If members that fail to obtain pre-authorization prior to all services listed above (except for emergency medical evacuation),

evacuation performed without Pre-Authorization will be subject to an Insured Person co-payment liability of 100%, with no

out-of-pocket limit.

If there is any insurance coverage change of the family members in the middle of the policy year (such as new marriage,
