PLASTIC BREAST SURGERY FOR women is often a misunderstood proce-dure that can be performed for various reasons, both functionally and cosmeti-cally. No matter the reason, a woman’s de-cision to have plastic breast surgery may cause her stress: physically, emotionally, and financially. However, this stress can be lessened if she is presented with the right information, including a description of when a procedure is necessary, symptoms that may hint towards surgery, how she may obtain insurance coverage, and what she can expect going through the surgery. With these pieces of information, a wom-an can be comforted by her knowledge of what to expect in her journey through plastic breast surgery.

There are two types of plastic breast sur-gery: the functional procedure and the cosmetic procedure. Functional breast surgery will typically be covered by an insurance carrier due to the condition’s symptoms that prove to be detrimental to a woman’s health. Cosmetic breast surgery only improves the contour or the size of the breast. Because it is not performed to improve one’s health, this procedure would not be covered by an insurance carrier. The person inquiring about breast reduc-tion surgery is usually referred by a friend or her primary care doctor, as it is consid-ered functional breast surgery. This type of patient has been large breasted since her teenage years, and as a young adult

When Insurance Covers

Breast Reductionhas begun to notice symptoms that she at-tributes to her size. She has found it dif-ficult to be active in sports and tends to wear certain clothing to conceal her size. As she moves into child bearing years, her symptoms usually worsen due to further breast enlargement during pregnancy. The most common symptoms, in order of frequency, are as follows: • Chest wall heaviness • Neck strain • Bra strap irritation • Upper or lower back pain • Poor posture • Tingling or numbness of the fingers

Most candidates for this procedure begin as a D cup or higher and wish to be re-duced to a C cup or smaller. To combat the symptoms, the patient may have tried a course of treatments, designed to reduce the discomfort. Most commonly: • Chiropractic care • Physical therapy: massage, heat treat-ments, muscle stretching exercises • Oral medications: muscle relaxers, pain medication, anti-inflammatory drugs

If, after a trial of these conservative treat-ments, there is no improvement in their symptoms, she should be referred for a consultation regarding a breast reduc-tion procedure. At the initial consulta-tion, particular attention is focused on the symptoms, the conservative methods that

may have been used to reduce them and her height, weight and cup size. Additionally, an estimate is made regarding the amount of breast tissue that should be removed in order to reduce her breast weight. This information is forwarded to the pa-tient’s insurance carrier and a decision is made relative to coverage. The most impor-tant factors in attaining coverage, in order of importance, are as follows: • The estimate describing the amount of reduction from the procedure (most patients will go from a D or DD cup to a C cup) • A detailed list of the symptoms present and their duration • Any records of a non-successful conser-vative treatment period • Documentation regarding her condition and her attempt to reduce symptoms from her primary care doctor

In the 2,000 breast reduction cases that I have performed, when all of these pieces of information are submitted, the patient has a 75 to 85 percent chance that her breast re-duction surgery will be covered. The two and a half hour procedure can then be scheduled.

The expected outcome after plastic breast surgery will include: • Overnight stay in the hospital • Pain level roughly between two and five on a scale of one to ten • A return to work within around two to four weeks (dependent upon the patient’s type of employment) • Anticipation of partial or full return of nipple sensation and ability to breast feed in 90 percent of my patients • The breast shape lifted as well as reduced

During my 30 years as a plastic surgeon, the breast reduction procedure has been by far the most appreciated by my patients. I at-tribute this to the fact that they not only feel better with the reduction of symptoms, but they also have an improved appearance. The most frequent statement that we hear after the procedure is: “I should have done this years ago.”

Over 30 million Americans are affected by varicose veins yet do not seek treatment due to the belief that the treatment of varicose veins is often seen as simply cosmetic, but this is not so. When varicose veins are left untreated, symptoms such as pain, swelling and fatigue may worsen. In the past, “stripping” - a process of bluntly remov-ing the vein with a long wire- was a surgical procedure to remove painful, enlarged vari-cose veins. Fortunately “strip-ping” has been made obsolete by new and advanced surgical methods.

Today’s TreatmentI believe the best way to treat varicose veins currently is to use radio frequency ablation or heat destruction. Radio frequency ablation can be per-formed as an outpatient proce-dure with a simple, local anes-thetic. It is a minimally invasive procedure that takes typically under an hour. In the proce-dure, controlled heat is deliv-ered to the vein wall so that the collagen in the wall shrinks and the vein closes. This forces the blood to re-route itself to healthy veins. The new ad-vancement results in a much better patient experience with much less pain, bruising and a quicker recovery – typically only a few days. In fact, most patients report feeling little, if

any, pain during and after the procedure. Patients can go on and start living a life free of the pain of varicose veins.

Qualified CandidatesThe first step in deciding if you are a qualified candidate for this treatment is testing

whether or not you have Ve-nous Reflux Disease. Venous Reflux Disease develops when healthy leg veins become dys-functional and cannot prop-erly assist blood back to the heart. When left untreated, varicose veins can lead to

symptoms such as restless legs, heaviness and fatigue, leg or ankle swelling, pain, cramping, burning or itching of the skin, skin changes and ulcers. Other factors that may play a role in whether or not you are sus-ceptible to varicose veins can include age, obesity, multiple pregnancies, gender, heavy lift-ing, family history and stand-ing profession. To determine if venous reflux is present, an ultrasound scan is required. Most insurance providers and Medicare will cover this vari-cose vein treatment if the pa-tient has been diagnosed with venous reflux.

Get a physician’s clearance. This should be the first thing you do, especially if you have any health or medical issues.

Hire a trainer. A certified trainer that specializes in long distance running (whether for general health/fitness or training for races) will assess your body mechanics, muscle imbal-ances, flexibility and core strength. All of these factors affect your running efficiency. Improper strength, flexibility, core strength and posture can lead to injuries.

Do not run every day. Even advanced runners often make the mistake of not going every other day. Use your days off for cross train-ing (biking, elliptical) and completing balance, core and resistance exercises.

Find your perfect running shoes. Everyone’s feet and gait are different, so it is important to find a pair that gives you enough support. Try going to a specialty shop or asking your trainer for assistance.

Increase your distance properly. A typical guideline is to increase no more than 10 percent of your mileage from one week to the next. For example, if you run a total of 12 miles one week, you will add 1.2 miles for a total of 13.2 miles the next.

Don’t just run. Incorporate foam rolling, flexibility, balance, core and strength training. Your trainer can help you custom-ize a routine that is suitable for your body and goals.

Use the run/walk method. Based on your 1 mile time, your “run” day can be a combination of running and walking. For example, run 3 minutes followed by a 1 minute walk and continue the pattern for a specific time du-ration (ex: 30 minutes.) This will help to stave off injury.

Aim to just complete it. Once you have completed your first half or full mara-thon, then become more specific with your goal when it comes to timing.
