Executive Summary
In accordance with the Patient Protection and Accountable Care Act, each hospital with a
501(c)(3) status must conduct a community health needs assessment and publicly report the
results. Surveys were distributed to a wide variety of community members through the use of
Pulaski Memorial Hospital’s website, church groups, and other civic organizations. 118 surveys
were returned. As a primary provider of health care related services in Pulaski County, the
hospital understands that it plays a key role in addressing the health needs of the county’s
residents.
Introduction
Pulaski Memorial Hospital undertook a community health needs assessment in January,
February, and March 2016. The assessment was designed to identify the most important health
issues in the county using comparative information. The assessment also identifies priority
health issues where better integration of public health and healthcare can improve access and
quality to residents of Pulaski County and surrounding areas. This document will be used as a
planning tool to assist in initiating strategic initiatives regarding medical service to meet the
critical health needs of citizens whose health is at-risk in our community. This project represents
Pulaski Memorial Hospital’s efforts to share information that can lead to improved health status
and quality of care available to Pulaski County and surrounding residents, while building upon
and strengthening the hospital’s existing infrastructure of services and providers.
The 2016 Pulaski Memorial Hospital Community Health Needs Assessment has the following
objectives:
Develop a comprehensive profile of health status, quality of care, and care management
indicators for residents of Pulaski County and surrounding communities.
Determine the health status of the community.
Identify specific health needs of the community.
Analyze the gaps between existing healthcare coverage and healthcare needs in the
community.
Methodology
The consumer survey portion of this project was designed based on the format of previous
successful needs assessments obtained from other facilities (PMH). The anonymous
questionnaire contained 46 questions. All respondents included in this assessment were
voluntary participants over the age of 18. Descriptive and inferential statistics were derived
through analysis utilizing Microsoft Excel. Qualitative data was summarized, categorized, and
rank ordered when possible.
Pulaski Memorial Hospital recognized that there are many factors that affect community health.
Community health is a positive concept that is influenced by social, physician and personal

resources and capabilities. Health depends not only on formal medical care but also on other
factors including individual behavior, genetic makeup, and social and economic conditions for
individuals and communities. This process included a multidimensional approach to examining
the broad range of factors that influence health including:

Findings and Recommendations
Community health needs assessments are designed, in part, to identify issues where changes in
the healthcare delivery system can improve both patient care and preventive services for those at
risk for health problems. This section presents an overview of the findings in the report and
recommendations based on those findings.
Similar to the rest of the nation, Pulaski County residents are faced with significant health risks
caused by heart disease, diabetes, hypertension, asthma, and depression. Access to quality
preventative care and disease management education is critical in minimizing future
hospitalizations as well as the seriousness of these diseases. Health insurance coverage is also a
factor in determining whether patients will seek preventative care or wait for symptoms to
become more severe, thus requiring more extensive treatments, including hospitalization.

Needs pre-school screenings, help for services, assisted living services
Cost of some tests, MRIs, MRAs were way over the market rates. Blood tests (routine) is
also way out of market rates resulting in 30-mile drive to have it done.
PMH is a great facility and offers great services for the community. Keep up the good
work!
Most of healthcare expenses for my wife and I are out-of-pocket. Rising insurance
premiums forced us to find affordable plan outside of PMH network. I am hopeful PMH
will be in an affordable network soon. We would prefer to use PMH services and have
Dr. Allman as our family physician.
I would like to see an additional pharmacy in Winamac as well as an urgent care.
Really appreciate the clinic in Francesville.
I am pleased that more and more specialists and tests can be done at Pulaski Memorial,
rather than needing to travel to and drive in a city. We have received good care every
time my family and I need it. Keep up the good work!!
Would appreciate having an urgent care facility in the county. It seems if/when I do get
sick, it’s on a weekend. Please try to expand health care services to west side of county
for ease of transportation & time. As our population gets older, Francesville is becoming
a “bedroom/seniors community”. Would appreciate services targeting this age group.
I am a health conscious person. I pay attention to my diet, exercise, etc. We need to
educate parents of our younger generation of proper health care. Stay at home and eat and
be together as a family. Thanks for your work and dedication.

Some type of Urgent Care facility for evenings and weekends when regular physician
hours are over would be great!
Pulaski County needs and Urgent Care Facility.
HealthCare is good in the county and we are expanding. I think it is a good idea to bring
more to the community. Anderson and Allman are great!
The county needs more than 1 grocery store and 1 funeral home. They monopolize it all.
All managers at the hospital should have their salary posted so all the public can see what
they are paid. They are county employees. The hospital works as the good ole boy
system. It’s who you know.
It would be nice to have a rheumatologist on staff at Winamac. Having dealt with RHC, I
thought when you couldn’t get in to see your doctor, it was supposed to be offered who
could see you that day if you are sick, but no offers. Just “no, Doctor is booked, you can
come see him in 2 weeks”. Why not offer which doctor can see you or what NP Is
available. I could be dead in two weeks or healthy.
