About the Incentive

To Participate

To join the 2015 Brickworks Luxury Incentive, all you need to do is purchase products from Austral Bricks, Bristile Roofing, Austral Masonry, Austral Precast and

Auswest Timber.

All purchases made during the qualifying period (1 July 2014 – 30 April 2015) accumulate Brickworks Dollars. At the end of the qualifying period you can use

these to redeem your incentive tickets. Brickworks Dollars will be awarded as follows:

• $100 spent on First Quality Face Bricks, Pavers, Masonry and Retaining Wall Blocks and Commons = $1 Brickworks Dollar

• $100 spent on Concrete and Terracotta Roof Tiles (supply only) = $1 Brickworks Dollar

• $200 spent on Concrete and Terracotta Roof Tiles (supply and lay) = $1 Brickworks Dollar

• $250 spent on Austral Precast products = $1 Brickworks Dollar

• $200 spent on Auswest Timber products = $1 Brickworks Dollar

To Qualify

Each place on the 2015 Brickworks Luxury Incentive will be worth $12,500 Brickworks Dollars. To join Brickworks in Monte Carlo, you must

earn an additional $4,000 Brickworks Dollars.

A minimum of $6,000 Brickworks Dollars must be accumulated to qualify for the incentive. Once this minimum threshold has been

achieved, you can join the incentive by earning the remaining Brickworks Dollars or buying in the balance.

All you have to do is continue doing exactly what you are good at; buying and promoting Brickworks products. There is no limit

to the number of incentive tickets one company can accrue. The more you achieve, the more you receive.

Terms & Conditions

1. The incentive qualifying period will run from 1 July

2014 through to 30 April 2015

2. All winners will travel together to Portugal, Spain &

Monaco in 2015

3. The 2015 Luxury Incentive trip is limited to 180

places

4. Brickworks Dollars will only be rewarded for first

quality product

5. “Purchases” refers to product that has been

delivered. All orders must be delivered by close of

business on the final day of the qualifying period for

the points to be added towards your incentive total.

6. A minimum of $6,000 Brickworks Dollars are

required to qualify for the 2015 Luxury Incentive.

This entitles participants to pay the outstanding

balance of one ticket.

7. One incentive ticket is worth $12,500 Brickworks

Dollars.

8. One Monte Carlo ticket is worth an additional

$4,000 Brickworks Dollars.

9. The trip is based on twin share accommodation. A single supplement fee will apply.

10. Brickworks Dollars are only to be used for the 2015

Luxury Incentive and are not redeemable for cash.

11. Trip participants must be employees or partners

(wife , husband etc.) of those involved.

12. Final total of Brickworks Dollars required are subject

to changes in currency fluctuation, airport and

government taxes and additional fuel surcharges.

Chilli CrabArguably one of Singapore’s greatest culinary inventions, this famous dish is a must have during any visit to Singapore. It is easily found in many

hawker centres and restaurants throughout the city.

What makes this dish so special is its sensuous, sweet

yet savoury sauce, created with a base of chilli and

tomato sauces which will electrify your palate and

satisfy your taste buds.

The red-tangy sauce is made deliciously fragrant

with light flavourings of garlic and rice vinegar,

while thickening flour and egg ribbons (produced

by adding beaten eggs towards the end of the

cooking process) give the chilli crab dish the fluffy

Singapore Sling was created by Ngiam Tong Boon at

the Long Bar of the Raffles Hotel in Singapore, some

time between 1900 and 1915.

The original Singapore Sling is a gin-based drink that

has a nice balance between the sweet pineapple

juice, the spice of Dom Benedictine and a dash of

Angostura bitters to finish.

30ml Gin

15ml Cherry liquor

120ml Pineapple juice

15ml Lime juice

7.5ml Cointreau

7.5ml Dom Benedictine

10ml Grenadine

Dash of Angostura Bitters

Mix ingredients well in a cocktail shaker and pour in

a tall glass full of ice cubes. Garnish with a slice of

pineapple and a cherry.

Welcome to Melbourne, the world’s most liveable city, where we invite you to come and explore what Melbourne has to offer, and more importantly, for you to discover and share the hidden gems within your professional lives.

Medicines Management 2015 explores the patient and practitioner journeys through an exciting and diverse program for all pharmacists, pharmacy technicians and associates practising in the healthcare system.

We are delighted to bring you plenary sessions from international and local leaders in our profession. Dr Ross Tsuyuki will question whether pharmacists themselves may be the ultimate barrier to advancing pharmacy practice, and will also share pearls of wisdom from the Canadian experience with pharmacist prescribing. Professor Tina Brock and Dr Neil Cottrell will explore the jewels to be found in interprofessional learning, giving us clarity on what is precious and semi-precious. Additionally, the Australian Institute for Patient and Family Centred Care will help us delve into the complex issues that arise when things go wrong in healthcare through the powerful and innovative play, “Hear Me”. In the final plenary, we will examine the State of the Nation, revisiting 2010 and discovering what 2020 may bring.

In addition to our plenary speakers, we have an exciting range of invited speakers from across the country, covering topics from pharmacists and technicians in diverse areas of clinical practice, through to ethical dilemmas in the context of rising costs and increased demand. The program also features a variety of workshops and conversations to develop your skills, from easy English training to mastering the art of being remarkable.

This year a record number of abstracts were submitted and with the extended conference program, more of these high quality abstracts can be showcased. We encourage you to support our delegate presenters and engage with them in exploring the ideas and initiatives presented. The Scientific Program Committee are particularly excited to be introducing lightning talks into the program, enabling many pieces of work to be showcased in a short space of time.

MM2015 has been accredited for both Group 1 and Group 2 CPD, so make sure you update your CPD record. Our program includes a range of opportunities to suit your learning style, all designed to allow you to convert your conference experience into practice change, which attracts Group 3 credits.

DR STEVE HAMBLETONChairman, National E-Health Transition Authority (NEHTA), NSW

Dr Steve Hambleton is a former Federal Vice President and President of the Australian Medical Association (AMA) 2009-2014. He was the State Manager of the corporate medical centre provider Foundation Healthcare in Queensland (now IPN) in 2001. He was President of AMA Queensland in 2005-2006 and served on the AMA Council of General Practice at a State and Federal level for more than 15 years. Dr Hambleton

was the AMA representative on the National Immunisation Committee from 2006-2010, and was a member of the Pharmaceutical Benefits Advisory Committee 2007-2009. He was a member of the AMA Taskforce on Indigenous Health in 2005 and was Chair from 2009 to 2014. Dr Hambleton is on the Board of the Australasian Medical Publishing Company. He is a Member of the Australian Commission on Safety and Quality in Health Care’s Australian Atlas of Healthcare Variation Advisory Group. Dr Hambleton was appointed Chairman of the National eHealth Transition Authority in June 2014 and for his services to general practice was awarded an honorary Fellowship of the RACGP in September 2014. Dr Hambleton joined the Board of Avant Mutual Group Limited in November 2014. In April 2015 he appointed the Chair of the Primary Health Care Advisory Group in to investigate options to provide better care for people with complex and chronic illness; innovative care and funding models; better recognition and treatment of mental health conditions; and greater connection between primary health care and hospital care. He is also a member of the Medicare Benefits Schedule Review Taskforce and a member of the eHealth Implementation taskforce Steering Committee. Dr Hambleton was awarded the AMA’s Gold Medal in May of 2015 for services to his profession.

ANDREW HARDINGSenior Pharmacist, Department of Pharmacy, Department of Emergency Medicine, Austin Health, Vic

Andrew Harding graduated from the Victorian College of Pharmacy in 1993. He has worked at various hospitals in various roles during his junior years in Melbourne and abroad. This included spending six months in India working as a pharmacist for the charitable organisation Calcutta Rescue. Returning to Australia, he worked at Monash Medical Centre

mainly in the Acute Medical Unit for the Aged and the Emergency Department – his first taste of an Emergency Department. Leaving Australia again, Andrew and his wife worked for Australian Volunteers International in the Marshall Islands. Andrew was involved in developing the pharmacy service in this small Pacific nation. Once again returning to Australia, Andrew was in a pharmacy management role at Eastern Health. Leaving that role, Andrew accepted a position working fulltime in an Emergency Department at Austin Health where he continues to work today.

RUTH HAYDirector of Medication Services, Queensland Health, Qld

Ruth Hay registered as a pharmacist in 1983 and worked in both community and private and public hospitals in the UK for a number of years before migrating to Australia, where she continued her career at one of Queensland’s largest public hospitals.

For the last 10 years, Ruth has worked within the Queensland Department of Health, initially in a support role to the Minister’s Advisor in Pharmacy. For the last five years, Ruth has

been the Director of Medication Services and the most senior pharmacist within the Department of Health.

The diversity of her responsibilities and a constantly changing “priority list” to meet the demands of her role keep her on her toes both in and out of the office, with her hobby of running providing an excellent way to organise her thoughts for the day ahead.

LORAH HICKMANSenior Pharmacy Technician, Education and Development, Alfred Health, Vic

Lorah Hickman is a UK trained Pharmacy Technician with 8 years post qualification experience. Lorah has worked in Australian hospital pharmacy (at the Alfred) since August 2013. Lorah has been Technician Observer on the SHPA Vic branch since August 2013, and has been the Chair of the SHPA National Pharmacy Technician Network since January 2015.

KIRSTIE GALBRAITHDirector, Postgraduate Studies and Professional Development Unit, Faculty of Pharmacy and Pharmaceutical Sciences, Monash University, Vic

Kirstie Galbraith is the Director of the Postgraduate Studies and Professional Development Unit at the Faculty of Pharmacy and Pharmaceutical Sciences, Monash University. In her education-focused role, she works closely with the profession

on practitioner development initiatives. Kirstie is also the coordinator of activities for the National Alliance of Pharmacy Education (NAPE), a consortium involving Monash University, the University of Queensland, University of South Australia, and the University of Sydney. NAPE was formed to actively support the ongoing advancement of the pharmacy profession as a key contributor to the healthcare team. Kirstie has worked closely with SHPA on a number of initiatives including the development and implementation of the Clinical Competency Assessment Tool (clinCAT) into Australian pharmacy practice. She is the International Pharmaceutical Federation Education Development Team (FIPEd) Domain Lead for Competency (Advanced Practice) and is a member of the Advanced Pharmacy Practice Credentialing Committee which is implementing an advanced practice credentialing pathway in Australia.

Kirstie’s contribution to education and advanced practice in Australia was recently recognised with the awarding of the 2014 SHPA Medal of Merit.

DR ROCHELLE GELLATLYPharmacy Department, Alfred Hospital, Vic

Dr Rochelle Gellatly is the Clinical Lead Senior Pharmacist in Cardiology at the Alfred Hospital overseeing the clinical pharmacy services for general cardiology, cardiothoracic surgery, advanced heart failure, mechanical circulatory support and heart transplantation. Rochelle primarily works with the Advanced Heart Failure/Transplant team and also holds a clinical cross-appointment in the Masters of Clinical Pharmacy program at Monash

University. She obtained her Bachelor of Science in Pharmaceutical Sciences at the University of British Columbia in Vancouver, Canada and subsequently completed a pharmacy practice residency and Doctor of Pharmacy in 2008. Most recently, she was awarded the William Mercer Young Achiever Award and successfully became a Board Certified Pharmacotherapy Specialist with Added Qualifications in Cardiology. Dr Gellatly’s research and professional interests include ventricular assist devices, immunosuppression and cardiovascular drug therapy.

Sharon Goldsworthy is the Associate Director of Pharmacy, Central Adelaide Local Health Network for The Queen Elizabeth & Glenside Hospitals and Hampstead Centre. She is the SA Pharmacy workforce portfolio lead and is progressing to become a Calderdale Framework Facilitator. She undertook a Pharmacy degree at the South Australian Institute

of Technology and Masters Degree at the University of South Australia. She is a Fellow of The Society of Hospital Pharmacists of Australia.

GEOFFREY GRIMAPharmacist – Advanced (Oncology), Team Leader Oncology and Surgery, The Prince Charles Hospital, Qld

Geoffrey Grima has worked as a Clinical Pharmacist in various clinical areas and is currently the Pharmacy Team Leader for Cancer Services and Surgery at The Prince Charles Hospital in Brisbane.

Geoff is involved with many national and local professional working parties related to Cancer Services and is currently the Chair of the Metro North Cancer Care Clinical Advisory Group. Geoff is an Associate Lecturer for Undergraduate Pharmacy Students at the University of Queensland and is the Director of his own consulting business PharmAdvice Consulting Services. Geoff is a reviewer of various drug information books such as Australian Medicines Handbook and Australian Injectable Drugs Handbook and is currently involved in the Advanced Pharmacy Practice Pilot through the Australian Pharmacy Council. His major professional interests include lung cancer, targeted therapies in treating malignancies and thromboembolic disorders associated with malignancy.

VAUGHN EATONDirector, Flinders Medical Centre and Noarlunga Health Services, SA Pharmacy, SA Health, SA

Vaughn Eaton is the Director, SA Pharmacy for the Southern Adelaide Local Health Network (SALHN) which incorporates Flinders Medical Centre, Noarlunga Hospital and the Repatriation General Hospital. Graduating from the University of South Australia and completing his training at the Royal Adelaide Hospital, Vaughn later managed the expanding Clinical Services at Flinders Medical Centre where he completed his Masters and Fellowship,

while working in renal medicine and infectious diseases. In 2001 he was appointed as Director of Pharmacy Services at FMC, and more recently has taken responsibility as Director, SA Pharmacy SALHN.

Vaughn’s interests include the development of improved services throughout the medicines management pathway to ensure high standards of patient care by the adoption of new methodologies and advances in technology to ensure the best outcomes for our patients. He has been integral to strategic planning and service delivery that has led to significant beneficial changes for the profession. Dr Margie Danchin is a University of Melbourne graduate who currently works as a general paediatrician at the Royal Children’s Hospital in the Department of Paediatrics and as a senior research fellow at The Murdoch Children’s Research Institute. She obtained her PhD in 2006 after completing a fellowship at The Hospital for Sick Children, in Toronto. As

an NHMRC post-doc, she completed advanced training in vaccinology through the NHMRC Centre for Clinical Research Excellence (CCRE) in Child and Adolescent Immunisation and the Rotavirus Vaccine Research group, the Immunisation Adverse Events Surveillance Group (SAEFVIC), and through the Vaccine and Immunisation Research Group (VIRGO), at MCRI.

Margie has extensive expertise in vaccine research focused on vaccine clinical trials, vaccine safety and new strategies to enhance the performance of existing vaccines in different settings, particularly rotavirus vaccines. Margie also does international child health research focussed on the prevention of diarrhoea and pneumonia in developing countries. Recent research has focused on understanding parental attitudes, behaviours and concerns regarding vaccines and the development of new, innovative training methods to improve the knowledge and communication methods of health care providers to reduce parental vaccine hesitancy.

Dr Catherine Duggan is the Director of Professional Development and Support at the Royal Pharmaceutical Society of Great Britain. has a leadership role in developing programmes of professional development, advancement and recognition. Prior to this, John worked as Deputy Director of Pharmacy at North East Health, Wangarratta, Victoria.

Amy Chan is one of the clinical pharmacist team leaders at the Auckland District Health Board inpatient pharmacy department. She is interested in the role of technology in supporting medicines management and is currently completing her PhD in patient adherence and the role of electronic adherence monitoring. She has presented nationally

and internationally on her research and published several papers in this area, including a recent publication in The Lancet Respiratory Journal. She was awarded the Value of Medicines Award 2015 for her work in adherence and asthma. She is the Deputy Convenor of the Mental Health Special Interest Group for the NZ Hospital Pharmacists’ Association, and is on the team of writers for The Scientista Foundation, founded in Harvard.

BRONWYN CLARKChief Executive Officer, Australian Pharmacy Council, ACT

Bronwyn Clark is the Chief Executive Officer of the Australian Pharmacy Council, the Accreditation Council for pharmacy education in Australia. Prior to this she was the Chief Executive and Registrar of the Pharmacy Council of New Zealand. She has had a varied career in pharmacy that spans practice in hospital, community, GP facilitation, education and regulation in New Zealand, Australia and the UK.

Bronwyn has a keen interest in the development of new and expanded scopes of practice for pharmacists and was instrumental in implementing the pharmacist prescribers in New Zealand. In 2012, she was awarded a Fellowship of the Pharmaceutical Society of New Zealand for a significant and outstanding contribution to the practice of pharmacy. Bronwyn is a member of the American ACPE International Services Program Advisory Group (ISPAG) and an international committee member of the Life Long Learning in Pharmacy conferences.

Tara Clayson-Fisher began her career in community pharmacy, mainly in dispensary and nursing home medication management. After six years Tara moved into hospital pharmacy in 2011 and she hasn’t looked back since.

Tara is currently employed at Flinders Medical Centre as a Clinical Pharmacy Technician for the Respiratory and Cardiac units. This position is thoroughly enjoyable and has provided her

with many learning opportunities. Outside of work Tara enjoys playing field hockey, brunching and searching local markets for unusual home additions.

Associate Professor Neale Cohen is currently the General Manager of Diabetes Services at the Baker IDI Heart and Diabetes Institute in Melbourne. He is an endocrinologist in clinical practice with over 20 years, having worked for the International Diabetes Institute and now the Baker IDI and has extensive experience in diabetes services in both private and public systems. He is an Adjunct Associate Professor at RMIT University. In his current position,

Dr Cohen is responsible for the management of one of the largest diabetes services in Australia including two Melbourne based clinics, and an indigenous diabetes service based in the Northern Territory.

He has long standing interest and involvement in clinical research and has numerous publications relating to the management of diabetes and its complications. His areas of interest and publication include insulin pump therapy, indigenous diabetes, experimental diabetic nephropathy and incretin therapy.

The Society of Hospital Pharmacists of Australia (SHPA) is the national professional organisation for more than 3,400 pharmacists, pharmacists in training, pharmacy technicians and associates working across Australia’s health system. SHPA is the only professional pharmacy organisation with a strong base of members practising in public and private hospitals and other health services.

SHPA is committed to facilitating the safe and effective use of medicines, which is the core business of pharmacists, especially in hospitals. SHPA supports pharmacists to meet medication and related service needs, so that both optimal health outcomes and economic objectives are achieved for individual Australians, for the community as a whole and for healthcare facilities.

SHPA was established in 1941 following the pioneering efforts of 25 public hospital pharmacists from Victoria. From 1947 to 1964 other branches were developed. The inaugural meeting of the national council and first national conference were held in Adelaide in 1961.

SHPA is governed by a Federal Council which is supported by branches, committees of specialty practice and other reference and advisory groups. All councillors and branch committee members are volunteers and are elected by the membership.

TRANSFERS

If you have booked your ights with Axis Events to arrive in Dubai on Wednesday 9 October and depart on Monday 14 October, a group transfer will be provided by Daikin between Madinat Jumeirah, Al Qsar and Dubai Airport. Details will be con rmed in your personal itinerary.

Guests travelling on alternate dates will be responsible for making their own way to and from the hotel.

Every international air traveller entering or leaving Australia carrying creams, liquids, gels and aerosols in their hand luggage will be limited to containers of 100ml or less. The containers must then be carried together in one resealable, transparent plastic bag (included in your travel

wallet), where the four sides of the bag’s sealed area must add up to no more than 80 centimeters. The bags will be screened separately from carry on bags.

AIRLINE SECURITY

Security is a serious issue in every airport including airports in the UAE, so please take care to avoid jokes regarding carrying prohibited items during airport security checks. Any comments will be taken seriously and the consequences could be dire.

When travelling on international ights it is recommended that you

check in at least three hours prior to departure. Security within the UAE,

• Upon check in you will be required to supply credit card details to cover incidental costs that are not included in the program eg. Telephone, mini bar, room service etc. Please note the hotel will place a hold of 500 AED ($150) per night on your credit card during your stay. This will be refunded after check out.

• A full buffet breakfast will be served daily from 6:30am in Arboretum located on the ground oor.

• Guests travelling on group dates arriving into Dubai on Wednesday 9 October will be able to check in to their rooms on arrival at the hotel. For guests arriving on earlier dates, check in is 4:00pm.

• Check out time is 12:00pm. Late checkout can be organised through the hotel at your own expense. If you have a late departure the hotel will store your bags until you depart.

The Dubai Metro is an easy way for visitors to get around the city and to see large parts of it from the Metro’s elevated tracks.
