1. Principal, DG Khan Medical College/ Hospital for the Scheme

Provision of Missing Specialties for Up-Gradation of DHQ Hospital into

Teaching Hospital; intends to prequalify the firms for “Supply,

Installation & Commissioning of Angiography Machine as turn Key

Project” during financial year 2019-20. The manufactures having their

Sole Agents with established credentials in terms of technical,

financial & managerial capabilities are eligible to apply.

2. A complete set of Prequalification Documents in English can be

purchased from the office of the undersigned, on the submission of

written application and copy of CNIC by paying Rs.1,000/- (non-

refundable). These can also be downloaded from the website

[www.ppra.punjab.gov.pk]&[www.dgkmc.edu.pk].

3. The applications along with all required documents should be

submitted in the office of under signed till 24.03.2020 till 11.00am

which shall be opened on the same date at 11.30am.

4. The complete Application shall be in the proper binding state; loose

papers shall not be acceptable.

5. After concluding the prequalification Process, the Technical &

Financial Bids shall be called from the Prequalified Firms only.

6. The process shall be governed by the Punjab Procurement Rules

2014 (amended). False, Fabricated or materially incorrect

information; if found at any stage will lead to disqualification under

rule 18 of PPRA

2014.

FOR UP-GRADATION OF DHQ HOSPITAL INTO TEACHING HOSPITAL”for the

YEAR 2019-20.

1.2 A complete set of Prequalification Documents in English can be

purchased by the Manufacturer or Sole Agents on its behalf; from the

office of the undersigned, on the submission of written application and

copy of CNIC by paying Rs.1,000/- (non-refundable).

materialtotheinvestigationormakingfalsestatementstoinvestig

atorsinordertomateriallyimpedeaBankinvestigationintoalleg

ationsofacorrupt,fraudulent,coerciveorcollusivepractice;and

/orthreatening,harassingorintimidatinganypartytopreventitfr

omdisclosingitsknowledgeofmattersrelevanttotheinvestigati

onorfrompursuingtheinvestigation;or

(b) theProcuringAgencywillrejectaproposalforawardifitdeterminesth

atthebidderrecommendedforawardhas,directlyorthroughanagent,en

gagedincorrupt,fraudulent,collusive,coerciveorobstructivepracticesinc

ompetingforthecontractinquestion;

(c) theProcuringAgencywillsanctionafirmorindividual,includingdecla

ringineligible,eitherindefinitelyorforastatedperiodoftime,tobeawardeda

contractifit,atanytime,determinesthatthefirmhas,directlyorthroughana

gent,engagedincorrupt,fraudulent,collusive,coerciveorobstructiveprac

ticesincompetingfor,or

(d) Procuring Agency will have the right to require that a provision

be included in bidding documents requiring bidders, suppliers and

manufacturers and their agents to permit the Procuring Agency to

inspect their accounts and records and other documents relating to

the bid submission and contract performance and to have them

audited by auditors appointed by the Purchaser;

3.EligibleAppli

cants

3.1 AnApplicantmust be the manufacturer of the Angiography machine

having its Sole Agent/ Sole Distributor in Pakistan for the last three years.

3.2 Manufacturersofacountrymaybeexcluded, ifasamatteroflaw or officialregulation,theGovernmentofPakistanprohibitscommercialrelationswith thatcountry or for other reasons.

3.3 Afirm/

manufacturerdeclareddisqualified/blacklistedbyanyofthe private/

publicsectororganizationinPakistanshallbeineligibletobidforacontractdu

ringtheperiodofembargo.

3.4

ApplicantsandallpartiesconstitutingtheApplicantshallnothavea

conflictofinterest.Applicantsshallbeconsideredtohaveaconflictofinter

est,iftheyparticipatedasaconsultantinthepreparationofthetechnicals

pecificationsofthegoodsthatarethesubjectofthisprequalification.Wher

eafirm,orafirmfromthesameeconomicorfinancialgroup,inadditiontoc

onsulting,alsohasthecapabilitytomanufactureorsupplygoodsortoconstr

uctworks,thatfirm,orafirmfromthesameeconomicorfinancialgroup,cann

otnormallybeasupplierofgoodsorworks,ifitprovidedconsultingservicesfor

thecontractcorrespondingtothisprequalification,unlessitcanbedemonstr

atedthatthereisnotasignificantdegreeofcommonownership,influence

orcontrol.

bleorliableforthosecosts,regardlessoftheconductoroutcomeoftheprequ

Theapplicationaswellasallcorrespondenceanddocumentsrelati

ngtotheprequalificationexchangedbytheApplicantandtheProcuringA

gency,shallbewritteninthe English

language.Supportingdocumentsandprintedliteraturethatarepartofthe

applicationmaybeinanotherlanguage,providedtheyareaccompanied

byanaccuratetranslationoftherelevantpassagesinthe English

language,inwhichcase,forpurposesofinterpretationoftheapplication,th

etranslationshallgovern. All such documents should be signed and

stamped by the applicant.

24.2 The Prequalification thus issued (addressed to the manufacturer

as well as local partner), shall include the name of the manufacturer

(brand) along with origin and Sole Agent.

24.3 This Prequalification shall be valid for 02-years.

25.Invitationt

oBid

25.1

Afternotificationoftheresultoftheprequalification,theProcuringA

gencyshallinitiatethe further procurementprocess and issue the Bidding

Documents to the pre-qualified firms.

25.2 The prequalified firms shall bid according to their

prequalification award.

26.Arbitration 26.1 The Arbitrator will be appointed by mutual consent of the

Parties. The decision of the Arbitrator will be final and binding on the

applicant applying for prequalification.

QUALIFICATIONCRITERIAANDREQUIREMENTS

Introduction

The prequalification shall be conducted for the manufacturers and their Sole Agents of

Angiography machinewith allied accessories as a Turn key Project.

The firm should be a manufactureror its Sole Agent. The manufacturers should have

been installing their unit’sworld widely. The complete installation and testing/

commissioning shall be conducted by the professional technical staff of the Sole

Agent/ manufacturers’ but final testing, commissioning shall be conducted by the

manufacturer itself. The installation is purely a technical/ engineering job and would

be mandatory for itsSole Agentsto be registered with Pakistan Engineering Council

(PEC) of Pakistan.

The Sole Agent should have import documentation from the concerned authority to

import the equipment in Pakistan (DRAP License for Medical Device Import) as well as

from PNRA for importing and installation of the radiation equipment.

A completed applicationneeds to be submitted covering following information with

documentary evidence;

Details of Manufacturing Sites.

Certification to provide evidence for Conformity and Compliance with the

standards for Quality.

Current /previous Angiography projects with name of the country and hospital.

Signed Confirmation of availability of Spare Parts of Product to be prequalified for

at least 10 years.

Confirmation that the Sole Agenthas requisite technical personnel, tools and test

equipment to install, test and commission this equipment.

Company Profile of both the Manufacturer and its Sole Agent.

Litigation history; if any for the Manufacturer and Sole Agent.

Requirements of Sole Agent (Mandatory)

List of available concerned special Tools/ Test/calibration for these two machines.

List of graduate qualified engineers trained on Angiography machine by the

manufacturer in their training centers (At least Two).

Import License for Medical Devices by DRAP to import these machines.

Import& installation License for Radiation Devices by PNRA.

Above stated specific and general requirements / information to be provided by the

Sole Agent/ Manufacturer. Physical Inspection of Sole Agent/ Local Partner (if required)

will cover all aspect where document-based evidence is not sufficient.

the Sole Agent in Pakistan; if required.
