What is the Central Equipment Pool for High Technology Wheelchairs (CEP)? (Section 6)
• CEP provides ADP clients throughout Ontario with high quality new and recycled power wheelchairs with power tilt/recline units at discounted prices and provides a rebate on the client's costs once the equipment is returned to the pool.
• For the purpose of ADP funding assistance, a high technology wheelchair is defined as a power wheelchair and/or power base that includes a power dynamic positioning device – tilt and/or recline – as a component
• In addition to the opportunity to purchase recycled equipment, a consistent level of service to ADP clients across the province has been established by consolidating all high-technology services in one responsibility centre.

Recycling
• Bases and power dynamic positioning devices only are recycled
• Only items in good condition are considered for recycling
• Recycled items are refurbished—staff take pride in ensuring they look and operate like new
• Carry the same 3 year warranty as new components
• Typically these components are less expensive than new components
• If 3rd party funding is an issue, recycled equipment can also be specifically funded. CEP will work with the authorizer to provide options where possible.

Only the power dynamic positioning component (tilt, recline) needs to go through the pool, other vendors can

Accessing CEP
• The local CEP representative is the primary point of contact for the client and authorizer.
• The client, Authorizer and CEP representative work together to determine the specifications for the CEP chair
• Detailed/accurate and complete the requests result in faster turn around times, and accurate builds
• Common pitfalls include• Omitting key measurements (e.g. depth)
• CEP reviews the request and determines whether the required items are in stock. Requests are filled with recycled equipment first

Accessing CEP (continued)
• Typically a trial of the equipment occurs to determine the final prescription
• The chair is returned to CEP while awaiting funding
• The chair the client had on trial is dispensed to the client once funding has been received
• As per ADP policy, the authorizer is responsible to follow up

Assistive Devices Program Central Equipment Pool
For High Technology Wheelchairs
CLIENT CONTRACT Dear ADP Client, The Assistive Devices Program (ADP) has established the Central Equipment Pool (CEP) for High Technology Wheelchairs to provide clients in Ontario with power dynamic seating systems. This program has been established to ensure responsible management of ADP’s financial resources, and to provide a consistently high level of service throughout Ontario. Shoppers Home Health Care presently administers the program on behalf of the Assistive Devices Program throughout Ontario. Upon approval for funding by ADP, equipment will be provided to you for your use as long as you require it. This equipment is supplied according to the clinical specifications detailed on the ADP authorizer prescription form, which outlines your basic and essential needs. Although the equipment is provided for your use as long as you require it, ownership of the major components is retained by ADP. This differs from most other ADP programs. Once the equipment is no longer required, it must be returned to the Central Equipment Pool, administered through your local Shoppers Home Health Care location. You are required to pay a client portion in most cases, which is a user fee for the use of the equipment. Upon return of the recyclable equipment to the CEP, a certain portion of your user fee may be refunded to you. The amount refunded will be based upon the length of time the unit was used, and the overall condition. For this reason, you are encouraged to keep this equipment in good working condition. As stated, when the equipment is no longer required, it must be returned to the CEP. Arrangements for pick-up can be made by calling 1-800-395-6661, or by contacting your local Shoppers Home Health Care. Seating and upholstered products are not recycled, and therefore do not need to be returned to the CEP. All service work, modifications, and additional components for this equipment must be supplied through the Central Equipment Pool (presently operated by Shoppers Home Health Care). You may contact your local Shoppers Home Health Care store or the
