NOW….A Change in the process

§ As  of  May  2015,  the  state  of  Ohio  (OHSAA)  is  now  using  an  electronic  system  to  start  the  physical  process;  “PrivIT  e-­‐PPE”  

 § It  is  REQUIRED  per  Adena  Sports  Medicine.        

Online Physical Form - PrivIT

§  The  new  process  includes:  §  Registering  for  PrivIT  account  online  §  Filling  out  personal  informaQon  and  health  history  online  

§  Joining  your  “teams”  online  §  PrinQng  off  forms  and  taking  them  to  your  physical  exam  §  Mass  physical  date  OR  individual  physician    

§  CompleQng  all  forms  and  returning  them  to  your  school  

Benefits of PrivIT

§  Each  year,  you  will  only  have  to  update  your  informaQon  §  LESS  WORK!!  

§  If  you  have  siblings  in  the  same  school,  you  can  copy  your  family  history  §  LESS  WORK!!  

§  At  your  physical  exam  the  doctor  will  be  be\er  able  to  focus  on  relevant/current  health  issues  and  READ  the  form  §  LESS  TIME!!  

§  Your  coaches  will  be  able  to  view  your  clearance  status  easily  §  NO  DIGGING  AROUND  FOR  PAPERWORK!!

And That’s It!

§ Once  your  completed  paperwork  has  been  processed,  you  can  view  your  “Cleared  for  Sports  Status”  on  your  PrivIT  account  

§ You  may  begin  parQcipaQon  in  your  sport/acQvity  ONLY  when  your  status  is  “Cleared”  § This  includes  organized  school  pracQces,  games  and  compeQQons
