Introduction to Prosthodontics III

 *Complete denture fabrication steps:

-How many basic minimum clinical steps are required to fabricate a C.D

without a recall?

- 5 clinical steps, then we have a recall and 4 laboratory steps

*Clinic 1:

1-History taking: the collected data about individual, family,

environmental factors (including medical/dental history) and any other

information that may be useful in analyzing and diagnosing conditions or for instructional purposes

2- Examination.

3-Treatment planning.

4-Primary Impressions…using impression compound in stock tray

*Lab1:

-Primary casts…made from gypsum type II (dental plaster)

-Custom trays: an individualized impression tray made from a cast

recovered from a preliminary impression. It is used in making a final

impression

-Secondary/master/final impressions: the impression that represents the

completion of the registration of the surface or object

-secondary impression is taken by zinc oxide eugenol

*Lab 2:

-Secondary/master/final casts: a replica of the tooth surfaces, residual

ridge areas, and/or other parts of the dental arch and/or facial structures

used to fabricate a dental restoration or prosthesis….then we fabricate

record base (record block).

-Record base: base plate and wax rim

-base plates: a rigid, relatively thin layer of wax, shellac, or

thermoplastic (heat activated polymer) or chemical activated polymer,

Or photo- activated polymer adapted over edentulous surfaces of a

definitive cast to form a base which, together with an attached occlusalrim made of wax or similar material, serves as the record base

-Wax rims: occluding surfaces fabricated on interim or final denture

bases for the purpose of making maxillomandibular relation records and

arranging teeth — called also record rim

*Clinic 3:

-Jaw Registration stage: a registration of any positional relationship of

the mandible relative to the maxillae.

-These records may be made at any vertical, horizontal, or lateral

orientation

-we record these relations according to the following order:

1. Aesthetics (maxillary wax rim): the effect produced by a dental

prosthesis that affects the beauty and attractiveness of the person…the

first factor we depend on during registration of the relation between themaxilla and the mandible is the esthetics

-we should have enough labial fullness, incisal show and buccal corridor

 Nasolabial angle

should be 90 degree

Incisal show should be

1-2 mm at rest position

- Once we make the (inter pupillary line & the camper's plane & theocclusal plane) once they are parallel to each other and we are satisfied

that the upper rim is in the right position. There are certain checks that wecan do: the phonetic check & the anatomic check . The phonetic check

such as the "S" sound, "F" & "V" sounds.

-Once we finish the maxillary rim we need to mark certain lines which

are the midline of the patient's face & the canine lines. In order to knowwhere to put the teeth & the size of the teeth

2. Vertical (VD) & horizontal (RCP) relations.

>>> What happens if we change the vertical dimension or we didn'tgive the patient the right vertical dimension and give him too high

vertical dimension??

>>> What will happen if we do the opposite (give the patient a small

vertical dimension)??

*Lab3:

-Mounting on articulator: the laboratory procedure of attaching a cast toan articulator

-Dental Articulator: a mechanical instrument that represents the

temporo-mandibular joints and jaws, to which maxillary and mandibular

casts may be attached to simulate some or all mandibular movements

-Teeth Arrangement (Tooth Setting): the placement of teeth on a

denture with definite objectives in mind or can be defined as the placement of teeth on trial bases

*Clinic 4:

-Try in: the process of placing a trial denture in the patient’s mouth for

evaluation of a preliminary arrangement of denture teeth that has been prepared for placement into the patient’s mouth also used to evaluateesthetics and maxillomandibular relationships

-during this step we shortly repeat all the steps of JRR (Repetition of

previous visit: JRR) also at the end of this visit we determine the post

dam area.

-Post dam determination: the seal area at the posterior border of amaxillary removable dental prosthesis or the soft tissue area at or beyond

the junction of the hard and soft palates on which pressure, within

physiologic limits, can be applied by a complete RDP to aid in its

retention.

*Lab 4:-Denture processing: that means by which the denture base materials are

polymerized to the form of a denture or the conversion of the wax pattern

of a denture or a portion of a denture into resin or other material

-Flasking: the act of investing in a flask or the process of investing thecast and a wax replica of the desired form in a flask preparatory to

converting the restorative material into the desired product.

-Dewaxing: the act of removing the wax pattern in order to put acrylic

instead to get the final denture

-Packing: the act of pressing a denture base material into a mold within a

refractory flask.
