Introduction

This is the second Provider Data Summary Report, which provides updates on activities completed by

the Office of Provider Development (OPD) in the Division of Developmental Services (DDS) at the

Department of Behavioral Health and Developmental Services (DBHDS). As with the previous report, the

focus is on identifying service development needs based on a review of service authorizations and

developmental disability (DD) waiver population data in each locality in Virginia. The “Baseline

Measurement Tool (BMT),” which is used by OPD in conducting this review, has been updated to include

the most recent six months of data.

Providers are encouraged to review the BMT in conducting market research and in strategic planning

efforts. Provider Data Summary webinars continue on a semiannual basis to provide a forum for sharing

the results of ongoing analysis of the opportunities for DD services development across all regions.

Webinars include a basic overview of findings, provide support on using the data provided, and

encourage the development of business acumen in the DD provider community.

This report is organized in four sections: Introduction, Executive Summary, Key Performance Measures,

Regional Data, and Development Opportunities. The Executive Summary provides updates on various

efforts to support provider development, the Key Performance Measures section focuses on measures

designed to track Virginia’s success in moving to more integrated options, the Regional Data section

provides information specific to each region around availability, and the Development Opportunities

section encourages the exploration of opportunities based on analysis of the Baseline Measurement

Tool (BMT) from June 2018 through April 2019. The BMT provides baseline and subsequent data on

integrated services, collected at six month intervals, across all cities and counties in Virginia. The BMT

also considers the DD Waiver population in each locality including type of waiver and Supports Intensity

Scale© (SIS©) level.

Executive Summary

The Provider Data Summary Report provides a means to track provider development efforts and

communicate changes observed in the DD services system over time. As of April 2019, DBHDS has

enough data to begin evaluating progress across three data points – June 2018, November 2018, and

April 2019. In future reports, a similar method will be followed comparing the past six months with the

original baseline set in June 2018. In this report, data is frequently summarized along these three points,

so that a more meaningful understanding of progress can be achieved. In addition, this report

introduces three measures that are related to the departmental key performance area of Provider

Capacity and Competency. These measures and current targets are established as:

1. Individuals and families on the DD waivers have access to services that support independent living.

20% of Virginia localities have a minimum of two out of four service options supporting individuals with independent living (i.e. in-home support services, independent living supports, supported living, and/or shared living)

2. Individuals and families on the DD waivers have access to Community Engagement (CE) services.

45% of Virginia localities have a minimum of two CE service providers supporting individuals

3. Providers enter Virginia's DD System of supports with an understanding of the rules and requirements affecting them.

15% of all newly licensed providers of DD services complete Provider Readiness Education Program (PREP) training

DBHDS has continued to make progress with various initiatives designed to improve DD waiver provider

capacity in Virginia. The following list highlights the status of Provider Development activities since the

last report:

Provider Roundtables and Regional Support Coordination Meetings were held in all regions in

January 2019. Roundtables were attended by a total of 347 provider representatives and SC

Meetings by 70 Support Coordination representatives. These meetings serve as a forum to exchange

information about topics impacting providers and support coordination, as well as provide space for

shared learning.

Provider Development Activities in the third quarter of FY19 included Community Resource Consultants meeting with 10 providers seeking to diversify or expand services. Seven of the 10 visits were on site at the provider organization and included considerations of Skilled Nursing, Supported Living, Community Guide, Community Engagement, Community Coaching, Independent Living Supports, Employment and Community Transportation, Group Home, and In-home supports. Three barriers to expansion were noted: funding issues, staffing issues, and the lack of regulations and a policy manual. These providers were looking to expand into the following areas:

Alleghany Highlands Augusta County Charlottesville and surrounding counties Mathews Portsmouth

Richmond Rockingham County Virginia Beach Western and Southwestern Regions


In the fourth quarter of FY19, Community Resource Consultants met with 8 providers (19 participants) seeking to diversify or expand services. Services considered in these meeting included: Community Engagement, Group Home with ASL trained staff, Supported Employment, Workplace Assistance, Group Home (mergers and acquisitions), Supported Living, In-home Support Services, Independent Living Supports, Community Coaching, Therapeutic Consultation, and a shift from consideration of 8 bed group home to more integrated model. Barriers were noted as related to Licensing wait, staffing, initial capital, workforce availability, contract consultation needed, and a lack of CARF accreditation. Areas of expansion considered included: Augusta County Harrisonburg Rockingham Bedford Huddleston Moneta Franklin County Portsmouth

Spotsylvania Fredericksburg King George Caroline Stafford Prince William

The Jump-Start Funding Program has received numerous inquiries, and four applications; two of

which were awarded funds for Independent Living and Supported Living options in the Central and

Western Regions of Virginia. Funds continue to be available to assist providers with service

expansion into underserved localities in all regions. To date, estimates are that 20 people have

accessed Supported and Independent Living Services through Jump-Start Funding.

Virginia has completed its first year as part of the Business Acumen Learning Collaborative with the

HCBS Business Acumen Center (http://www.hcbsbusinessacumen.org/). The focus of the project is

supporting providers to develop skills and competencies in financial acumen, market orientation,

and strategic perspectives so that business acumen is built, expanded, and maintained among the

provider network in Virginia, particularly those waiting to be licensed.

Held in March 2019, DBHDS hosted a Provider Innovation Collaborative that had 400 attendees and

a focus on best practices. The event included speakers from Virginia and other states, including

some speakers that were known nationally, who provided information across four themes: Quality

Improvement, Best Practices, Supporting Crisis and Business Acumen. The event was well received

and is being considered again in 2020. Slide presentations can be viewed at

The Provider Readiness Education Program (PREP) is designed to support a network of quality DD

providers in Virginia. It enables potential providers to increase their understanding of Virginia’s

expectations and requirements, and to give them the tools they need to be successful. PREP is based

on a mixed method approach and includes online training, followed by an in person session, and

access to additional resources to support their learning. The program was initiated in June 2019 and

will continue on a semi-annual basis. Priority is given to providers who have been in queue for a

license for at least six months or who have been licensed with the past 12 months. There were 106

providers invited in June, 50 of them registered, and 38 completed the process. Initial data indicates

that 96% of attendees agree that their knowledge and skills have increased as a result.

Next steps for Provider Development include:

finalizing an online provider database,

participation in a community of practice initiative around Charting the Lifecourse©,

exploring how to identify providers with capacity to support complex needs, and

centralizing access to training resources

As we move closer to having regulations and a Community Services Manual for the DD waivers, we will

also focus on updating DDS guidance materials related to the provision of DD waiver services. The

following areas of this report Key Performance Measures, Regional Data, and Development

Opportunities provide visualizations of data derived from the BMT. In particular, DBHDS is requesting

that providers carefully review the Development Opportunities section as this section provides some

indication of where an increased need for development may exist. If you have questions about pursuing

Key Performance Measures

The following chart represents gains in relation to two measures – one for integrated living and

one for meaningful day (figure 1). These measures are used by DBHDS in tracking the increase

of related services across regions. The measures, current status, and June 2018 baseline data

are included below.

Regional contributions to the Integrated Living Measure described on page five are delineated

in the chart below. Increases are noted in Regions 1 and 3 with the remaining regions remaining

consistent with the number of localities meeting the measure (figure 2). The second chart

shows that regions 2, 4 and 5 increased localities meeting the Meaningful Day measure (figure

3).

Regional Data

At the regional level, there are noted a majority of small to modest increases in the number of

providers per region. The follow charts detail changes in each of the developmental services

regions.

Additional information is provided related to services having fewer than three providers and

localities where no integrated services are authorized. For comparison, the current provider

count in the former and the total number of people with DD Waiver are provided in the latter.

This report considers services across the five developmental service regions (figure 4).

Development Opportunities

Information provided in this section relates to possible opportunities for program development.

Providers are encouraged to use this information along with the Baseline Measurement Tool in

conducting market research and during strategic planning discussions. DBHDS is ready to assist

providers who are interested in adding more integrated models of service to the options they

currently provide to people in their services and communities.

The following chart is provided to assist with projecting future community needs. It shows the

FY20 slot distributions by waiver type and slot count per region. It can be seen that in all

regions the Family and Individual Supports Waiver slots outpace the Community Living Slots by

a wide margin. This shift in slot availability increases the need for services available under that

waiver.

The graphic below illustrates the division of FY20 slots across regions. By and large, slots for the

Family and Individual Supports Waiver (FIS) far outpace the creation of slots in the other two

DD waivers (figure 5). Since the FIS waiver contains options that do not include congregate-

based residential supports, providers may consider the more integrated models that will be

needed to support people moving into the FIS waiver such as In-home Support Services, Shared

Living, and Supported Living services.

Group Home Data

People with support needs typically associated with SIS© Levels 1 & 2 might find more

independence and satisfaction in independent housing. The graphic below provides the count

and total percentage of people based on regional DD waiver totals at SIS© Level 1 and 2 who

are currently residing in group home settings (figure 6). Support Coordinators are encouraged

to discuss more integrated housing options with people at these levels. Further, providers

considering expansion could diversify their service options through the addition of In-home

Support Services and/or Supported Living to support these individuals, as well as others moving

into the waivers.

Levels 1 & 2 are defined as:

SIS© Level 1 Mild Support Needs Individuals have some need for support, including little to no support need for medical and behavioral challenges. They can manage many aspects of their lives independently or with little assistance. SIS© Level 2 Moderate Support Needs Individuals have modest or moderate support needs, but little to no need for medical and behavioral supports. They need more support than those in Level 1, but may have minimal needs in some life areas.

Positive Behavioral Support Providers

Behavioral interventionist services are available in all regions. The following chart illustrates the

number of active Positive Behavioral Support Facilitators (PBSF) providers in each region (figure

7). The second number “OAO” (i.e. own agency only) show how many of the total only support

people within their own agency. SIS© Levels 3 & 4 include behavioral support needs as

described in the following definitions:

SIS© Level 3 Mild/Moderate Support Needs with Some Behavioral Support Needs Individuals have little to moderate support needs as in Levels 1 and 2. They also have an increased, but not significant, support needed due to behavioral challenges. SIS© Level 4 Intensive Behavioral Support Needs Individuals have intensive behavioral challenges, regardless of their support needs to complete daily activities or for medical conditions. These adults typically need significantly enhanced supports due to behavior.
