The Non-Emergency Medical Transportation Resource Guide is provided to you as a free service of the Rural Medical Transportation Network (RMTN), a project of the Center for Rural Health & Social Service Development at Southern Illinois University-Carbondale. Developed in cooperation with the mass transit districts of the RMTN service region, the guide is designed to facilitate the transportation of your patients whom reside in southern Illinois.

The RMTN Service Region map delineates the four mass transit districts that are currently participating in the RMTN. The Transit District by City/County table details each municipality that is located within the RMTN service region, and its respective county and transit district. After using the map and table to determine which mass transit district provides service for the counties to/from which your patient needs transport, please refer to the contact and services information for the appropriate district.

This guide is designed to help you in your efforts to provide quality health care to your patients. Your comments and ideas of how we can improve the Non-Emergency Medical Transportation Resource Guide are important. The staff of the RMTN welcomes your participation in the development of the guide, as well as your thoughts and experiences with regard to any issues that are relevant to the emergency or non-emergency transportation of patients.

Due to limited resources, we ask that you schedule your reservation at least twenty-four hours in advance. We understand that unforeseen circumstances arise and we will make

every effort to serve same day requests when possible.

• You will need to contact the mass transit dispatch office to schedule a ride.• Customers are scheduled on a first come, first serve basis, which means that there may be wait time involved.• Passengers may also be required to ride with other passengers • Adults must accompany minor children. This limitation for minors is fact dependent and special arrangements must be handled in advance.• If you require special arrangements due to age, gender, or other factors, please discuss this when scheduling your ride.• Escorts provided by the passenger or facility, ride at no additional charge unless a return trip is required. • Payment for all transportation must be arranged in advance. • All Mass Transit Districts are Medicaid carriers. You will need to obtain prior approval through First Transit and have the approval number available when making reservations. • Other forms of payment such as credit cards (MC and Visa) direct billing or private insurance are accepted if approved.

The following information is needed when scheduling transportation:

o Form of payment• Public Aid/Medicaid, will need client’s recipient and/or prior approval number if eligible.• Insurance or credit card information if applicable.• Mass Transit Districts require PAYMENT GUARANTEE form to be completed (form included on page 3).
