About RPM Plus RPM Plus works in more than 20 developing and transitional countries to provide technical assistance to strengthen drug and health commodity management systems. The program offers technical guidance and assists in strategy development and program implementation both in improving the availability of health commodities—pharmaceuticals, vaccines, supplies, and basic medical equipment—of assured quality for maternal and child health, HIV/AIDS, infectious diseases, and family planning and in promoting the appropriate use of health commodities in the public and private sectors. Abstract The Global Fund to Fight Aids, Tuberculosis and Malaria convened a sub regional workshop for twelve Central and West African grant recipient countries in Yaoundé, Cameroon, with representatives of Principal Recipients (PR), MOH/National AIDS Control programs and Civil Society. The workshop aimed at supporting countries of West and Central Africa receiving Global Fund grants in finding practical operational solutions to technical problems encountered in the implementation process of programs. RPM Plus, at the request of USAID/WAfrica facilitated at the workshop; specifically the session on forecasting, management and distribution of HIV/AIDS medicines to reach universal access. RPM Plus also facilitated group work sessions on management of medicines and supported three countries (Liberia, Sierra Leone and Gambia) as they developed their action plans.

Background 40 million people are living with HIV/AIDS, 6 million of which need ARV treatment now. Current estimates show that only 440,000 people living with HIV/AIDS (PLWHA) are being treated with ARVs. The health and socio-economic impact of HIV/AIDS could be reduced if most of those in need of treatment had access to care and ARV treatment. The international community has expressed its commitment to fight HIV/AIDS, particularly in the most affected countries, through the Global Fund to fight AIDS TB and Malaria (GFATM) which has provided grants to many countries in need. The challenges most of these countries are facing is the capacity to effectively implement these programs, including managing procurement and logistics of HIV/AIDS drugs and related supplies and rational use of ARVs. The Global Fund and other partners are working together to provide the technical assistance required to increase the absorption capacity of these countries and the effective implementation of HIV/AIDS programs. Working with various partners, the Global Fund to Fight Aids, Tuberculosis and Malaria convened a sub regional workshop for twelve Central and West African recipient countries in Yaoundé, Cameroon, with representatives of Principal Recipients (PR), MOH/National AIDS Control programs and Civil Society. The workshop was aimed at supporting countries of West and Central Africa receiving Global Fund grants in finding practical operational solutions to technical problems encountered in the implementation process of programs. Specific objectives were to:

identify and analyze HIV/AIDS program implementation bottlenecks and develop solutions;

assess and address countries’ needs for technical support in accelerating Global Fund supported programs;

develop country specific action plans to address HIV/AIDS program implementation bottlenecks.

RPM Plus facilitated at the workshop, specifically the session on forecasting, management and distribution of HIV/AIDS medicines. In addition, RPM Plus facilitated group work sessions on management of medicines and supported three countries (Liberia, Sierra Leone and Gambia) as they developed their action plans.

Activities

Participate in the workshop and provide facilitation

RPM Plus participated in the three-day workshop and provided technical assistance in the area of pharmaceutical management of HIV/AIDS commodities during the plenary and small group work sessions, particularly in the areas of forecasting, quantification and rational medicine use. Bannet Ndyanabangi chaired and facilitated the plenary session on forecasting, management and distribution of HIV/AIDS pharmaceuticals. He also participated in and facilitated small group sessions on pharmaceutical management and supported the development of action plans by Liberia, Sierra Leone and Gambia country teams.

Provide an arrival/departure briefing to US- Embassy in Cameroon Jointly with the USAID team of Sosthene Bucyana and Letitia Sam, Bannet Ndyanabangi met with Mr. Daniel Goons, Special Projects coordinator at the Embassy of the United States in Cameroon, to discuss the purpose of the visit and the mechanisms for providing TA to GFATM recipient countries.

Key pharmaceutical management challenges emanating from the group discussions and country action plans include:

o Quantification and Forecasting: Inaccurate quantification of medicines with some countries reporting overstocking, leading to expiry and wastage of ARVs. Other countries reported experiencing stock outs leading to lack of access to medicines and consequent treatment interruptions.

o Selection of Medicines: Few of the countries still have to update their standard treatment protocols and essential medicines list for HIV/AIDS.

o Rational use of Medicines: The majority of countries reported lack of interventions to monitor and promote adherence to ARTs. The participants voiced major concerns of possible development of resistance to ARVs, leading countries to resort to more complex and expensive second and third line treatments and eventual treatment failures if interventions to promote adherence are not instituted.

o Pharmaceutical management systems: Participants recommended the strengthening of existing procurement, storage and distribution systems for pharmaceuticals rather than the creation of new parallel unsustainable ones.

Next Steps Immediate Follow-up Activities

• Debrief the Embassy of the United States in Yaounde, Cameroon. • Discussion with USAID/W Africa and the AWARE/HIV and RH projects interventions

to address implementation constraints and challenges identified in the workshop. Agreement or Understandings with Counterparts Some of the constraints and challenges identified at the workshop will be addressed in the two upcoming July and August 2006 training activities organized under the USAID/W Africa program. The initial course, scheduled to take place in July 2006 in Dakar, Senegal, will address general pharmaceutical and supply management issues identified at the GFTAM PSM workshop that was organized by RPM Plus in January 2006 in Accra, Ghana. At this workshop, similar constraints and challenges to the ones above were identified. This course is jointly organized by MSH/RPM Plus and JSI/DELIVER. The second course in August 2006 under the USAID AWARE/RH and HIV projects will emphasize forecasting and quantification, including the use of the Quantimed tool to support quantification. MSH/RPM Plus will facilitate at this course.
