a broad range of state-of-the-art equipment to give the best possible diagnosis and treatment options. As a teaching hospital affiliated with Aga Khan University’s Health Sciences, the clinical ward rounds are typically medical education oriented. Residents are available to patients round the clock.

Dialysis is offered inhouse for the ICU patients who require dialysis. Facilities are also available for Continuous Renal Replacement Therapy (CRRT). The NICU is located next to the Maternity unit on 3rd Floor.

The Nutrition Care Clinic provides counseling to patients regarding nutritional and other non-pharmacological interventions for the management and/or prevention of chronic diseases. The approach is holistic and involves not only patients but also their families. The goal is to guide the patient towards an optimum nutritional status and a healthier lifestyle that will benefit the patient as well as his or her family.

Diseases that particularly require nutritional counseling include Obesity, Diabetes, Dyslipidemias and Heart Disease, Chronic Renal Failure, Liver Disease, Multiple Food Allergies and intolerances, as well as under-nutrition in both children and adults. The Nutrition Care Clinic offers specialized services in Cardiac nutrition care, cancer nutrition care, Diabetic nutrition care, enteral nutritional care, Paediatric nutritional care, Prenatal nutritional care, Surgical nutritional care and weight education.

The Clinic also provides counseling to families who are interested in developing healthy eating and exercise habits in their young children, so as to maximize their growth and health potential and minimize the risk of chronic diseases in later life.

Women & Child Services

The Aga Khan University Hospital (Nairobi) Maternity Services is accredited by the Royal College of Obstetrics and Gynaecology in the United Kingdom. The service provides quality care at an affordable price offering would-be mothers a comfortable environment through each stage of their pregnancy: ante-natal, delivery and post-natal.

Qualified nurses, midwives and a team of resident doctors under the stewardship of senior Obstetrics and Gynaecology consultants cater for both mother and child’s needs.

Labour Delivery and Recovery services consolidate the process of Labour, Delivery and Recovery (LDR) in one room saving mothers the inconvenience of being moved from room to room at various stages of labour. The unit offers an opportunity for mothers to deliver in an environment where they feel nurtured, comfortable and their privacy and preferences respected. The first of its kind in East Africa, and located at the prestigious Princess Zahra Pavilion, the unit is the answer to many expressed needs of women in maternity care; privacy, comfort, and choice. This also addresses the psycho-social needs of the family.

The LDR concept encourages natural childbirth and allows the mother the option of remaining with family members and friends. The concept is enhanced by the use of a special bed, which allows the mother to deliver in any position of her choice in a state-of-the-art room. There is round the clock monitoring of the mother during the birthing process by an Obstetrician and mid-wives. A Neonatalogist and Neonatal nurses are available for the baby.

Lamaze (Child birth preparation classes) is a method of childbirth preparation based on proven scientific methods to provide a positive experience for the expectant mother. Recognized internationally, the programme is a popular method of childbirth education and preparation.

Couples are encouraged to attend the six week programme which includes exercises, relaxation and breathing techniques, labour and delivery experience, nutrition, sexuality and coping skills for the postpartum period.

The Maternity ward has a bed capacity of 25. It has state-of-the-art delivery rooms with piped oxygen, neo-natal resuscitation units, comfortable delivery couches and electronic external foetal monitoring devices. The ward has 12 beds in the general ward; 8 semi private rooms; 2 private rooms & specifically for monitoring high risk expectant mothers.

The Newborn Nursery can accommodate 25 newborn babies with facilities for critical care of premature babies. It has incubators, ventilators, cardiac monitors and neo-natal resuscitation units.

The hospital also has NICU/NHDU units. There is a 2 bed fully equipped Neonatal care unit and an 8 bed Neonatal High Dependency Unit. The units are staffed with highly qualified and experienced Neonatalogists, Paediatricians and Neonatal nurses who provide skilled care to the high risk neonates.

The Aga Khan University Hospital (Nairobi)’s Nursing and Midwifery Services are committed to providing effective and efficient care to meet the needs of patients. The Department of Nursing delivers a high standard of patient care that is intended to exceed expectations of patients and their families. Nurses are trained on a regular basis to cope with technological advances and societal complexities.

In general nursing, the hospital meets international standards with a nurse-patient ratio of 1:6. In Intensive Care and High Dependency Units, this ratio is 1:1.5. All critical patients receive individual care from nurses.

Since 2002, the hospital has developed a multidisciplinary incident reporting policy providing feedback and information on various clinical and non-clinical incidents. This policy helps to minimize incidents and has improved timely reporting, immediate investigations/actions, documentation and enhanced patient satisfaction.

Training of nurses is a regular and on-going activity. Nurses are regularly appraised on the quality of care, infection control, care of the patients environment and day-to-day nursing practices. The Hospital conducts an in-depth analysis to identify relevant, immediate and remote training requirements for its nurses.

The Hospital’s Nursing Education Service works closely with ward-based clinical nurse instructors who are responsible for developing Continuing Nursing Education (CNE) modules. Besides training, the service sponsors nurses for local and international postgraduate courses annually. Enrolled community nurses also have the opportunity to upgrade to Kenya Registered Nurses with the Aga Khan University, East Africa.

Physiotherapy is the assessment, treatment and prevention of injuries and disorders by physical and mechanical means (massage, regulated exercise, water, heat, and electricity). It enables patients to function at their optimal physical level and often complements other therapies and treatments.

The Department works with patients to prevent disability, relieve pain, develop, improve and restore motor functions. The Unit/Department has a state-of-the-art hydrotherapy pool and gymnasium especially useful for treating post surgery and neurological patients.

Physiotherapists at The Aga Khan University Hospital (Nairobi) practice the latest techniques in neuro-rehab, ante-natal/post-natal exercises and sports medicine. These services are available for both inpatients and outpatients. The department is open 24 hours a day with no extra charge for work outside normal working hours. The key features of the Pharmacy include:

This ensures uniformity on the prices of drugs and other items. The satellite pharmacies have enabled a multidisciplinary environment between the doctors, nurses and the pharmacists.

The Pharmacy Department participates in the One Stop Medical Clinic and the One Stop Diabetic Clinic among others as part of the multidisciplinary approach in patient care.
