TRuE OR FAlSE?

Adhesive dentistry has not fully replaced traditional mechanically retained dentistry for many reasons. lack of trust in adhesives, perceived difficulty, and “It isn’t what we were taught in dental school” are commonly heard. Now that we have clinically proven methods to strongly adhere not only to enamel, but in fact to dentine and all restorative dental materials, there are many compelling reasons to stop doing “hack and pack”, tooth destructive dentistry. Replacing it with tooth conserving adhesive dentistry is usually easier and always far more enjoyable to provide.

This is a fast paced presentation. Detailed procedure notes will be provided, optionally in advance by request. Extensive scientific references will be cited and a summary of these provided with the procedure notes.

• Bonding agents are getting better but some perform better than others. • Posterior composite placement technique for long term success • Resin modified GIC base under composite accelerates failure • Immediate dentine sealing “IDS” has many compelling benefits • Eliminate the destructive action of MMP’s on the dentine bond • H and H stringless VPS impressions, even in a bloody field • Upper and lower impression and bite registration in about 3 minutes • Bonding of metal and ceramic, side-by-side, st rong and fast • Protecting cusps without crowns • Greatly reduce the need for endo after crown

lONG TERM SuCCESS WITh POSTERIOR COMPOSITES • No post-op sensitivity• understanding polymerization shrinkage is the key• Eliminate the primary source of recurrent caries• The best base is “no base”, proven by clinical trial

hyDRAulIC AND hyDROPhOBIC IMPRESSIONS• Stringless, subgingival vPS impressions • Impression displaces blood • Triple tray gets opposing arch and bite too• ultra accurate, rapid procedure

SIlJET FOR uNIvERSAl BONDING• Bond all ceramics, even zirconia, with no hF etching• Bond all metals, no worries about which metal• Ideal for metal and porcelain repairs• What else you need: a Microetcher

OPTIMIzING CAD-CAM• IDS - “immediate dentine sealing” enhances success• The non-crown reduces need for endodontics• Bathtub prep design for short teeth• Temps for ideal “flat” preps

PRE-TEST yOuR KNOWlEDGE

1. The best way to prevent bite pressure sensitivity when placing posterior composites is to line the dentine with resin-modified GIC.

2. low polymerization shrinkage of composites enhances clinical success.3. Etch and rinse (aka. “total etch”) adhesives remain the “gold standard”. 4. vPS impressions must be made in a dry field. 5. Separate arch impressions (and a bite registration) produce the best accuracy of occlusion. 6. Silica based ceramics must be hF etched and have silane applied for the best bond durability. 7. Bonding metal and ceramic side-by-side is problematic, requiring different treatment of the

metal and ceramic. 8. Immediate dentine sealing reduces sensitivity, with little other benefit. 9. vMK’s are still the best choice for cracked teeth. 10. Sub gingival margins are the most ideal for aesthetics.
