Dear Sir, We have to Outsourcing of Laboratory Tests for One Year Period for Shri

Saibaba & Sainath Hospital, Shirdi as per Technical Specification given in enclosed Annexure “A” & the following terms and conditions.

Terms and Condition

01. The name of the Lab, details of tests, rate per test, total cost etc details shall be mentioned in the tender.

02. The Laboratory must quote details of Test Menu with Charges clearly along with coloured brochure. In absence of these details, tender will not be considered.

03. The rates are to be given F.O.R., Shri Saibaba & Sainath Hospital, Shirdi

inclusive of all taxes, transportation etc. No extra amount will be paid for packing, forwarding, transportation etc.

04. The delivery of the Report is to be made at SHRI SAIBABA & SAINATH

HOSPITAL, SHIRDI.

05. Authorized Laboratory should submit latest copy of NABL (Accreditated Lab) if fails then your quotation may liable for rejection.

06. Laboratory should have qualified staff including Techinician & Pathologist for full time. The Report should be duly signed by Pathologist.

07. Laboratory should submit last three years (2015-16, 2016-17, 2017-18) Turnover

more than 50 Lakhs certified by the CA, if fails your quotation may liable for rejection.

08. No escalations, for any reason will be given / allowed in the quoted prices. 09. Substitute or alternative provisions / quotations are not acceptable. 10. In case of any loss of Report prior to the delivery to the Sansthan supplier will

have to replace the same immediately, failing which payment will be stopped until satisfactory replacements.

11. The Report should be delivered within stipulated period, if supplier fails to deliver, the order will be treated as cancelled and your Security Deposit will be fortified.

12. Tests which are done in Laboratory should be quoted rates only. 13. It is not mandatory on Sansthan to provide space for collection. 14. It is mandatory for successful Laboratory to collect the sample within one hour

after call. 15. The Labarotory which will be L1 for maximum test will be selected for services

but in such case if he is not L1 for the remaining tests, it will be mandatory for him to give L1 prices quoted by another unsuccessful Vendor.

16. The successful Labarotory will have to deposit of Rs.50,000/- as a security deposit.

17. No interest will be paid on retention money / security deposit. 18. Late submitted quotation will not be accepted after due date. 19. Payment will be made by the concern patient only, In this Sansthan will not be

responsible for recovery of any bill. 20. The quotation must be clearly mention on the sealed envelops “Quotation for

Outsourcing of Laboratory Test for One Year Period for Shri Saibaba & Sainath Hospital, Shirdi“.

21. The quotation in sealed cover may please be submitted to our Shirdi office on or before Dt. 10/01/2019.
