Speaker, Diagnostic Imaging Regional Rounds, April 9th, 2020

Associate Clinical Professor (Adjunct), Department of Radiology, McMaster University

The first Diagnostic Imaging Regional Professor Rounds that took place on April 9th, 2020, was a BIG success. Thank you to over 100 attendees, mostly from McMaster Radiology, including participants from Niagara Health Systems. There was also good representation from Respirology/Internal Medicine, with 14 attendants in total, including participants from Infectious Diseases, Hematology, Geriatrics, Gastrointestinal and Ophthalmology.

We give special thanks to the speaker, Dr. Ali Sabri, a radiologist from Niagara Health Systems (NHS). Dr. Sabri’s excellent presentation on the Covid-19 situation included the following objectives;

1. Covid-19: How did it start? Why a pandemic?

2. Initial Challenges: Real world experience.

3. The current situation in Canada and the world.

4. Covid-19 Literature: What have we learned?

5. The role of Radiologists: What do we need to know?

This was all possible due to the help of a great team of Radiology Residents (Sam, Elton and Zach), McMaster University Radiology Staff, Dr. Walker, Dr. Tan and Dr. Haider and also administrative staff, Marilynn Scott, Sue Gaudet, Lori Power and Tori Howes-Jones. Thanks for the guidance and support from Dr. Julian Dobranowski, who’s idea could be materialized in these rounds.

The feedback received was excellent and among it were comments such as: “excellent review on pertinent issues”, “timely and nicely organized talk”, “very interesting to learn of the Iranian experience with COVID-19”, “interesting as a technologist to see what COVID-19 looks like on the images”, and that this presentation needs “to be repeat-ed”. The help and feedback from the radiology community is very appreciated as the intention is to continue work-ing with the local and regional staff, as a contribution from the Radiology Department to healthcare education in our area of expertise.

ANNOUNCEMENTS Leslie Gauthier - Vice President of Clinical

Support Services & Surgery

On Tuesday, April 14th, it was officially announced through a memo from Dr. Wes

Stephen, Executive Vice President, Clinical Operations and Chief Operating Officer,

Leslie Gauthier is now the Permanent Vice President of Clinical Support Services &

Surgery.

“I am pleased to announce the permanent appointment of Leslie Gauthier as the Vice President of Clinical Support

Services & Surgery at Hamilton Health Sciences (HHS), effective April 1st, 2020.

Leslie’s appointment follows an extensive, nationwide recruitment process. Leslie’s leadership experience, her pas-

sion for patient care, her deep understanding of the clinical world, and her track record of successfully steering

transformational change initiatives at HHS were key to her being the successful candidate to lead the Clinical Sup-

port Services & Surgery portfolio.

Leslie has had a long and distinguished career at HHS in both clinical and leadership roles. Since November, Leslie

has served as Interim Vice President of Clinical Support Services & Surgery. Most recently, Leslie has been a key

partner in the establishment of the Management Equipment Services (MES) agreement, which involves the wide-

spread renewal and maintenance of HHS’ diagnostic imaging equipment.

A graduate of McMaster University, Leslie holds a Bachelor of Science Degree in Nursing and a Master of Science in

Teaching. Part of the Faculty of Health Sciences at McMaster University School of Nursing, she has been a clinical

lecturer and is, at present, an Assistant Clinical Professor. Leslie is a Board Member of Mark Preece Family House,

and has been involved in mentoring activities through the Registered Nurses Association of Ontario.”

Congratulations to Leslie Gauthier on her permanent appointment as Vice President of Clinical Support

Services and Surgery!

Jean Matone - Temporary DI Manager Assignment

Please welcome back Jean Matone. Sarah Jane Adams, Director of DI & MDU, recently announced that Jean

Matone, DI Manager has returned as of Tuesday, April 14th. With COVID-19 planning and policy changes well

underway Diagnostic Services will not be changing the management structure at this time and as such, Jean will

be on a temporary assignment for the remainder of the Covid-19 pandemic.

For this time, Jeanine Risk will continue to manage JH DI on an interim basis. Jean Matone will be working tem-

porarily in a dual role as DI WLMH Manager as well as WLMH Manager. In the WLMH Manager role, Jean will

be assisting Cindy MacDonald in Covid-19 planning for the site and CT implementation planning.

Please join me in welcoming Jean Matone on her return to HHS Diagnostic Services.

ANNOUNCEMENTS Susan Kinnear - Manager, Diagnostic Services MUMC/UCC

On behalf of Sarah Jane Adams, Director of DI and MDU, it is with mixed emotions that

Sue Kinnear has chosen to move into the next chapter of her career, and has given her

notice of resignation.

Sue joined HHS in 2002 as a Sonographer, and later moved to a Senior Technologist

position until she assumed the Manager, MUMC role in 2015. In addition to her

Management position, Sue also sits on the Board of Directors, Sonography Canada.

During her time in Diagnostic Services, Sue has lead a multi-disciplinary team, serving

patients across our sites. Sue will be missed for her extensive expertise in ultrasound,

her comradery and her sunny disposition.

Sue’s last day with us will be Friday, May 1, 2020. The permanent replacement

process is underway, and the position is currently posted on the HHS website. In the

interim, Eric Ricker, who is well known to the MUMC DI team will assume managerial

duties, under the oversight of Cheryl Malcolmson.

Please join Sarah Jane and all of us in congratulating Sue and wishing her all the best in

her future endeavours! She will be missed.

Patti Forrest a.k.a. “Speedy” - JH Senior CT Technologist

On behalf of Jean Matone, Temporary Manager, DI WLMH & WLMH, it was announced that Patti Forrest, Senior CT Tech, and otherwise known as “Speedy”, will be retiring this July after 32 years of dedicated service. Patti started her career with HHS in DI on March 29, 1988 as a permanent afternoon technologist working closely with the emergency department. She has a long-standing career in CT and general radiography. Patti devel-oped the nickname “fast Patti" as she is such a fast worker and the department is hard pressed to find anyone that can keep up to her especially in general x-ray. Her colleagues know that if they really need to switch a shift she can be easily persuaded with a bottle of red wine. Her work ethic is strong consistently providing high quality imaging for the radiology team in general x-ray and CT.

Patti stepped into the senior position adeptly filling the senior role in August temporari-ly and then permanently in January 2020. She provided a calm, steady, collaborative and supportive leadership to her team during this brief reign. Patti is friend to many, a good listener and always willing to help. Please join Jean and all of us in wishing Patti success and happiness in the next phase of her life.

Patti’s last day will be July 17, 2020. Congratulations and Happy Retirement!

Universal Masking Update - What’s New? HHS is mandating that staff and physicians wear a surgical mask as soon as they enter any HHS site to help address concerns related to physical distancing and help stop the spread of COVID-19. It’s important to note that the need to keep at least two metres from any other person still applies even when wearing a mask.

Starting Friday (April 24th) at all sites:

Staff and physicians will get one surgical mask at screening at current designated staff entrances to our sites.

Everyone will don their mask when they enter their site.

Anyone working in an area where a Level 2 mask, or other mask, is clinically indicated will then change their mask on the unit based on a Point of Care Risk assessment.

Anyone working in an area where another level of mask is clinically indicated should store their Level 1 mask for reuse when their shift ends and wear that mask until they leave the site. how in this video

Extended Use of Masks REMINDER Everyone will not be universal masking and should practice extended use, and in appropriate areas, reuse of the mask through their daily workflow, as instructed.

Once they don any mask, everyone should continue to wear the mask for the duration of their shift, regardless of where they go (unless they are required to don additional protection based on a Point of Care Risk Assessment)

Staff should reuse their mask following breaks by using a plastic container. See how in this video

NOTE: if they don’t already, staff should bring a plastic container to store their mask during the day when they don’t need to wear it (because they’re alone in an office or protected by a plexiglass barrier.

HHS Resources:

The Resilience Support Toolkit is meant to help us all strengthen our resilience by working resilience-building practices into our daily routines and workflow, using the framework of PAUSE, RESET, NOURISH or PRN. These practices are based on solid neuroscience and the idea of “neuroplasticity”, that our brains are constantly changing based on how we think and what we choose to focus on. These tools help to calm our nervous system and enhance our focus and well-being.

Of course our EAP provider remains an option and has provided some practical support tools for staff. See the COVID-19 Toolkit.

5 Ways to Handle Change

One thing is certain these days...and that is change. You might be working shifts at a different hospital site with colleagues you do not know. You might have children who are now home from school and relying on you to help them with school work. You might be finding it difficult to keep up with changing work procedures. Change affects us all differently at different times. While change can cause some level of stress or anxiety, this stress and anxiety does not have to take over your thoughts. You can handle change!

Know What You’re Dealing With

The first step in dealing with unexpected change is to identify what is really going on. Too often, we feel stressed or generally anxious, without zeroing on the real problem.

Take Control If You Can and Accept What You Can’t Control

Identify something that you can take control of during this time of change. Having a feeling of control in an un-known situation may help change your perspective and lessen your fear, stress, or anxiety. On the other hand, when change is out of your control, don’t waste your energy fighting against it. Find a way to accept it, even if it’s not what you want, and adapt to the new reality.

Take Care of Yourself

In our new current reality, we are dealing with many changes at work and home, and managine this change can be difficult and draining. Be compassionate with yourself and practice self-care, even if all you can manage are small acts of self-care.

Get Support

You’re probably not the only one affected by this change, so why go through it alone? Reach out to others in a similar position and get their support, reach out to friends and loved ones, and if needed seek out professional support. The Employee Assistance Program (https://hub.hhsc.ca/yourhhs/hsw/eap) is available to you as well as the recently activated Resiliency Team (https://www.hamiltonhealthsciences.ca/covid19/staff-physician/resilience-ethics/resilience-support-toolkit/)

Find the Positive

We might view change through the lens of fear, but often there is a positive aspect even to difficult situations. Perhaps, you have more time at home with family. Perhaps you are working frontline helping people. Look for something good in each day or write down three things that you are grateful for each day.

ONTARIO MEDICAL ASSOCIATION RESIDENT ACHIEVEMENT AWARD

The Ontario Medical Association (OMA) presents a number of Awards each year in-

cluding the Resident Achievement Award (RAA). The OMA RAA recognizes outstand-

ing contributions to the advancement of medical education by a postgraduate train-

ee. There are up to six awards presented, one from each Ontario medical school.

Judging criteria includes leadership, teaching, advocacy, resident as professional, sus-

tainability and degree of impact.

DR. ROBERT CHU AWARD - SUPPORTS GRADUATING STUDENTS

In 2017, a generous donor provided funding to support an award in memory of the late Dr. Robert Chu. The

award supports graduating students from the School of Medicine who are pursuing a residency program in

RESEARCH GRANT—JURAVINSKI RESEARCH INSTITUTE

Congratulations to Dr. Michael Noseworthy on securing a $180,000 research grant

from the Juravinski Research Institute. Dr. Noseworthy and his team will be assessing

and predicting recovery from mild traumatic brain injury using specialized MRI brain

methods and looking at genetic and clinical factors.

Zonia Ghumman

The first citation below was a study that Zonia Ghumman was involved in with Dr. Michael Patlas (among others). The aim of this study was to evaluate the overall diagnostic accuracy of preoperative multidetector computed tomography (MDCT) in penetrating abdominal and pelvic injuries (PAPI). It found that the detection of clinically significant injuries to solid organs in trauma patients with PAPI on 64-MDCT is adequate. However, detection of injury to the remaining organ groups on MDCT, especially bowel, mesentery, and diaphragm, remains a challenge.

The second citation, was a study that Zonia was involved in with Dr. Katherine Zu-kotynski and others. The aim of this study was a comprehensive review of imaging modalities used in prostate cancer, with a focus on the utility of nuclear medicine imaging.

RESEARCH CORNER

MIIRC@M brings together radiologists and engineers in order to bridge the gap between clinical studies and computer

sciences and to solve specific issues in Medical Imaging; a cross-over program between the Faculty of Health Sciences and

Engineering, the centre has been funded by both Faculties and its goal is to provide a nurturing environment to the

researchers interested from McMaster or other institutions, match the adequate skills and resources, and facilitate access to

grants and industry partnerships.

COVID-NET Project

Hamilton Health Sciences (HHS), St. Joseph’s Healthcare (SJHH) and Niagara Health (NH) are teaming up with re-searchers at the University of Waterloo to contribute to the fight against COVID-19. Led by Dr. Alex Wong, the team is developing COVID-Net , an open source deep neural network for improved detection of COVID-19 from standard chest X-rays.

By creating COVID-Net, and making it available to the world in an open source, open access manner, the hope is that it will be used and built upon by clinicians, researchers, and citizen data scientists alike to accelerate the global use and development of effective radiography-based COVID-19 screening solutions and accelerate treat-ment of those who need it the most.

While initial results are quite promising, COVID-Net is still in its early stages and so the team needs many more chest x-rays for confirmed COVID-19 cases across as many geographic regions as possible (for both volume and diversity of data). In addition to access to images, the researchers are also engaging clinical experts who are cru-cial in helping to build the final AI system that can be deployed and used for detection and risk stratification.

The main objectives of the proposed study are as follows:

1. Investigate different deep neural network architectures for COVID-19 detection. The architecture needs to strike a balance between accuracy and speed, as well as be able to capture the nuances that differentiates COVID-19 infections from other forms of infections, to improve specificity and help clinicians make more ac-curate diagnostic judgements.

2. Investigate different deep neural network architectures for COVID-19 risk stratification. More specifically, de-sign neural networks that can predict risk level and survival likelihood to aid clinicians with triaging and priori-tization, and managers with bed planning and length of hospitalization.
