II. BENEFITS AND ASSISTANCE (HOSPITALIZATION,
FUNERAL AND DEATH BENEFITS)
Members in good standing of the League can avail of the benefits (A member in good standing is one
who has paid his term dues and membership fee)

HOSPITALIZATION BENEFIT ( Maximum of P15,000)
• Member of Good Standing• Accomplished Hospitalization Benefit Application
Form duly signed by the PFP/CCP• Medical Certificate• Photo Copy of Hospital Bill and• Officials Receipts of the expenses incurred during
confinement/hospitalization (i.e. room and board, laboratory, professional fees, medicines, etc.)

FUNERAL BENEFIT – P10,000
• Will be released immediately upon receipt of the Notice of Death signed by the Provincial Federation President or City Chapter President.

DEATH BENEFIT - P20,000• Will be released to the legal heir upon completion of
the death application form and supporting documents
• Member of Good Standing• Accomplished Death Benefit Application Form duly
signed by the PFP/CCP • Death Certificate of the deceased member• Marriage Contract in case the spouse is the legal
heir/Birth Certificate in case the child is the legal heir

Summary of Minutes• The group discussed several suggestions for submission of
letter-proposals re PCL Partnership. • There was a suggestion to put up Academic Scholarship for
sons/daughters of Councilors.• The group talked about the status of PCL Advocacy- GSIS,
Eligibility and Step Increment. For the next NEO NB in December, it was suggested to invite GSIS, Civil Service and Senate Pres/Speaker of the House to shed light on the matter.
• There was a suggestion that should Private Insurance Partnership will push through, a graduate councilor may have option to continue on his personal capacity.
• Suggestions re NEO NB in General Santos to be submitted to the National Board for action.
