    2. JOB PURPOSE

    The practitioner works in partnership with the rheumatology nurse praactitioner team, rheumatology consultants and the multidisciplinary team in the provision of a specialist rheumatology service to patients within Lothian.

    The practitioner will provide clinic based services independently including general rheumatology/ joint injection/ drug counselling/ DMARD & Biologics review/ support the telephone advice virtual clinics and help line.

    The practitioner as part of the specialist team uses audit to ensure that the quality of the rheumatology service is monitored.

    Will assist in providing specialist education, advice and training programmes for patients and other professional groups.

    3. DIMENSIONS

    The post holder in consultation with the Rheumatology Consultant Group and other Rheumatology Nurse Practitioners has clinical and management responsibility at an agreed level for the Lothian Region caseload of patients within the rheumatology speciality.

    The post holder is not responsible for managing the budget but needs to be aware of the resources available and the need to work within the financial envelope.

    5. ROLE OF DEPARTMENT

    The Lothian University Hospitals offers a diversity of first class patient services to a population of 850,000 in the City of Edinburgh, East, Mid and West Lothian. The Division promotes the highest standards of patient care and management. The Rheumatology service is based at the Western General Hospital with Ward, out-patients department, day unit and offices. There are also out-patients clinics at St Johns Hospital at Howden, Livingston and Roodlands Hospital where there is also secretarial support. The service provides specialist medical treatment and long term review of patients with chronic rheumatic diseases to the population of Lothian and as a tertiary referral centre for patients throughout Scotland.

    6. KEY RESULT AREAS

    Clinical

    Work with the specialist and multidisciplinary team to provide specialist clinical care to out patients within the rheumatology setting.

    Undertake clinical assessments for Rheumatology patients according to agreed protocols, which includes ensuring baseline bloods are satisfactory prior to commencing drug therapy.

    Accept responsibility and accountability for clinical decision- making within the parameters agreed with the advanced nurse specialists and rheumatology consultants.

    Discuss relevant changes in clinical condition with consultant rheumatologists.

    Effectively instruct and counsel patients and families about their disease, its management and relevant health problems.

    Refer patients to other members of the musculoskeletal team as appropriate.

    Provide Independent Clinics in general Rheumatology/ intra-articular joint injection/ DMARD & Biologic therapies.

    Via the Rheumatology Helpline, use specialist knowledge and clinical judgement to provide support and advice to patients, carers, the multidisciplinary team, G.Ps and primary care nurses. Identify those requiring earlier review.

    Act as a resource of specialist knowledge and clinical expertise to ensure care delivered to patient is optimum and evidence based.

    Promote and advise on health and lifestyle activities for healthcare professionals and general public when requested.

    Responsible for the development, implementation and evaluation of the teaching programme of Self administration of sub-cutaneous Methotrexate injections for patients.

    Accept responsibility and accountability for monitoring blood results prior to re-ordering Methotrexate injections and ensuring that appropriate action is taken for any abnormalities detected.

    Be available for work at the three sites and others should this be required as skills develop.
